The association of clinical conditions and serologic tests with CD4+ lymphocyte counts in HIV-infected subjects without AIDS.

Early intervention guidelines in HIV infection require knowledge of CD4+ lymphocyte count; however, CD4+ determinations require special laboratory procedures and may not be readily available in all situations. Using data from 207 HIV-seropositive homosexual men without AIDS, we evaluated the association of difference clinical conditions or serologic tests with CD4+ count. Men with conditions including seborrheic dermatitis, hairy leukoplakia, oral candidiasis and chronic diarrhea, and men with beta2-microglobulin levels greater than or equal to 4.0 mg/l had significantly lower CD4+ counts. Is the adult social care outcomes toolkit (ASCOT) relevant to health protection teams? Development of an outbreak support tool.

BACKGROUND: Outbreaks of infection in care homes can be mitigated by Public Health and Social Measures (PHSM) including isolation of cases, quarantine of exposed contacts, and shielding the most vulnerable. We aimed to assess the relevance and utility of Adult Social Care Outcomes Toolkit (ASCOT) in outbreak decision-making, with a view to reducing the adverse impact of control measures and informing pandemic planning. METHODS: We worked with health protection practitioners (HPPs) to determine the relevance of ASCOT domains to health protection. The domains were adapted into a 'Wellbeing Chec

Perspectives from the Canadian public health community on the use, adoption barriers to, and implementation supports needed for the Core Competencies for Public Health in Canada: Release 2.0.

The Core Competencies for Public Health in Canada: Release 2.0 come at a moment when the public health sector is grappling with uneven capacity, shifting expectations, and the need for clearer professional grounding. Reflections from national engagement sessions and a modified Delphi survey reveal that practitioners anticipate using the competencies to strengthen education and training, guide human resource practices, support public health advocacy, and inform organizational planning. These aspirations are tempered by concerns about the practical realities of implementation, including securing

Advancing Global Health Initiative Through Pediatric Vitamin A Supplementation.

Background: Malnutrition, particularly vitamin A deficiency, affects approximately 49% of children aged 6-59 months in sub-Saharan Africa, increasing the risks of morbidity, mortality, poor growth, and preventable blindness. Tanzania has followed the World Health Organization's (WHO) guidelines for biannual vitamin A supplementation (VAS) since 2004, but rural access remains limited. A preliminary assessment at a rural Tanzanian day care showed only 29% VAS coverage, indicating a substantial gap in coverage. Objective: To improve nutritional health among children aged 6-59 months through a com

Pregnancy and Preconception Care for Patients with Inflammatory Bowel Disease.

BACKGROUND: For both men and women, family planning, including decisions regarding marriage and childbearing, is a significant life event that contributes to overall "happy life." Patients with chronic diseases such as inflammatory bowel disease face not only concerns about common pregnancy-related complications but also disease-specific challenges, including the potential effects of medications on the fetus and the risk of disease exacerbation during pregnancy. In response to these concerns, patients may occasionally make misguided decisions, such as the self-discontinuation of necessary trea

When science remembers what is neglected: addressing the risks of rodent-borne diseases in urban Latin America.

Latin American cities face deep socio-economic inequalities that impact health, education and the environment. These disparities influence zoonotic disease dynamics through complex host-pathogen-environment-human interactions. This study explores zoonotic risks associated with rodent communities across diverse social and environmental settings in the Matanza-Riachuelo River Basin, one of the most polluted urban areas in Buenos Aires, Argentina. Between 2022 and 2023, rodent community composition and health status were assessed at 22 sites, ranging from low-income neighbourhoods to green areas

Allocation of Users of Mental Health Services to Needs-Based Care Clusters: An Italian Pilot Study.

In Italy, despite strong community-based mental health services, needs assessment is unsatisfactory. Using the Mental Health Clustering Tool (MHCT) we adopted a multidimensional and non-diagnosis dependent approach to assign mental health services users with similar needs to groups corresponding to resources required for effective care. We tested the MHCT in nine Departments of Mental Health in four Italian regions. After a brief training, 318 professionals assessed 12,938 cases with a diagnosis of schizophrenia, depression, bipolar disorder and personality disorder through the MHCT. 53% of ca

Direct health care cost of treatment and medication of biliary atresia patients using the National Database of Health Insurance Claims and Specific Health Checkups.

BACKGROUND: Treatment of biliary atresia (BA), which typically requires an initial surgical intervention called the Kasai procedure (KP) and possible liver transplant (LT) afterwards, is quite resource-intensive and would affect patients and families for a lifetime; yet a comprehensive view of the economic burden has not been reported. We estimated direct health care costs from the public payer perspective using the National Database of Health Insurance Claims and Specific Health Checkups of Japan. METHODS: Children newly diagnosed at ages 0 days to 4 years between April 2010 and September 20

Characterizing Disabilities in Preschool Aged Children with Congenital Zika Virus Syndrome with the ICF Model.

Understanding functioning and disabilities in children with Congenital Zika Syndrome (CZS) is essential for health planning. We describe disabilities present in children with CZS followed in a reference hospital in Rio de Janeiro, Brazil, based on the biopsychosocial model of the International Classification of Functioning, Disability and Health (ICF). This was a cohort study of children >3 years of age with CZS. Disability was characterized through outcomes related to ICF components assessed via clinical and motor development evaluations. Among 50 children, with a median age of 40 months, 47 …

Magnetic resonance imaging of the internal auditory meatus for vestibular schwannoma in ENT practice: a retrospective analysis with literature and guidelines review.

OBJECTIVES: Magnetic resonance imaging scans of the internal acoustic meatus are commonly requested in the investigation of audio-vestibular symptoms for potential vestibular schwannoma. There have been multiple studies into protocols for requesting magnetic resonance imaging for vestibular schwannoma, but none have been reported based on UK National Institute for Health and Care Excellence guidelines for investigating audio-vestibular symptoms. This study intended to identify the local magnetic resonance imaging detection rates and patterns of vestibular schwannoma, and to audit the conformit
