Adrenal failure in the critically ill.

Three critically ill patients suffered multiple organ failure secondary to sepsis. Despite adequate supportive therapy and appropriate antibiotic cover, they failed to improve and required inappropriate inotrope support. They had not been treated with steroids or other drugs known to suppress adrenal function. Adrenal insufficiency was suspected. A random cortisol concentration and a short synacthen test demonstrated concentrations below the range expected in all three cases. High-dose steroid therapy was commenced with marked improvement in the short-term. However, in each case sepsis eventua

Fetal heart rate patterns in growth-restricted fetal sheep induced by chronic fetal placental embolization.

OBJECTIVE: Our purpose was to test the hypothesis that chronic placental insufficiency and intrauterine growth restriction in fetal sheep causes a decrease in the number of fetal heart rate accelerations and fetal heart rate variability. STUDY DESIGN: Chronically catheterized fetal sheep were embolized (n = 6) daily with 15 to 50 microns latex microspheres for 21 days between 0.74 and 0.88 of gestation into the abdominal aorta, until fetal arterial oxygen content was decreased by 40% to 50% of the preembolization value. Control animals (n = 6) received saline solution only. Signals from chest

Increased defibrillation threshold due to ventricular fibrillation duration. Potential mechanisms.

The duration of ventricular fibrillation (VF) that precedes a high energy shock has been recognized as a critical determinant of defibrillation outcome. Factors such as metabolic acidosis or alkalosis do not affect outcome. The authors hypothesized that release of myocardial adenosine during VF could potentially mediate the time-dependent effects of VF duration on defibrillation. Defibrillation threshold (DFT) was therefore determined in dogs during concurrent infusion of adenosine and dipyridamole (a nucleoside transport blocker). Transthoracic DFT increased by approximately 50%, whereas tran

Medetomidine, an alpha 2-agonist, alleviates post-thoracotomy pain in dogs.

Twelve laboratory beagles underwent a routine left thoracotomy to insert permanent instrumentation. Every second dog was given 10 micrograms/kg of medetomidine, an alpha 2-agonist sedative. The rest of the animals were treated with 20 micrograms/kg of buprenorphine, an opioid agonist-antagonist, which is regularly used to treat postoperative pain in laboratory animals. The drugs were given at the end of operation (0) and 4, 8, 20, and 24 h postoperatively. Blood samples for catecholamines (adrenaline and noradrenaline) and blood gases (pCO2 and pO2) and pH were drawn immediately before any dru

Drug-induced actions on brain neurotransmitter systems and changes in the behaviors and emotions of depressed patients.

Despite cumulative evidence that the tricyclic drugs result in significant changes in the functioning of brain serotonergic (5-HT) and nordrenergic (NE) systems, such changes have not been found to be associated with recovery from depression. Based upon evidence that the 5-HT and NE systems were associated with different emotions, it was hypothesized that changes in these systems were associated with different components of behavior in drug-responsive patients and not with changes in the "whole" disorder. Findings from this multihospital study of 104 unipolar and bipolar depressed patients sho

Adrenaline-responsive electromechanical dissociation.

Electromechanical dissociation (EMD) occurs when there is no detectable blood pressure in the presence of organised electrocardiographic activity. This condition excludes pulseless ventricular tachycardia, and traditionally was thought to result from a failure of electrical depolarisation in the myocardium to elicit mechanical contraction. It is now understood from echocardiographic evidence that 'pseudo-EMD' is more common-where electrical activity is associated with myocardial contractions too weak to sustain a detectable blood pressure [1]. Three cases of EMD in association with subarachnoi

Psychotropic analgesic nitrous oxide and neurotransmitter mechanisms involved in the alcohol withdrawal state.

We relate the extremely rapid and lasting beneficial effects of psychotropic analgesic nitrous oxide (PAN) on the alcohol withdrawal state (AWS) to the underlying neurotransmitter system disturbances and clinical findings. PAN is an opioid and its main therapeutic effects are produced by stimulating the underactive endogenous opioid system (EOS) found in the AWS. In common with other opioids, PAN also acts on other neurotransmitter systems. While controlling the cholinergic and adrenergic overactivity and the concomitant stress state, through its opioid agonism, it simultaneously stimulates th

The efficacy of TAC (tetracaine, adrenaline, and cocaine) with various wound-application durations.

OBJECTIVE: To associate the efficacy of topical anesthesia of a tetracaine-adrenaline-cocaine solution (TAC) with the duration of skin contact. METHODS: A prospective, physician-blinded, observational study of anesthetic efficacy as a function of TAC application duration in pediatric patients (age range 1-12 years) who had superficial lacerations was performed. Patients who had lacerations less than 5 cm long were eligible for the study. Patients who had lacerations of the finger, toe, penis, nose, or pinna were excluded. Patients with histories of allergies to any of the topical anesthetic c

Transient hypoalgesia under physical exercise--relation to silent ischaemia and implications for cardiac rehabilitation.

A series of studies with humans as well as experiments carried out on animals could show that physical exercise leads to temporary hypoalgesia. Reduced sensitivity to pain is not only demonstrable after long-distance exercise (such as marathon run) but also after intensive physical exercise on a laboratory ergometer. Pain threshold elevation is most pronounced during maximal exertion, but hypoalgesia remains present also after exercise is stopped demonstrating that a systemic analgetic effect is induced by the exercise process. Pre-exercise pain threshold level is returned to approximately 60

The effects of naloxone on the cardiovascular and respiratory effects of centrally administered corticotrophin releasing factor in conscious rabbits.

1. The effects of intracerebroventricular (i.c.v.) and intracisternal (i.c.) administration of corticotrophin releasing factor (CRF) (0.5 nmol kg-1) were examined in conscious rabbits. The effect of opioid receptor antagonism was examined to determine whether the responses to CRF were mediated by endogenous opioid peptides. 2. After i.c.v. CRF there was a rise in mean arterial pressure (MAP), heart rate (HR), plasma noradrenaline and adrenaline, and increased behavioural activity. Respiration rate increased, PaCO2 fell, but PaO2 was unchanged. 3. The pressor and behavioural effects of i.c.v. C

Responsiveness of a human parotid epithelial cell line (HSY) to autonomic stimulation: muscarinic control of K+ transport.

Salivary electrolyte secretion is under the control of the autonomic nervous system. In this paper we report that HSY, an epithelial cell line derived from the acinar-intercalated duct region of the human parotid gland, responds to muscarinic-cholinergic (generation of Ca2+ signal) and beta-adrenergic (generation of cAMP signal), but not to alpha-adrenergic (lack of Ca2+ signal), receptor stimulation. The muscarinic response was studied in detail. Carbachol (10(-4) M, muscarinic agonist) or A23187 (5 microM, calcium ionophore) stimulation of HSY cells increases both 86Rb (K+) influx and efflux

Current review of anaphylaxis and its relationship to asthma.

Anaphylaxis is the most dramatic of hypersensitivity reactions with a two-fold relationship with asthma, in the immediate and late phases and drugs and immunotherapy may be triggers. There are many causes of anaphylaxis, with IgE-mediated reactions, especially those to some foods very common. Drug reactions are next most common, especially to beta-lactam antibiotics, sulphonamides, Dilantin and aspirin. Exercise-induced anaphylaxis is well documented and may be exacerbated by food. Careful examination of the etiology of anaphylaxis is essential. Anaphylaxis may occur during immunotherapy and s

Phorbol esters and cyclic AMP activate AMP deaminase in adult rat cardiac myocytes.

Using rapid deenergization as a probe for adenylate deaminase activity in intact adult rat cardiac myocytes, we have previously established that IMP formation is enhanced by alpha-adrenergic agonists. In the present study, the effect of adrenergic agents on adenylate deaminase was further characterized. Phenylephrine (PE)3 increased IMP production in a dose-dependent fashion with an EC50 of 8 x 10(-7) M. The response to PE was reversed within 10 min by the alpha 1-antagonist, prazosin. Likewise, adenylate deaminase was also activated in ventricular myocytes challenged with phorbol 12-myristate

Modification of noradrenergic innervation in the cerebellum of mutant rats with Purkinje cell degeneration (jaundiced Gunn rats).

In heterozygous (Jj) and homozygous Gunn rats (jj), cerebellar noradrenergic innervation was examined using immunohistochemical, neurochemical and electrophysiological techniques. Immunohistochemical analysis using an antiserum against tyrosine hydroxylase (TH) revealed a marked enhancement in immunoreactivity largely in the granular layer and the whole nuclei in the jj cerebellum, resulting from an increase in TH-immunoreactive varicose fibers forming synapse-like structures on the somata and dendrites of granule cells or nuclear neurons. The concentration of norepinephrine in both the cortic…

Effects of ticlopidine and indobufen on platelet aggregation induced by A23187 and adrenaline in the presence of different anticoagulants.

As Ca2+ is known to play a fundamental role in platelet function, the effect of combining two platelet aggregating agents (adrenaline and the ionophore A23187) with different effects on Ca2+ was studied at levels subthreshold for aggregation using platelet-rich plasma from eight atherosclerotic patients. Adrenaline lowered the A23187 threshold required to induce aggregation. The effects of treating patients with the antiplatelet agents, indobufen and ticlopidine, on A23187 and adrenaline induced aggregation of platelets prepared in hirudin or sodium citrate was also evaluated. Aggregation was

Combined effects of shiftwork and environmental hazards (heat, noise, toxic agents).

The paper deals with the results of studies or discussions concerning the problem of nightwork combined with other adverse working conditions. Special emphasis is laid on the untoward effect of high temperature during nightwork, as well as on noise and exposure to chemicals. It is shown that there is no substantial influence of heat stress on the circadian rhythm of adrenaline excretion under sitting working conditions with the subject performing a difficult mental task at warm climates up to 30 degrees C BET. Shiftwork and noise induce independent different effects which can be explained in t

Evidence that intravenous morphine stimulates central opiate receptors to increase sympatho-adrenal outflow and cause hypertension in conscious rabbits.

1. In conscious rabbits, intravenous morphine caused hypertension, bradycardia, hyperglycaemia and increased plasma adrenaline and noradrenaline. These effects were prevented by ganglionic blockade with pentolinium. 2. The cardiovascular responses to morphine were not altered by pretreatment with a vasopressin V1-receptor antagonist. 3. After bilateral adrenalectomy morphine caused a similar rise in noradrenaline but no increase in adrenaline. The rise in blood pressure was attenuated and the hyperglycaemia was abolished. 4. Adrenaline infused intravenously to mimic the levels that occurred af

Influence of topical anesthesia on the sedation of pediatric emergency department patients with lacerations.

Local anesthetic infiltration of wounds causes pain which distresses children. A painless topical anesthetic solution containing tetracaine, adrenaline, and cocaine (TAC) may reduce this distress. We hypothesized that the use of TAC for anesthesia may reduce the utilization of sedation for laceration repair. We performed a retrospective review of all pediatric emergency department (PED) patients receiving parenteral meperidine, promethazine, and chlorpromazine (DPT) during the period after TAC became routinely available. For comparison, data from a recent retrospective study of DPT (preTAC) us

Determinations of catechols in small volumes of plasma using ion-pair reversed phase liquid chromatography/electrochemistry.

A method with improved sensitivity for detection of catechols (CA) in small volumes of plasma using an ion-pair reversed phase HPLC system with electromechemical detection is presented. Fast isocratic separations were obtained by using 7.5 cm x 4.6 mm (i.d.) reversed phase columns with 3C18 3 micron silica particles. The CA:s L-DOPA, Noradrenaline (NA), Adrenaline (A), Dihydroxybenzylamine (DHBA, i.s.), DOPAC and Dopamine (DA) were separated in less than 4 min. Intrinsic gamma aminobutyric acid receptors modulate the release of catecholamine from canine adrenal gland in situ.

Immunohistochemical analysis documented the presence of gamma-aminobutyric acid (GABA)-containing fibers and GABA-containing chromaffin cells in canine adrenal glands. A dense network of fibers was visualized at the boundary between medullary and cortical cells, and, in the medullary tissue, GABA-containing fibers surrounded chromaffin cells. Some of these fibers enter the adrenal medulla together with splanchnic cholinergic nerves. The functional role of the GABAergic system in the regulation of catecholamine release from adrenal chromaffin cells was studied in canine adrenal glands in situ,
