Clinical observations of correction of square jaw in East Asian individuals.

OBJECTIVE: A square face along with a variety of facial features is considered to be unappealing in the East Asian population. Thus, the objective of this study was to assess the facial features and the various operative techniques available for surgical correction of a square jaw via the intraoral approach. STUDY DESIGN: All surgical procedures were performed via the intraoral approach. Surgical approaches such as curved mandibular ostectomy, outer cortex splitting ostectomy, "V-Line" ostectomy, and modified genioplasty were performed according to the different types of facial characteristic

Maxillary segmental distraction in children with unilateral clefts of lip, palate, and alveolus.

OBJECTIVES: Alveolar clefts are commonly closed by a bone grafting procedure. In cases of wide clefts the deficiency of soft tissue in the cleft area may lead to wound dehiscence and loss of the bony graft. Segmental maxillary bony transfer has been mentioned to be useful in such cases. Standard distraction devices allow unidirectional movement of the transported segment. Ideally the distraction should strictly follow the dental arch. The aim of this study was to analyze distraction devices that were adapted to the individual clinical situation of the patients. The goal was to achieve a distra

Anatomy of impacted lower third molars evaluated by computerized tomography: is there an indication for 3-dimensional imaging?

OBJECTIVE: Surgical removal of impacted third molar is one of the most frequent procedures in oral surgery. Today 3-dimensional (3D) imaging is occasionally used. The aim of this study was to describe and estimate the frequencies of anatomic variations of lower third molars in patients with panoramic findings at high risk for inferior alveolar nerve (IAN) injury. STUDY DESIGN: The investigators designed and implemented a retrospective cases series study with a study population composed of patients presenting with an impacted lower third molar with projection of the tooth over the full width o

Shoulder mobility after spinal accessory nerve-sparing modified radical neck dissection in oral cancer patients.

OBJECTIVE: Radical neck dissection (RND), a standard surgical procedure for lymph node metastasis of head and neck cancer for decades, causes various dysfunctions, such as pain or limited abduction of the shoulder. Various RND modifications have been made to reduce these postoperative dysfunctions, but the effect of preservation of the spinal accessory nerve is still controversial. The aim of this study was to explain our surgical method of sparing the accessory nerve during neck dissection and to clarify the effect of preserving the nerve on reduction in shoulder dysfunction. STUDY DESIGN: O

Metastases in odontogenic cysts: literature review and case presentation.

Malignant tumors in the oral cavity are relatively rare. About 5% of all malignant growths in the body are localized in the oral cavity. The vast majority of oral malignancies are primary tumors with squamous cell carcinoma being the most frequent and sarcomas occurring very seldom. Secondary tumors caused by hematogenous spread arising from a tumor localized elsewhere in the body are extremely rare. About 1% of all oral cancers are metastases to the jawbones and the surrounding soft tissues. Metastases to the jaws are mainly caused by malignant tumors of the breast, lung, kidney, bone, and co

Pseudogout of the temporomandibular joint: an uncommon cause of temporomandibular joint pain and swelling.

BACKGROUND: Pseudogout, or calcium pyrophosphate deposition, is a rare cause of pain, swelling, and trismus of the temporomandibular joint (TMJ). Diagnosis and management of the lesion are discussed. CASE DESCRIPTION: A 58-year-old female had a 2-month history of progressive swelling of right TMJ associated with trismus and facial pain. Imaging of the TMJ revealed a mixed radiolucent and radiopaque lesion associated with the right TMJ joint space. Surgical excision was performed successfully via preauricular approach. Pathology was consistent with calcium pyrophosphate deposition of the TMJ,

Conservative treatment of a pathologic fracture in a patient with Gorham-Stout disease.

Gorham-Stout disease, or massive osteolysis, is an extremely rare idiopathic condition characterized by spontaneous, localized relentless resorption of one or several contiguous bones. There is no known successful treatment. Autologous bone graft also resorbs. It is a condition with difficult diagnosis, treatment, and prognosis. We report an extreme case of massive osteolysis of the maxillofacial complex. Unique to this case is that the patient presented with a mandibular fracture, and was taking alendronate (Fosamax), one of the treatment options for patients with Gorham-Stout disease. We dis
