The MG-QOL15 for following the health-related quality of life of patients with myasthenia gravis.

The MG-QOL15 is helpful in informing the clinician about the patient's perception of the extent of and dissatisfaction with myasthenia gravis (MG)-related dysfunction. The aims of this study were to determine the usefulness of the MG-QOL15 for following individuals with MG and to guide clinical researchers who plan to use the MG-QOL15. We assessed sensitivity and specificity for detecting clinical change and evaluated test-retest reliability. Sensitivities and specificities of various cut-points of change in scores are presented. Also presented are means and standard deviations of MG-QOL15 sco…

Comparative clinical features and immune responses after extended thymectomy for myasthenia gravis in patients with atrophic versus hyperplastic thymus.

BACKGROUND: Although extended thymectomy is believed to be suitable for myasthenia gravis (MG) patients with hyperplastic thymus, it is not clear whether surgical treatment is indicated for MG patients with atrophic thymus. We therefore assessed the clinical features and immune responses in 175 MG patients who underwent thymectomy between 1990 and 2004. METHODS: All patients underwent extended thymectomy by the transsternal approach. Clinical features, prognosis, and immune response after extended thymectomy were compared in patients with atrophic and hyperplastic thymuses. RESULTS: Of the 1…

Matrix metalloproteinases in myasthenia gravis.

INTRODUCTION: Myasthenia gravis (MG) is an autoimmune disease with weakness in striated musculature due to anti-acetylcholine receptor (AChR) antibodies or muscle specific kinase at the neuromuscular junction. A subgroup of patients has periocular symptoms only; ocular MG (OMG). Matrix metalloproteinases (MMP) are increased in several autoimmune diseases, including generalized MG (GMG), and have been suggested to play a role in immune cell infiltration, basement membrane breakdown and autoimmune pathogenesis. METHODS: Total levels of MMP2, MMP3 and MMP9 were measured in serum by ELISA. RESUL…

Peripheral nerve hyperexcitability: a clinical and immunologic study of 38 patients.

OBJECTIVE: We studied a case series of peripheral nerve hyperexcitability (PNH) aiming to describe clinical characteristics, immunologic and … d nonparaneoplastic PNH (group B: 16 patients), and isolated paraneoplastic PNH (3 with thymoma and myasthenia gravis, 1 with thyroid carcinoma). PNH clinical features were similar in groups A and B. We found an overall high prevalence of clinical autoimmunity (33% of group A and 63% of group B) and systemic non-neuronal autoantibodies (42% of group A …

[Recent advancement in the basic and clinical studies in myasthenia gravis].

Myasthenia gravis (MG) is an autoimmune disease characterized by fatigable muscle weakness, and autoantibodies to nicotinic acetylcholine receptor (AchR) have been thought to play the key role in the pathogenesis. The recent great progress in the basic research is identification of a novel autoantibody to muscle-specific kinase (MuSK). This discovery led to elucidation of difference in pathogenesis between AchR antibody (+) and MuSK antibody (+) diseases. The recent progress in the clinical practice for MG is introduction of video-assisted thoracic surgery. Less invasive surgery is expected to…

Proton magnetic resonance spectroscopy of the intrinsic tongue muscles in patients with myasthenia gravis with different autoantibodies.

OBJECTIVE: To assess lipid composition of the intrinsic tongue muscles in patients with myasthenia gravis (MG). METHODS: This study included 15 MG patients with antibodies against muscle-specific kinase (MuSK), 15 matched MG patients with antibodies against acetylcholine receptor (AChR) and 15 matched healthy subjects. Middle posterior region of the tongue was analyzed by single voxel point-resolved proton magnetic resonance spectroscopy (MRS) using 1.5T MRI scanner. RESULTS: MRS obtained from subject with AChR MG showed a broad resonance arising from methylene groups of lipids (PMN) with no…

Thymoma-associated myasthenia gravis without acetylcholine receptor antibodies.

Patients with myasthenia gravis and thymoma usually present antibodies to the acetylcholine receptor (AchR-Ab). Only two cases of thymoma-associated myasthenia gravis without AchR-Ab have been previously reported. We describe a case of seronegative thymoma-associated myasthenia gravis as a further evidence of the variability of myasthenia gravis in terms of antibody profile and thymic pathological findings.

Proteasome inhibition with bortezomib depletes plasma cells and autoantibodies in experimental autoimmune myasthenia gravis.

Bortezomib, an inhibitor of proteasomes, has been reported to reduce autoantibody titers and to improve clinical condition in mice suffering from lupus-like disease. Bortezomib depletes both short- and long-lived plasma cells; the latter normally survive the standard immunosuppressant treatments targeting T and B cells. These findings encouraged us to test whether bortezomib is effective for alleviating the symptoms in the experimental autoimmune myasthenia gravis (EAMG) model for myasthenia gravis, a disease that is characterized by autoantibodies against the acetylcholine receptor (AChR) of …

Neuromuscular junction toxicity with tandutinib induces a myasthenic-like syndrome.

BACKGROUND: Tandutinib (MLN 518, Millennium Pharmaceuticals, Cambridge, MA) is an orally active multitargeted tyrosine kinase inhibitor that is currently under evaluation for the treatment of glioblastoma and has been used in the treatment of leukemia. In prior clinical and animal studies, a dose-dependent muscular weakness has been observed with this drug, though the etiology of the weakness has not been defined. METHODS: Standard neurophysiologic techniques, including repetitive nerve stimulation, needle EMG, and single-fiber EMG, were used to evaluate patients who developed weakness while

Thymectomy for inducing remission in juvenile myasthenia gravis.

PURPOSE: The aim of this study was to determine the efficacy of thymectomy to induce remission in juvenile myasthenia gravis. METHODS: A retrospective review of all patients undergoing a thymectomy for the treatment of juvenile myasthenia gravis was performed at a single tertiary referral centre between 1997 and 2009 (N = 8). All cases were moderate to severe cases (Osserman stage ≥ 2a). All operations were open. Median follow-up was 18 months (range 1-77). Postoperative progress was assessed using the De Filippi classification of remission. MAIN RESULTS: Only two minor complications were re…

Diagnostic utility of stimulated single-fiber electromyography of the orbicularis oculi muscle in patients with suspected ocular myasthenia.

Stimulated single-fiber electromyography (SSFEMG) is a valuable diagnostic tool in cases of myasthenia gravis with limited disease. From 1990 to 2008 SSFEMG in the orbicularis oculi muscle (OO) was performed in a cohort of 456 patients referred with clinical suspicion of myasthenia gravis (MG) and exclusively ocular symptoms. A diagnosis of MG was made on clinical grounds in 103 patients. In this patient cohort, the specificity of SSFEMG for myasthenia was 97%, and the sensitivity was 80%. MG patients with a normal SSFEMG had a benign clinical course. This study confirms a high specificity and…

Psychosocial aspects in myasthenic patients treated by plasmapheresis.

Knowledge about the postintervention psychosocial status of myasthenia gravis (MG) is limited and based on questionnaire studies. In this study, the effects of improvement in muscle strength with plasmapheresis treatment on both quality of life (QoL) and psychological status of MG patients were studied. Between January 2008 and December 2009, 29 MG patients were enrolled to receive one course of plasmapheresis treatment. Differences in baseline and posttherapy clinical, laboratory, and psychosocial measures were determined. The mean MG score decreased from 7.8 points at baseline to 4.2 after p…

Neuropeptides of human thymus in normal and pathological conditions.

Human thymus of healthy subjects and patients affected by thymoma-associated Myastenia Gravis were studied in order to visualize and compare the morphological distributive pattern of four neuropeptides: vasoactive intestinal peptide, substance P, neuropeptide Y, and neurotensin. Based on our observations, we formulated hypotheses on their relations in neuro-immunomodulation under physiological and pathophysiological conditions. Immuno-histochemical staining for neuropeptides was performed and morphological and morphometrical analyses were conducted on healthy and diseased thymus. In normal thy…

Mosaic caveolin-3 expression in acquired rippling muscle disease without evidence of myasthenia gravis or acetylcholine receptor autoantibodies.

Inherited rippling muscle disease is an autosomal dominant disorder usually associated with caveolin-3 mutations. Rare cases of acquired rippling muscle disease with abnormal caveolin-3 localisation have been reported, without primary caveolin-3 mutations and in association with myasthenia gravis and acetylcholine receptor autoantibodies, or thymoma. We present three new patients with electrically-silent muscle rippling and abnormal caveolin-3 localisation, but without acetylcholine receptor autoantibodies, or clinical or electrophysiological evidence of myasthenia gravis. An autoimmune basis …

Delayed presentation of invasive thymoma in myasthenia gravis: a case report.

Current practice favors imaging of people with myasthenia gravis (MG) at the time of diagnosis to look for evidence of thymoma or thymic hyperplasia. However, there is no evidence to allow any recommendation about repeat imaging in people with normal scans at presentation, and there is little evidence to recommend surgical exploration in such cases. We present a patient with a delayed presentation of invasive thymoma 11 years after a normal thoracic computerized tomography.
