Under-recognition of prenatal alcohol effects in infants of known alcohol abusing women.

Medical records of 124 women and their infants were analyzed for: (1) documentation of maternal alcohol and other substance abuse and (2) evaluation of exposed infants. These results were compared with the study interview and infant examination. More obstetric nurses documented the presence or absence of alcohol and substance abuse than did pediatricians. More women reported using alcohol in the study interview than documented in the medical records. There was slightly better documentation for cocaine use than for alcohol use in the medical records. One of the 19 infants with documentation of

A female case with the Kleine-Levin syndrome and its physiopathologic aspects.

A female case with the Kleine-Levin syndrome (KLS), which first occurred at the age of 19, was discussed in relation to the following four characteristics: 1) a female case, 2) a loss of memory and the appearance of slow waves in the pathosis, 3) the abnormal pattern of growth hormone (GH) secretion during sleep in the pathosis, and 4) a prolonged latency between the peaks of III and V in the auditory brainstem response (ABR) in both the pathosis and nonpathosis. These characteristics may suggest that there is a slight disturbance of consciousness in the pathosis, and that there is a functiona

Pregnancy with Wilson's Disease: A Case Series.

Wilson's disease is an autosomal recessive disorder that affects copper transport due to deficiency of ceruloplasmin and causes deposition of copper mainly in the liver, brain and cornea. It causes hepatic and/or neuropsychiatric manifestations. This copper deposition causes cirrhosis of the liver, encephalopathy and liver failure. It also causes disorders of the menstrual cycle and subfertility. During pregnancy, there is an increased risk of the development of preeclampsia, fetal growth restriction, and neurologic damages to the fetus. Pregnancy with decompensated liver cirrhosis developed m

Clinical and Genetic Aspects of Phelan-McDermid Syndrome: An Interdisciplinary Approach to Management.

Phelan-McDermid syndrome (PMS) is a rare, heterogeneous, and complex neurodevelopmental disorder. It is generally caused by a heterozygous microdeletion of contiguous genes located in the distal portion of the long arm of chromosome 22, including the SHANK3 gene. Sequence variants of SHANK3, including frameshift, nonsense mutations, small indels and splice site mutations also result in PMS. Furthermore, haploinsufficiency in SHANK3 has been suggested as the main cause of PMS. SHANK3 is also associated with intellectual disability, autism spectrum disorder and schizophrenia. The phenotype of PM

Endocrine disorders in Prader-Willi syndrome: a model to understand and treat hypothalamic dysfunction.

Prader-Willi syndrome is a rare genetic neurodevelopmental disorder resulting from the loss of expression of maternally imprinted genes located in the paternal chromosomal region, 15q11-13. Impaired hypothalamic development and function is the cause of most of the phenotypes comprising the developmental trajectory of Prader-Willi syndrome: from anorexia at birth to excessive weight gain preceding hyperphagia, and early severe obesity with hormonal deficiencies, behavioural problems, and dysautonomia. Growth hormone deficiency, hypogonadism, hypothyroidism, premature adrenarche, corticotropin d

A case of Ménétriere´s disease treated with the monoclonal antibody cetuximab.

Ménétriere´s disease is a rare disorder of the body and fundus of the stomach, characterized by a massive proliferation of the foveolar cells and subsequent excess mucous secretion. This results in hypoproteinemia due to loss of serum proteins across the gastric mucosa. The cause of Ménétriere´s disease is unknown, and due to the irreversible and premalignant character of the disorder, the patients affected have been subdued to gastrectomy as the only curable treatment. Epidermial growth factor (EGF) has been implicated in the pathogenesis, a finding that makes the disorder receptive to monocl

Cholestasis caused by panhypopituitarism and acquired cytomegalovirus infection in a 2-month-old male infant: A case report.

RATIONALE: Septo-optic dysplasia (SOD) is a rare congenital disorder that may cause jaundice in infants. However, it is usually prone to neglect and misdiagnosis in infants with cholestasis because endocrine disorder such as panhypopituitarism is rare in the cause of infantile cholestasis. We report a case of SOD concurrent with acquired cytomegalovirus (CMV) infection, who presented with prolonged jaundice as the first clinical sign. PATIENT CONCERNS: The patient was a 2-month-old male infant who presented with cholestasis, combined with fever and panhypopituitarism. DIAGNOSES: He was diagn

Updates and knowledge gaps in cholesteatoma research.

The existence of acquired cholesteatoma has been recognized for more than three centuries; however, the nature of the disorder has yet to be determined. Without timely detection and intervention, cholesteatomas can become dangerously large and invade intratemporal structures, resulting in numerous intra- and extracranial complications. Due to its aggressive growth, invasive nature, and the potentially fatal consequences of intracranial complications, acquired cholesteatoma remains a cause of morbidity and death for those who lack access to advanced medical care. Currently, no viable nonsurgica

Overproduction of vascular endothelial growth factor/vascular permeability factor is causative in Crow-Fukase (POEMS) syndrome.

Crow-Fukase or POEMS syndrome of polyneuropathy, organomegaly, endocrinopathy, M-protein, and skin changes is a rare multisystem disorder of obscure pathogenesis that is associated with microangiopathy, neovascularization, and accelerated vasopermeability. We examined the levels of the vascular endothelial growth factor/vascular permeability factor (VEGF) in the serum and cerebrospinal fluid (CSF) from 10 patients with this syndrome. Serum VEGF levels were about 15-30 times those in control subjects or patients with Guillain-Barré syndrome (GBS), chronic inflammatory demyelinating polyneuropat

Clinical Features of Hypo- and Hypervitaminosis of Fat-Soluble Vitamins in Pediatric Patients.

Fat-soluble vitamins (A, D, E, and K) are crucial for pediatric health, contributing to normal cellular function, growth, immune defense, and development. Unlike water-soluble vitamins, they are absorbed with dietary fats and stored in the liver and adipose tissue, leading to risks of deficiency (hypovitaminosis) and toxicity (hypervitaminosis) in certain physiological and pathological conditions. This narrative review aimed to summarize the clinical manifestations, diagnostic considerations, and management of hypo- and hypervitaminosis of fat-soluble vitamins in pediatric populations. Platelet-derived growth factor receptor alpha regulates fetal testis differentiation via an ERK-CREB axis.

Platelet-derived growth factor receptor alpha (Pdgfra) plays a crucial role in mesenchymal cell differentiation, but the molecular signaling involved in this process remains unclear, particularly within the fetal testis. Here, we use XY Pdgfra-null gonads to investigate the molecular mechanisms underlying testicular organogenesis, focusing on the formation of testicular architecture and the differentiation of fetal Leydig cells (FLCs), the steroidogenic lineage arising from mesenchymal precursors within the testicular interstitial compartment. The extracellular signal-regulated kinase (ERK) pa

The Runx2-SMOC axis in skeletal development: Expanding roles of Smoc1 and Smoc2 in development and disease.

BACKGROUND: Secreted modular calcium-binding proteins (SMOCs), including SMOC1 and SMOC2, are secreted protein acidic and rich in cysteine family matricellular proteins that regulate cell-matrix interactions and growth factor signaling across development and disease. Recent studies positioned SMOC1 and SMOC2 as direct downstream targets of Runt-related transcription factor 2 (Runx2), the master transcription factor for osteoblast differentiation, and revealed dose-dependent modulation of bone morphogenetic protein 2 (BMP2) signaling by SMOC2. HIGHLIGHT: SMOC1/2 exhibit distinct yet complement

Preferential crosstalk between perifollicular capillary vessels and dermal papilla cells during hair cycling homeostasis.

The tissue-specific capillary system supports the unique function of each organ and dynamically remodels in response to local requirements. In this study, we found that perifollicular vascularization flexibly adjusts to accommodate physiological changes. Notably, not all capillary vessels responded and migrated equally. However, those around the dermal papilla (DP) exhibited preferential mobilization. Treatment with minoxidil, a hair growth agent, significantly increased perifollicular vessel mobilization around the DP, whereas it was inhibited in experimental models of tissue aging, such as t

Targeting the Gut-Brain Axis Through Insulin-like Growth Factors: Therapeutic Implications and Future Directions.

The gut-brain axis represents a sophisticated bidirectional communication network connecting the gastrointestinal tract and central nervous system through neural, endocrine, and immune pathways. Insulin-like growth factors (IGFs), particularly IGF-1 and IGF-2, function as pivotal mediators within this communication framework. These polypeptide growth factors regulate intestinal barrier integrity, microbiota homeostasis, neurogenesis, and synaptic plasticity mechanisms. Clinical evidence from 1989 to 2024 demonstrates that gut microbiota-derived short-chain fatty acids enhance IGF-1 production

Signaling Pathways in Gliomas.

Changes in cell signaling pathways, which in normal conditions determine the maintenance of cell homeostasis and the correctness of its basic processes, may cause the transformation of a normal cell into a cancer cell. Alterations in cellular metabolism leading to oncogenesis are considered to be a hallmark of cancer cells. Therefore, a thorough understanding of cellular enzymes affecting metabolism and respiration, as well as intracellular pathways connected with them, seems crucial. These changes may be both prognostic and predictive factors, especially in terms of using molecularly targeted

[XVI Jesús Culebras Lecture. Development of eating habits in children: sense and sensitivity].

The first years of a child's life are periods of rapid growth and significant neurocognitive development. Adequate energy and macro- and micronutrient intake are essential for proper growth and development. The development of feeding skills is a very sensitive stage that requires, on the one hand, exposure to new foods and different textures and, on the other, an appropriate response from parents to their children's demands and cues. This development of the foundations of eating habits occurs in utero and progresses throughout infancy and early childhood (3 to 5 years). A woman's diet during p

INTRODUCTION: The aim of this article is to present the diagnostic and therapeutic approach to unilateral posterior vertical insufficiency. MATERIAL AND METHODS: The authors describe the management protocol. RESULTS: Posterior vertical insufficiency (PVI) manifests clinically as obliquity of the maxillo-mandibular occlusal plane and bicommissural line, and deviation of the chin. Radiographically, PVI is characterized by an elevation of the mandibular angle in relation to the base of the odontoid. Among the etiologies, congenital disorders are the most frequent, dominated by condyle hypoplasi
