Safety, efficacy, and monitoring of bipolar radiofrequency ablation in beating myopathic human and healthy swine hearts.

BACKGROUND: The safety and efficacy parameters for bipolar radiofrequency (RF) ablation are not well defined. OBJECTIVE: The purpose of this study was to investigate the safe range of power, utility of transmyocardial bipolar electrogram (EGM) amplitude, and circuit impedance in ablation monitoring. METHODS: Sixteen beating ex vivo human and swine hearts were studied in a Langendorff setup. Ninety-two bipolar ablations using two 4-mm irrigated catheters were performed at settings of 20-50 W, 60 seconds, and 30 mL/min irrigation in the left ventricle. RESULTS: For low-power ablations (20 and

The brain-heart interaction in epilepsy: implications for diagnosis, therapy, and SUDEP prevention.

The influence of the central nervous system and autonomic system on cardiac activity is being intensively studied, as it contributes to the high rate of cardiologic comorbidities observed in people with epilepsy. Indeed, neuroanatomic connections between the brain and the heart provide links that allow cardiac arrhythmias to occur in response to brain activation, have been shown to produce arrhythmia both experimentally and clinically. Moreover, seizures may induce a variety of transient cardiac effects, which include changes in heart rate, heart rate variability, arrhythmias, asystole, and ot…

Smartwatch-based detection of cardiac arrhythmias: Beyond the differentiation between sinus rhythm and atrial fibrillation.

Within the span of a few years, watches have functionally morphed from objects that tell time to wearable minicomputers that allow real-time recording of electrocardiograms (ECGs). Considerable information can be deduced from these single lead tracings, and it is now not uncommon to see patients in whom diagnostic tracings of clinically relevant but elusive arrhythmias are captured using a smartwatch. Empowering individuals to record their own ECG tracings in scenarios such as palpitations, syncope, and for risk stratification of sudden death intuitively has considerable potential, but its val

Combined epicardial and endocardial ablation for atrial fibrillation: Best practices and guide to hybrid convergent procedures.

The absence of strategies to consistently and effectively address nonparoxysmal atrial fibrillation by nonpharmacological interventions has represented a long-standing treatment gap. A combined epicardial/endocardial ablation strategy, the hybrid Convergent procedure, was developed in response to this clinical need. A subxiphoid incision is used to access the pericardial space facilitating an epicardial ablation directed at isolation of the posterior wall of the left atrium. This is followed by an endocardial ablation to complete isolation of the pulmonary veins and for additional ablation as

Normal ranges and potential modifiers of T-wave morphology parameters among healthy individuals: A meta-analysis.

BACKGROUND: T-wave morphology parameters, such as total cosine R-to-T (TCRT), T-wave loop dispersion (TWLD), T-wave morphology dispersion (T … or TCRT, TWLD, TMD, and TWR. Linear regression analysis was conducted for age, percentage of males, heart rate, and QTc. RESULTS: The weighted TCRT was 0.40 ± 0.05, significantly higher than the various cutoffs previously suggested to identify high risk. There was some overlap between the results of weighted normal TMD (19.42 ± 6.77°), TWLD (38.51 ± 0.31), …

Noninvasive Electrocardiography in the Perinatal Mouse.

Electrocardiography (ECG) has long been relied upon as an effective and reliable method of assessing cardiovascular (and cardiopulmonary) function in both human and animal models of disease. Individual heart rate, rhythm, and regularity, combined with quantitative parameters collected from ECG, serve to assess the integrity of the cardiac conduction system as well as the integrated physiology of the cardiac cycle. This article provides a comprehensive description of the methods and techniques used to perform a noninvasive ECG on perinatal and neonatal mouse pups as early as the first postnatal…

Mortality Prediction in Severe Congestive Heart Failure Patients with Multifractal Point-Process Modeling of Heartbeat Dynamics.

Multifractal analysis of human heartbeat dynamics has been demonstrated to provide promising markers of Congestive Heart Failure (CHF). Yet, it crucially builds on the interpolation of RR intervals series, which has been generically performed with limited links to CHF pathophysiology. We devise a novel methodology estimating multifractal autonomic dynamics from heartbeat-derived series defined in the continuous time. We hypothesize that markers estimated from our novel framework are also effective for mortality prediction in severe CHF. We merge multifractal analysis within a methodological fr

Cost-Effectiveness of Ventricular Assist Device Destination Therapy for Advanced Heart Failure in Duchenne Muscular Dystrophy.

Destination ventricular assist device therapy (DT-VAD) is well accepted in select adults with medically refractory heart failure (HF) who are not transplant candidates; however, its use in younger patients with progressive diseases is unclear. We sought to evaluate the cost-effectiveness of DT-VAD in Duchenne muscular dystrophy (DMD) patients with advanced HF. We created a Markov-state transition model (5-year horizon) to compare survival, costs, and quality of life (QOL) between medical management and DT-VAD in DMD with advanced HF. Model input parameters were derived from the literature. We

Improved outcomes after heart transplantation for cardiac amyloidosis in the modern era.

BACKGROUND: Cardiac amyloidosis, caused most commonly by deposition of light chain (AL) or transthyretin (ATTR) type fibrils, has an extremely poor prognosis. In this retrospective single-center study, we evaluated temporal trends in survival after heart transplantation for cardiac amyloidosis. METHODS: We analyzed 48 patients with cardiac amyloidosis (AL, n = 32; familial ATTR, n = 16) who underwent heart transplantation from May 2002 to March 2017. Patients were analysed in 2 periods, Era 1 (2002- 2007) and Era 2 (2008- 2017), separated by altered patient selection in both, AL and ATTR amyl

Study of factors affecting the progression and termination of drug induced Torsade de pointes in two dimensional cardiac tissue.

INTRODUCTION: To study the conditions leading to the initiation and termination of drug induced Torsade de pointes (TdP) along with QT prolongation. METHODS: A 2D anisotropic transmural section of the ventricular myocardium is modeled using the TP06 equations and the cells are interconnected with gap junction conductances (GJC). The tissue is remodeled by reducing the repolarization reserve (by increasing calcium current (ICaL)) of all cells thus making them vulnerable to development of early after depolarizations (EADs). RESULTS: Clinical risk conditions like decreased potassium current (IK

Conversion of atrial dissociation with lidocaine in a dog.

A 3-year-old healthy dog was presented for abdominal surgery following ingestion of razor blades. An electrocardiogram revealed a regular sinus bradycardia with normal P waves at a heart rate of 45 bpm. In addition, low-amplitude positive deflections (p' waves) were visualized at a regular interval and rapid rate of 250 bpm, dissociated from the normal sinus P waves. A tentative diagnosis of atrial dissociation was proposed. Administration of lidocaine abolished the p' waves. This case describes atrial dissociation observed following premedication that was successfully terminated using lidocai…

Vagal afferents, sympathetic efferents and the role of the PVN in heart failure.

Sympatho-excitation is a characteristic of cardiovascular disease including heart failure (HF). The paraventricular nucleus of the hypothalamus (PVN) is an important site for central integration of sympathetic outflow. Atrial volume receptors (AVRs) in the wall of the right atrium transduce cardiovascular variables (pressure/volume) into an input that is integrated centrally, in for example, the PVN. Descriptions of the location and structure of the AVRs as well as the molecular mechanism initiating transduction remain scarce, nevertheless preautonomic neurons of the PVN have been consistently

Suppression of vagal cardiac modulation by blue light in healthy subjects.

BACKGROUND: In the contemporary life environments, our body is increasingly exposed to various sources of colored light, which may affect our physiological functions as non-image-forming effects. We examined the impacts of colored lights on the autonomic functions by the analysis of heart rate variability (HRV). METHODS: A lighting device consisting of four organic light-emitting diode (OLED) modules (55 × 55 mm2) with adjustable red-green-blue color was secured 24 cm above the eyes of subject lying supine in a light-shielded laboratory. Following a 15-min supine rest, electrocardiogram and r…

Survival on the heart transplant waiting list: impact of continuous flow left ventricular assist device as bridge to transplant.

BACKGROUND: Continued donor organ shortage and improved outcomes with current left ventricular assist device (LVAD) technology have increased the number of patients supported with bridge-to-transplantation (BTT) therapy. Using the United Network of Organ Sharing (UNOS) database, we assessed the impact on survival in patients supported with BTT while on the heart transplant waiting list. METHODS: The UNOS database was queried from January 2005 to June 2012 to identify patients listed for heart transplantation as UNOS status 1A or 1B. Patients implanted with a pulsatile-flow device or an LVAD o

Outcomes of heart transplantation in small children bridged with ventricular assist devices.

BACKGROUND: Ventricular assist devices (VADs) have been used with increasing frequency to bridge small children to heart transplantation (HTx), but outcomes in large cohorts are not well established. METHODS: Small children (≤ 10 kg) bridged to HTx with VADs between 2004 and 2010 were identified in the United Network for Organ Sharing database. Survival was modeled using the Kaplan-Meier method, and 2:1 propensity matching was used to compare outcomes with a well-matched control cohort of nonbridged HTx recipients. RESULTS: Of the 803 small children who underwent HTx during the study period,

Prevalence and clinical correlates of early repolarization and J wave in a large cohort of subjects without overt heart disease.

BACKGROUND: Recent studies have suggested that early repolarization (ER) is associated with increased risk of ventricular tachyarrhythmias. … Variables independently associated with both ER and J wave included young age, male sex, and lower heart rate. There was no increased history of symptoms (palpitations and syncope) possibly related to arrhythmias in STE or J-wave subjects. CONCLUSIONS: Typical ER pattern and J wave are common in noncardiac subjects, particularly in young people, and are no …

Exercise improves cardiac autonomic function in obesity and diabetes.

Physical activity is a key element in the prevention and management of obesity and diabetes. Regular physical activity efficiently supports diet-induced weight loss, improves glycemic control, and can prevent or delay type 2 diabetes diagnosis. Furthermore, physical activity positively affects lipid profile, blood pressure, reduces the rate of cardiovascular events and associated mortality, and restores the quality of life in type 2 diabetes. However, recent studies have documented that a high percentage of the cardiovascular benefits of exercise cannot be attributed solely to enhanced cardiov
