Treatment and outcome of nocardia keratitis.

OBJECTIVE: To report our experience in treatment and outcome ofNocardia keratitis. METHODS: Medical and microbiology records of seven cases of culture-provenNocardia keratitis seen between January 1997 and March 1999 were reviewed retrospectively. In all patients, corneal scrapings were obtained for direct microscopic evaluation and culture. Drug sensitivity was determined by the Kirby-Bauer disk-diffusion method. The minimum inhibitory concentration of ciprofloxacin and amikacin for these isolates was determined by agar dilution method. Response to medical therapy and the end result were ana

Pneumococcal keratitis after laser in situ keratomileusis.

A 20-year-old man developed keratitis in his right eye 2 days after laser in situ keratomileusis (LASIK). The patient had rubbed the eye with unclean fingers the night before the onset of symptoms. Examination showed an inferior corneal ulcer with dense infiltration at the junction of the lamellar flap and the surrounding cornea associated with a hypopyon. Streptococcus pneumoniae was isolated on culture. The ulcer resolved with combination therapy of cephazolin 5% and tobramycin 1.3% eyedrops. Patients having LASIK should be instructed that inadequate patient hygiene may predispose to bacteri

Fungal keratitis as an indicator of HIV infection in Africa.

A 1-year hospital-based study was undertaken on 212 patients at Muhimbili Medical Centre (MMC) from October 1994 to October 1995. The objective was to determine the aetiology of corneal ulceration. A comparison of the prevalence of HIV infection between patients with fungal keratitis and those with non-fungal keratitis was included. Bacterial infection (32.1%), and mycotic infection (15.1%) were the leading causes of corneal ulceration. There were 32 patients with fungal keratitis and 180 patients with non-fungal keratitis. The male:female ratio for patients with fungal keratitis was 1.7:1 and

Sutureless wound infection by unusual acid-fast organisms.

PURPOSE: To report infection of self-sealing wounds in cataract surgery by unusual acid-fast organisms. METHODS: Two patients who underwent phacoemulsification and developed corneal tunnel infection by acid-fast organisms were included. Both were managed with appropriate medical and surgical treatment modalities. RESULTS: Diagnosis was facilitated in the first case by diagnostic and therapeutic lamellar keratoplasty and in the second case by the analysis of scrapings obtained from the inner lip of the tunnel. Ziehl-Neilsen staining helped diagnosis of acid-fast bacteria and implementation of

Surgical Management of Vision Loss in Cerebral Venous Thrombosis: Case Series from a Tertiary Care Stroke Center.

BACKGROUND AND PURPOSE: Cerebral venous thrombosis (CVT) presenting as vision loss is uncommon. Raised intracranial tension in CVT is proposed as one of the mechanisms (13.2%). There are still unknown underlying mechanisms to explain vision loss in CVT. The safety and outcome of the surgery (optic nerve sheath fenestration [ONSF] or theco-peritoneal shunt [TPS]) to reduce intracranial hypertension and prevent vision loss has not been studied. METHODS: A retrospective case record review of CVT patients with impending vision loss who underwent ONSF/TPS from 2007 to 2019 was performed from the s

Staphylococcus aureus from ocular and otolaryngology infections are frequently resistant to clinically important antibiotics and are associated with lineages of community and hospital origins.

Staphylococcus aureus is an important human pathogen that causes serious antibiotic-resistant infections. Its population structure is marked by the appearance and dissemination of successful lineages across different settings. To begin understanding the population structure of S. aureus causing ocular and otolaryngology infections, we characterized 262 isolates by antimicrobial sensitivity testing and multilocus sequence typing (MLST). Methicillin-resistant S. aureus were subjected to SCCmec typing and Panton-Valentine leukocidin (PVL) screening. Although we detected a high level of genetic di

Bacillary angiomatosis with cytomegaloviral and mycobacterial infections of the palpebral conjunctiva in a patient with AIDS.

We report the clinical and histopathologic findings of bacillary angiomatosis involving the palpebral conjunctiva with concomitant infection by cytomegalovirus and Mycobacterium species in a patient with acquired immune deficiency syndrome. After debulking, the conjunctival tissue was studied with the use of light and electron microscopy; stains for bacteria, acid-fast bacilli, and Bartonella species; and immunohistochemical studies for cytomegalovirus and herpes simplex virus. We observed the typical histopathologic findings of bacillary angiomatosis, the presence of bacilli stained by the St

Intravenous Tobramycin Inhalation for Patients With Advanced Bronchiectasis With Pseudomonas aeruginosa Infection in Home Medical Care: A Report of Two Cases.

Home medical care faces limitations in the number of doctor and nurse visits, availability of medical devices, and economic factors, making daily injections difficult for in-home patients. We describe two cases of advanced bronchiectasis with Pseudomonas aeruginosa infection treated with inhaled tobramycin in a home setting, demonstrating clinical effectiveness. Using commercially available empty eye drop containers to prepare an aseptic inhalation solution and nebulizers easily usable at home, our experience suggests that this could be a viable therapeutic alternative in home medical care.

Isolated Intramuscular Cysticercosis in Children: A Case Report.

Human cysticercosis is an emerging infection caused by the larvae of Taenia solium (Cysticercus cellulosae). The most common sites for cysticercosis are the central nervous system, subcutaneous tissues, eyes, and muscles. Isolated intramuscular cysticercosis without brain involvement is rare and only a few reports are available in children. Here, we report two children with isolated intramuscular cysticercosis who presented with the swellings that were diagnosed by ultrasonography and fine-needle aspiration cytology. Both of them responded well to steroids and albendazole treatment, showing a

First report of concurrent mixed germs induced destructive emphysematous pyelonephritis and emphysematous endophthalmitis in the patient with COVID-19.

A 35-year-old diabetic woman was referred to the emergency department with fever, left flank pain, pneumaturia, and impaired vision of the left eye from 4 days ago. Fever, tachycardia, tachypnea, low blood pressure, metabolic acidosis, and azotemia were the first findings. The diagnosis was a coincidence of emphysematous pyelonephritis and emphysematous endophthalmitis due to computerized tomography of the patient. Immediate fluid and electrolytes resuscitation, intravenous antibiotic administration, and nephrectomy save the patient. Urine, blood, and vitreous cultures revealed mixed germ infe

[A foreign object in the frontal lobe].

In this case report a 77-year-old male tumbled in his lavatory and hit his head against a toilet roll holder. The holder penetrated his eye and orbita and left a 0.5 × 1 cm cylindrical piece of plastic inside his frontal lobe. He was admitted at a neurosurgical department, where the foreign object was removed. In this report, we summarise some of the basic principles of handling patients with a penetrating brain trauma, including risk of infection, control of bleeding and technical considerations before surgically removing a foreign object.

Human papillomavirus 52 positive squamous cell carcinoma of the conjunctiva.

Human papillomavirus (HPV) infection is strongly associated with several human cancers; the most known genotypes involved being HPV 16 and HPV 18. We report the detection of HPV 52 in a sample taken from a 47-year-old patient with squamous cell carcinoma of the conjunctiva of the left eye. The method used for the detection of HPV was real time polymerase chain reaction. The evolution was favorable after surgical removal of the tumor and the patient was explained that long-term follow-up is essential to avoid recurrence.

Acute syphilitic posterior placoid chorioretinitis in late latent syphilis.

Diverse manifestations of ocular syphilis may involve any structure in the eye, at any stage of the disease. Posterior uveitis in the form of posterior placoid chorioretinitis has been described in secondary- and tertiary-acquired syphilis. In this case report, we present a 47-year-old man with late latent syphilitic infection and fundoscopic, as well as angiographic findings consistent with acute syphilitic posterior placoid chorioretinitis. To our knowledge this form of patchy multifocal choroiditis has never been described in the latent stage of the disease.

Loiasis: African eye worm.

The filarial parasite Loa loa is transmitted by Chrysops fly bites. Loiasis is endemic in rainforest areas of West and Central Africa, and sporadic cases have also been diagnosed in travellers and migrants. Whilst many infected persons are asymptomatic, microfilariae may be detected in the blood or adult worms may be seen under the skin or the sclera of the eye. Mass treatment programmes for onchocerciasis have raised concern about the risk of severe adverse effects when ivermectin is distributed in areas co-endemic for onchocerciasis and loiasis.

Harlequin ichthyosis: a case study.

The most severe manifestation of congenital ichthyosis presents itself in the form known as harlequin ichthyosis. Harlequin ichthyosis is characterized by a profound thickening of the fetal skin. Affected infants are born with a thick "horny shell" of dense, waxy, platelike scales with deep, irregular fissures covering the body. Contraction of the extremities is seen, along with deformities of the ears, eyes, mouth, and digits. This "armor" limits movement and compromises the skin's barrier defense, leaving the newborn at risk for metabolic abnormalities and infection. A case study of our unit

Concurrent chromomycosis and aspergillosis of the maxillary sinus.

The authors describe a rare case of an infection of both the chromomycosis and aspergillosis of the maxillary sinus in an immunocompetent 72-year-old female who presented with progressive visual loss and dull aching pain of the left eye. Sinuscopy of the left maxillary sinus showed swelling of the mucosa with clay-like materials. Biopsy from the left maxillary sinus showed the typically characteristic morphology of chromomycosis and culture from sinus tissue which yielded Aspergillus. The patient responded to a combination therapy of surgical excision and antifungal agent.
