Anatomic Considerations for Sinonasal and Ventral Skull Base Malignancy.

Malignancies of the sinonasal region and ventral skull base include a varied group of uncommon tumors that are a challenge to treat. These malignancies, with few exceptions, often present late because of their insidious growth and bland symptomatology. As with malignancies of other sites, the primary goal in surgical management is complete resection with negative margins. This presents a unique surgical challenge in that these lesions lie within a region of densely populated anatomic real estate. This fact reinforces the importance of complete preoperative work-up and a sound anatomic understa

Freshwater drowning in a child: A case study demonstrating the role of post-mortem computed tomography.

In recent years, modern imaging techniques have gained ground in forensics. A crucial question is whether virtual autopsy is capable of replacing traditional autopsy. Forensic diagnosis of freshwater drowning (FWD) is based on the evidence of findings from external inspection (e.g. frothy fluid exuding from the mouth and nostrils), internal examination (e.g. pulmonary congestion, enlargement of heart chambers) and biochemical analysis (haemodilution), findings which are non-specific. The detection of diatoms in organs of the systemic circulation may be of some assistance, but this analysis is

Maxillary osteomyelitis by mucormycosis: report of four cases.

Mucormycosis is a fungal infection commonly affecting structures in the head and neck, such as the air sinuses, orbits, and the brain. Common predisposing factors include diabetes mellitus and immunosuppression. We describe our clinical experience with four cases of mucormycosis of the maxillary antrum associated with uncontrolled diabetes mellitus managed at our centre. Early diagnosis and prompt treatment can significantly reduce the mortality and morbidity of this lethal fungal infection.

Transport of a patient with massive traumatic epistaxis using a cricket helmet and posterior nasal packing.

In developing countries, when patients with traumatic epistaxis cannot be adequately treated at their local medical facility and require further treatment at a distant tertiary care center, it is important that bleeding be controlled before their transport. We describe a patient with a traumatic anterior ethmoidal artery bleed who needed to be taken to a tertiary care center 8 hours away for endoscopic ablation, which was not available at our hospital. The inflated balloon of an 18-Fr Foley catheter attached to the face guard of a cricket helmet was used as a posterior nasal pack. The patient

Mandibular cortical bone graft part 2: surgical technique, applications, and morbidity.

The mandibular cortical grafts, including the ramus graft, are reliable donor sites for the reconstruction of the alveolar ridge deformities before dental implant placement. The highest concentrations of promoter proteins (e.g., bone morphogenetic proteins) are found in the mandibular cortical grafts. Part 1 of this article (April issue) reviewed the anatomical considerations of mandibular cortical grafts, the associated healing process, and the factors influencing graft incorporation. This article describes mandibular ramus graft before implant placement including the surgical technique, appl

Resurfacing the nasomaxillary cavity for management of epistaxis in Osler-Rendu-Weber disease.

Recurrent massive epistaxes due to heriditary haemorrhagic telangectasia were managed in two patients by ablation of the mucosa of the nasal and maxillary cavities and resurfacing with radial forearm fasciocutaneous free flaps. Both patients had undergone multiple procedures attempting to control the epistaxes prior to the definitive procedures described. Both patients remained epistaxis free postoperatively and retained their nasal airways. Olfactory function was preserved in the second patient due to preservation of the superior turbinate and cribriform olfactory mucosa.

Nasal physiology in swimmers and swimmers' sinusitis.

To investigate the pathogenesis of swimmers' sinusitis we questioned and examined a group of 20 swimmers and 20 controls. Clinical examination was followed by rhinomanometry, sonography of the maxillary sinuses, a saccharine test to assess mucociliary transport in the nose, and a viable cytological sampling of the nasal mucosa. The history showed a preponderance of symptoms of sinusitis and chronic rhinitis in the swimmers' group, as well as a reddening of the nasal mucosa and a swelling of the adenoids. Sonography showed mucosal thickening in the antrum in more cases among the swimmers. Rhino…

Juvenile nasopharyngeal angiofibroma presenting as a facial swelling. A case report.

Juvenile nasopharyngeal angiofibroma is a rare benign neoplasm occurring almost exclusively in adolescent males. When it is confined to the nasopharynx, surgery is often curative. In 20% of cases, there is intracranial extension, and radiotherapy may be used to avoid the risk of life-threatening haemorrhage. The authors report an unusual case which presented with a swelling of the cheek and an abducens nerve palsy. The lesion extended from the nasopharynx across the pterygomaxillary fissure, as well as intracranially. Radiotherapy was given, and the patient remains disease-free after one year.

Rhinomaxillary mucormycosis in a kidney transplant patient.

Rhinomaxillary mucormycosis is an uncommon but important pathologic entity with a rapidly fatal course if treatment is delayed. This is of particular interest to dentists, since many of the earliest clinical signs appear in the oral and perioral regions. Successful therapy demands early diagnosis by biopsy, institution of amphotericin B therapy, and surgical débridement. A case of nonfatal rhinomaxillary mucormycosis is presented, and all other reported cases in transplant patients are reviewed.

[Subdural empyema as a complication of maxillary sinusitis (description of 3 cases)].

The authors report 3 cases of subdural empyema with different course and outcome. In all of them subdural empyema developed as a complication of chronic maxillary sinusitis. In the first case a coexistent brain abscess was the cause of death. In the second case operation carried out when the patient was in extremely severe condition failed to prevent death since the diagnosis had been made too late. Complete recovery was obtained in the third case owing to early clinical diagnosis confirmed with angiography, followed by early surgical treatment of the primary focus and early craniotomy. The im…

Clinical study of dynamic navigation-assisted immediate implant placement in posterior maxillary alveolar bone defects.

OBJECTIVE: To evaluate the accuracy and clinical effect of immediate implant placement(IIP) using real-time dynamic navigation in the poster … oup. All implants were successfully placed without serious complications such as perforation of the maxillary sinus mucosa. The initial stability of the implant in the navigation group was (28.53 ± 5.81)N.cm and (18.47 ± 3.64)N.cm, respectively. The initial stability of the implant in the navigation group was higher than that in the free hand group (P < 0.05). Th …

Comparison of the efficiency and safety of endonasal and open rhinosurgery in children.

OBJECTIVES/HYPOTHESIS: The objective of this study was to compare the efficiency and safety of endonasal endoscopic maxillary surgery and the Caldwell-Luc approach in children. STUDY DESIGN: Case series. METHODS: A total of 121 children aged 9 to 17 years with chronic rhinosinusitis were included in the study. The patients were operated on using Caldwell-Luc (n = 36) and endoscopic maxillary surgery (n = 85) procedures. Efficiency and safety of the surgeries were assessed using pre- and postoperative endoscopic evaluation of the mucosa of the nasal cavity, 20-item Sino-Nasal Outcome Test (SN

The employment of thermographic examinations in the diagnostics of diseases of the paranasal sinuses.

OBJECTIVES: The article was aimed at assessing the diagnostic value of a thermographic examination of the pathology of the paranasal sinuses. METHODS: 39 patients met the inclusion criteria for the study. The cases of unilateral inflammations of the maxillary sinuses (odontogenic and non-odontogenic, acute and chronic) were chosen on the basis of a comprehensive panel of diagnostic tests including among other things: a radiological examination, endoscopy, the histopathological assessment of the state of the collected mucous membrane. The thermographic examination was carried out on the day pr…
