Regeneration of phasic synapses on a crayfish slow muscle following allotransplantation of a mixed phasic-tonic nerve.

Separate phasic or tonic nerves allotransplanted to reinnervate a denervated slow superficial flexor muscle (SFM) in the abdomen of adult crayfish regenerate synaptic nerve terminals with phasic or tonic properties. To test competitive interactions between tonic and phasic axons, we allotransplanted the sixth abdominal ganglion with its third nerve root containing a mixture of phasic and tonic axons onto the denervated SFM. The resulting reinnervation of the SFM was compared to the normal innervation on the contralateral intact SFM, which receives innervation only from tonic motoneurons. Varia

Acute onset of infantile spinal muscular atrophy.

Two patients with acute generalized weakness and areflexia are presented. The electrophysiologic studies in both revealed evidence of decreased conduction velocity and mixed axonal and demyelinating neuropathy, suggestive of the diagnosis of Guillain-Barré syndrome. The young ages of the patients and their failure to respond to immunoglobulin therapy were the major clues to the final diagnosis of spinal muscular atrophy type I. Blood for DNA study revealed homozygous deletion mutation in exons 7 and 8 of the survival motor neuron gene. This diagnosis should be considered in every child under 1

Selective inhibition of tumor necrosis factor-alpha prevents nucleus pulposus-induced thrombus formation, intraneural edema, and reduction of nerve conduction velocity: possible implications for future pharmacologic treatment strategies of sciatica.

STUDY DESIGN: The possibility to prevent nucleus pulposus-induced functional and structural nerve root injury by selective tumor necrosis factor-alpha inhibition was assessed in an experimental model in the pig spine. OBJECTIVE: The objective of the study was to evaluate the role of tumor necrosis factor-alpha in the mediation of nucleus pulposus-induced nerve injury by using selective inhibition. SUMMARY OF BACKGROUND DATA: The cytokine tumor necrosis factor-alpha has been suggested to play a key role in the nerve root injury induced by local application of nucleus pulposus. However, previo

Evaluation of blink reflex results obtained from workers previously diagnosed with solvent-induced toxic encephalopathy.

We reviewed blink reflexes recorded from 51 railroad workers with long-term occupational exposure to solvents who were diagnosed by others with solvent-induced toxic encephalopathy. No worker fulfilled conventional clinical criteria for dementia or trigeminal mononeuropathy. All workers had normal R1 and R2 blink reflex latencies. R1 latencies correlated significantly with several nerve conduction measures, including F wave latencies, suggesting that some intersubject variability reflected intrinsic conduction properties, not isolated brain-stem function. Although normal, the workers' R1 laten

Assessment of serum-mediated neurotoxicity in Navajo neuropathy.

Navajo neuropathy is a unique sensorimotor neuropathy which is geographically restricted to Navajo children living on the Navajo Reservation. Affected patients present with weakness, loss of sensation in extremities, corneal ulcerations, and a high incidence of childhood infections. Metabolic complications, such as severe liver disease, may further contribute to peripheral nerve injury in affected patients. In this study, serum-mediated injury to rat peripheral nerve was critically assessed. Serum samples from affected Navajo patients were tested in vivo for effects on peripheral nerve functio

Electrodiagnostic examination of lumbosacral radiculopathies.

To determine the diagnostic efficacy of late responses and of magnetic stimulation in the electrodiagnostic evaluation of lumbosacral radiculopathies, 42 patients with acute monoradiculopathies of L5 or S1 were examined. We performed conventional nerve conduction studies, F-wave studies, needle electrode examination (NEE) and magnetic stimulation. The results were compared with a control group of 36 persons. In the patients with weakness, we found a diagnostic sensitivity for NEE of 90% in L5 and of 80% in S1. F-waves had the same sensitivity as NEE in the patients with weakness and were more

Motor conduction alterations in patients with lumbar spinal stenosis following the onset of neurogenic claudication.

The pathogenesis of neurogenic claudication is thought to lie in relative ischemia of cauda equina roots during exercise. In this study we will evaluate the effect of the transient ischemia brought on by exercise on motor conduction in patients suffering from lumbar spinal stenosis (LSS). We will also evaluate the sensitivity of motor evoked potentials (MEPs) in detecting motor conduction abnormalities before and after the onset of neurogenic claudication. Thirty patients with LSS and 19 healthy volunteers were enrolled in the study. All LSS patients had a history of neurogenic claudication an

Hyperlipidemia and neuropathy.

Detailed histories/physical examinations (n = 47) and nerve conduction studies (n = 34) were performed in a large kindred with congenital partial lipodystrophy (CPL) in an effort to evaluate the association of hyperlipidemia and neuropathy. CPL, in the absence of diabetes, did not predispose to either focal mononeuropathy or generalized polyneuropathy, and the likelihood of either did not correlate with the degree of cholesterol or triglyceride elevation. This study does not provide support for an association between neuropathy and hyperlipidemia.

Endogenous pH shifts facilitate spreading depression by effect on NMDA receptors.

Rapid extracellular alkalinizations accompany normal neuronal activity and have been implicated in the modulation of N-methyl-D-aspartate (NMDA) receptors. Particularly large alkaline transients also occur at the onset of spreading depression (SD). To test whether these endogenous pH shifts can modulate SD, the alkaline shift was amplified using benzolamide, a poorly permeant inhibitor of interstitial carbonic anhydrase. SD was evoked by microinjection of 1.2 M KCl into the CA1 stratum radiatum of rat hippocampal slices and recorded by a proximal double-barreled pH microelectrode and a distal

Guillain-Barré syndrome: MR imaging findings of the spine in eight patients.

PURPOSE: To evaluate magnetic resonance (MR) imaging findings of the spine in patients with Guillain-Barré syndrome. MATERIALS AND METHODS: MR imaging findings in eight patients (three male, five female; age range, 2-47 years) with Guillain-Barré syndrome were retrospectively reviewed. Guillain-Barré syndrome was diagnosed mainly on the basis of symptoms and also on the basis of supportive ancillary data, such as the results cerebrospinal fluid analysis and electrophysiologic evaluation. In addition, follow-up MR imaging was performed in three patients, who had slight clinical improvement. R

Progressive cerebellar ataxia, proximal neurogenic weakness and ocular motor disturbances: hexosaminidase A deficiency with late clinical onset in four siblings.

Tay-Sachs disease is a genetically determined neurodegenerative disorder, resulting from mutations of the hexosaminidase (Hex) A gene coding for the alpha-subunit of beta-D-N-acetyl-hexosaminidase. Clinically, there is severe encephalomyelopathy leading to death within the first few years of life. Hex A activity is usually absent in tissue and body fluids of these patients. Juvenile and adult Hex A deficiencies are less severe but rare variants with some residual Hex A activity. All these variants are most prevalent among Ashkenazi Jews. We describe a non-Jewish family in which four adult brot

Peripheral neuropathy associated with essential mixed cryoglobulinaemia: a role for hepatitis C virus infection?

BACKGROUND: The prevalence of hepatitis C virus (HCV) infection has been estimated at 43 to 84% in patients with essential mixed cryoglobulinaemia in recent large series. Some of these cases have been successfully treated with interferon-alpha. The objective was to evaluate the prevalence and the possible role of HCV infection in essential mixed cryoglobulinaemia. METHODS: Fifteen patients (eight men and seven women; mean age: 61.2 (SD 16.5) years) with peripheral neuropathy (10 polyneuropathies and five multifocal mononeuropathies) and essential mixed cryoglobulinaemia were tested for serum

Long-term neurotoxicity in patients treated with cisplatin, vinblastine, and bleomycin for metastatic germ cell cancer.

Thirty patients treated for germ cell cancer with six cycles of cisplatin, vinblastine, and bleomycin participated in a follow-up examination of neurotoxicity 49 to 106 months after treatment. Of these, 22 patients (73%) had sensory loss; half of them complained of paresthesias. The vibration perception threshold was increased in 24 patients (80%). Auditory stimulation revealed a normal latency of the first component of the brain stem-evoked potentials Pl but an increased interpeak interval between this and Pv; reflecting a central conduction defect. Motor conduction velocity (CV) along the pe

The role of thiamine in nervous tissue.

The possibility that thiamine (vitamin B1) has a role in nervous tissue that is independent of its well-documented coenzyme function is discussed. After reviewing the localization and metabolism of the vitamin and its phosphate esters, the effects of either thiamine deprivation or antimetabolites of thiamine on conduction and transmission, and the relationship between thiamine triphosphate and the genetic, neurological disease, subacute necrotizing encephalomyelopathy (Leigh's disease), it is suggested that despite the lack of hard evidence, it is likely that the vitamin possesses this alterna

Abnormal breathing patterns.

In health, breathing is regular and the respiratory rate is sufficiency constant to be useful as a vital sign of health and disease. This regularity depends on a complex interplay of chemical and neural control systems that operate automatically to reset the rate and depth of breathing as changes occur in posture and activity, to adjust the level of ventilation so that changes in gas tensions and pH in the blood and in the brain intersitial fluid are exceedingly modest despite wide swings in metabolic rate and in environmental conditions, and to coordinate ventilation and circulation so that t

Mechanism of action of local anaesthetic agents and biotoxins.

Four current theories for the mechanism of action of local anaesthetics are that these drugs: (1) interfere with some chemical, such as acetylcholine, that is involved in nervous conduction; (2) alter the density of fixed charges on the surface of the membrane; (3) cause an expansion of some volume of membrane that is critical for conduction; and (4) react with some specific receptor in the nerve membrane. The first theory fails for lack of evidence that acetylcholine is involved physiologically in conduction. The other three theories, however, all seem to apply.
