Perivascular-submandibular lymph node metastasis in squamous cell carcinoma of the tongue and floor of mouth.

AIMS: The goal of this study was to investigate the incidence of occult metastasis in perivascular lymph node and nodal recurrence in these nodal pads in squamous cell carcinoma (SCC) of the tongue and floor of mouth. METHODS: We performed a prospective analysis of the incidence of an occult metastasis in the perivascular lymph nodes in 55 patients (41 with an oral tongue carcinoma and 14 with a mouth floor carcinoma) who underwent an elective supraomohyoid neck dissection (SOHND) for SCC of the tongue and floor of mouth, from 1997 to 2002. 99 SOHND procedures were performed as follows: 72 in

Case report of malocclusion with abnormal head posture and TMJ symptoms.

Abnormal cervical muscle function can cause abnormal head posture, adversely affecting the development and morphology of the cervical spine and maxillofacial skeleton, which in turn leads to facial asymmetry and occlusal abnormality. There can be morphologic abnormalities of the mandibular fossa, condyle, ramus, and disk accompanying the imbalance of the cervical and masticatory muscles activities. Two normally growing Japanese female patients with Class II Division 1 malocclusion presented with TMJ symptoms and poor head posture as a result of abnormal sternocleidomastoid and trapezius cervic

Value of three-dimensional computed tomography in craniomaxillofacial surgery.

Three-dimensional (3-D) computed tomographic (CT) scans of 72 adult patients were evaluated by two observers in a blinded fashion by comparing the 3-D images with the two-dimensional (axial and coronal) CT scans. With 3-D CT, the observers found improved visualization of facial anatomy in certain patients (i.e., asymmetries of the whole face and skull, large defects of the midface and skull vault, and fractures with major dislocation). In these defects, the information can be assimilated more rapidly. The documentation of effects of cranial surgery on craniofacial growth and resorption after r

[The effect of multiple and combined shock-inducing trauma on the processes of bone tissue reparation and on the recovery of work capacity in the victims].

Terms of consolidation of fractures and working ability were studied in 273 patients. Patients with multiple fractures of bones and with fractures associated with cranio-cerebral traumas were found to have slow consolidation (in 66.41%) or formation of a false joint (in 6.1%) due to severity of the trauma. It is just the cause of prolonged treatment and disability of the I or II group (in 66.6%). Active surgical treatment of such patients is necessary.

[Importance of intraoperative monitoring of ABR and compound action potential of the eighth cranial nerve during microvascular decompression surgery].

An intraoperative recording of a direct compound action potential of the 8th cranial nerve with pre, intra and postoperative monitorings of auditory brainstem evoked response (ABR) was carried out for the purpose of clarifying mechanism and prevention of postoperative hearing dysfunction following microvascular decompression surgery as a treatment for hyperdysfunction syndrome of cranial nerves. In 221 patients with hemifacial spasm and tic douloureux out of 510 patients operated on by microvascular decompression surgery, ABR was monitored before, during and after surgery and furthermore, in t

[Swellings in the maxillofacial region (author's transl)].

The most frequent swellings in the maxillofacial region can be traced to odontogenic inflammations. Chronic dental foci exacerbate and break through the soft tissue layer surrounding the jaw. There the purulent material quickly spreads along the muscular pocket and simulates a phlegomonous inflammation. Inflammations of the individual soft part niches have characteristic symptoms. Because of the loose interconnections the infectious material can spread very quickly into the parapharyngeal space and from there downwards into the mediastinum or upwards into the base of the skull. Because of the

Reconstruction of midface defects with the osteocutaneous radial forearm flap: Evaluation of long term outcomes including patient reported quality of life.

BACKGROUND: Maxillectomy defects significantly impair quality of life. Prosthetics can overcome some of these issues, but has limitations. The role of the osteocutaneous radial forearm free flap (OC-RFFF) has been established for reconstruction of smaller maxillectomy defects, but its role in larger defects is not well defined. We aim to evaluate outcomes after midface reconstruction utilizing the OC-RFFF. METHODS: Retrospective review of prospective database collected between 2005 and 2014 of midface reconstruction using OC-RFFF in a tertiary care centre. Donor site complications and acute a

[Prosthetic supply of tissue defects in head and neck surgery].

BACKGROUND: Cancer surgery, malformation or trauma may cause broad facial defects that cannot be covered by patients because of their exposed site. Such defects lead to functional deficits and enormous psychological strain and require rehabilitation at all ages. Prosthetic supply has been developed into a functional and aesthetic alternative to plastic and reconstructive surgery. METHODS AND PATIENTS: The condensed experience with prosthetic supply after persistent surgical defects of more than twenty patients during the recent years is described. RESULTS: Efficient prosthetic supply in the

Restoration of reaching and grasping movements through brain-controlled muscle stimulation in a person with tetraplegia: a proof-of-concept demonstration.

BACKGROUND: People with chronic tetraplegia, due to high-cervical spinal cord injury, can regain limb movements through coordinated electrical stimulation of peripheral muscles and nerves, known as functional electrical stimulation (FES). Users typically command FES systems through other preserved, but unrelated and limited in number, volitional movements (eg, facial muscle activity, head movements, shoulder shrugs). We report the findings of an individual with traumatic high-cervical spinal cord injury who coordinated reaching and grasping movements using his own paralysed arm and hand, reani

Skin-fat composite grafts on full-thickness facial skin defects.

When used to cover facial skin defects, skin grafts and local flaps have limitations such as color mismatch, volume depletion, and long scar. We used skin-fat composite grafts harvested with enough fat tissue over the whole plane to cover full-thickness facial skin defects. This study enrolled 15 composite grafts, and the results were rated by 2 investigators on a scale of 1 to 5 based on skin texture, color, and volume. Skin color analysis using a spectrophotometer was performed on 9 grafts using the L*a*b* score. There were no major complications, but 1 partial loss. The clinical assessment

Delayed repair of facial nerve trauma: an experimental study in guinea pigs.

CONCLUSION: The curative effect in the group where facial-facial anastomosis (FFA) was delayed for 7 days was similar to that in the immediate FFA group and had a better repair within 60 days. OBJECTIVE: Due to the uncertainty about optimal timing of delayed facial nerve repair, we designed an experimental study in guinea pigs to find out the cut-off duration for delayed nerve repair. METHODS: In this study, 64 male guinea pigs were randomly divided into 8 groups: normal, immediate FFA, and delayed FFA for 7, 14, 21, 30, 60, and 90 days (n = 6). Two months after suture of the nerves, the pas

Achieving aesthetic results in facial reconstructive microsurgery: planning and executing secondary refinements.

BACKGROUND: Free tissue transfer to improve bulk and contour in facial deformities has been proven useful, yet refinements that turn an acceptable result into an excellent result are essential to reconstruction. The authors reviewed their experience and described these refinements. METHODS: The charts of 371 free tissue transfer cases (1989 to 2010) performed by the senior author (J.W.S.) were reviewed. Free tissue transfer of a circumflex scapular variant flap or superficial inferior epigastric was performed to treat deformities arising from hemifacial atrophy (n = 126), hemifacial microsomi
