A Review of the Measurement of the Neurology of Gait in Cognitive Dysfunction or Dementia, Focusing on the Application of fNIRS during Dual-Task Gait Assessment.

Poor motor function or physical performance is a predictor of cognitive decline. Additionally, slow gait speed is associated with poor cognitive performance, with gait disturbances being a risk factor for dementia. Parallel declines in muscular and cognitive performance (resulting in cognitive frailty) might be driven primarily by muscle deterioration, but bidirectional pathways involving muscle-brain crosstalk through the central and peripheral nervous systems are likely to exist. Following screening, early-stage parallel declines may be manageable and modifiable through simple interventions.

Practice parameter: early detection of dementia: mild cognitive impairment (an evidence-based review) [RETIRED]. Report of the Quality Standards Subcommittee of the American Academy of Neurology.

OBJECTIVE: The goal of this project was to determine whether screening different groups of elderly individuals in a general or specialty practice would be beneficial in detecting dementia. BACKGROUND: Epidemiologic studies of aging and dementia have demonstrated that the use of research criteria for the classification of dementia has yielded three groups of subjects: those who are demented, those who are not demented, and a third group of individuals who cannot be classified as normal or demented but who are cognitively (usually memory) impaired. METHODS: The authors conducted computerized l

Parental Perspectives on Future Outcomes for Children Diagnosed With Fetal Neurological Conditions.

BACKGROUND: Current literature on fetal neurological consultation contains limited input from families. We aimed to determine which potential future outcomes associated with prenatally diagnosed neurological conditions are the most important to parents. METHODS: We performed a cross-sectional discrete choice experiment with parents who had undergone fetal neurological consultation at two children's hospitals. Parents ranked the relative importance of potential future outcomes associated with fetal neurological conditions. A hierarchal Bayesian application of multinomial logistic regression pr

Reversible cerebral vasoconstriction syndrome: a severe neurological complication in early puerperium.

Reversible cerebral vasoconstriction syndrome (RCVS) is an uncommon entity characterized by transient, segmental vasoconstriction of the cerebral arteries, which may be associated with sudden-onset headache and acute neurological complications. The puerperium represents a recognized risk period for its development. We describe the case of a 29-year-old woman who, in the early puerperium, presented with sudden-onset orthostatic headache, followed by severe acute headache and impaired level of consciousness. Brain computed tomography revealed bilateral intraparenchymal hemorrhages. Angiographic

Empowering recovery: the role of self-management in motor-functional neurological disorder.

A Self-management refers to patients' independent use of knowledge and skills to manage symptoms, maintain health, and respond to setbacks, both during and beyond active therapy. This mini-review summarises the current literature on self-management in motor-FND, including its key components, definitions, and role in supporting recovery during rehabilitation. FND-focused education, strategies to normalise movement, goal setting with graded activity, and the development of a relapse-prevention plan are described as components of self-management. Empirical evidence is limited but promising. Futur

Investigating the Multiple Regulatory Mechanisms and Therapeutic Targets of PHLDA1 in Neurological Diseases.

PHLDA1 (pleckstrin homology-like domain family A member 1) is a pleiotropic regulatory protein that affects key biological processes such as apoptosis, pyroptosis, immune inflammation, autophagy, metabolism, and oxidative stress. PHLDA1 plays a significant role in the pathological mechanisms of neurological diseases. This article systematically reviews the molecular characteristics of PHLDA1 and its core role in cerebrovascular diseases such as cerebral ischemia/ reperfusion injury, cerebral hemorrhage, subarachnoid hemorrhage, epilepsy, amyotrophic lateral sclerosis (ALS), and Parkinson's dis

Magnetic Field-Guided Magnetic Nanoparticles as Neurotherapeutics for Neurological Disorders and Glioblastoma.

Neurodegenerative diseases, including Alzheimer's disease (AD), Parkinson's disease (PD), and stroke, are among the most devastating neurological disorders worldwide. Glioblastoma (GBM) is a rapidly growing cancer that originates in astrocytes in the brain. It invades and damages the nervous system. Current treatment options remain limited, primarily due to poor blood-brain barrier penetration, lack of targeted delivery, and limited efficacy in slowing disease progression or promoting functional recovery. In recent years, magnetic fields (MFs) have emerged as a promising therapeutic approach,

Insulin Resistance as a Shared Pathophysiological Driver in Neurological Disorders: a Narrative Review.

PURPOSE OF REVIEW: Insulin resistance has traditionally been viewed as a peripheral metabolic abnormality; however, accumulating evidence indicates that impaired insulin signaling within the central nervous system plays a critical role in the pathogenesis of multiple neurological disorders. The purpose of this narrative review is to synthesize current evidence supporting insulin resistance as a shared pathophysiological driver across neurodegenerative, neurological, and neuropsychiatric conditions, and to evaluate the potential of nutrition- and metabolism-based interventions as modulatory str

ZNFX1 Deficiency: An Interferonopathy Mimicking Wilson Disease with Hepatic and Neurologic Manifestations.

ZNFX1 deficiency is a recently recognized type I interferonopathy with heterogeneous systemic manifestations. We report a 15-year-old boy presenting with chronic liver disease, hepatic fibrosis, gait disturbance, and persistent proteinuria. Neuroimaging demonstrated bilateral globus pallidus signal changes and diffuse cerebral calcifications, raising suspicion for a metabolic disorder, particularly Wilson disease. However, copper metabolism studies and hepatic copper quantification were normal, and ATP7B sequencing was negative. Liver biopsy showed portal-to-portal bridging fibrosis without st

Intraosseous access in neurological emergencies: a critical narrative review and research framework.

Timely vascular access is critical in neurological emergencies, where delays in treatment may worsen secondary brain injury and functional outcomes. Intraosseous access offers a rapid alternative when intravenous cannulation is difficult, delayed, or unsuccessful, but its role in neurocritical care remains incompletely defined. PubMed, ScienceDirect, and Scopus were searched from inception through August 2025, with a targeted evidence update through 10 September 2026. This critical narrative review examines the feasibility, pharmacologic considerations, safety, evidence quality, and research g

When R-Loops Go Awry: Genome Instability and Neurological Diseases.

The basic structure of DNA is a double helix formed by base pairing between complementary strands. However, during transcription, RNA hybridizes with the template DNA, whereas the complementary DNA strand becomes displaced and remains unpaired. This process forms a DNA-RNA hybrid structure known as an R-loop; similar structures can also occur in a non-co-transcriptional manner. In recent years, R-loops have been reported to be involved in various cellular functions. However, when not properly regulated, they can compromise genomic DNA stability. R-loops play roles in gene expression, DNA repli

Microbial-derived mediators of the gut-brain axis: Implications for neurological disorders.

Gut-brain communication is mediated by metabolic and signaling pathways that play important roles in brain development and cognitive function. Recently, the bidirectional role of the gut microbiota in the gut-brain axis has drawn greater scientific attention. The gut microbiota, a group of bacteria that naturally colonize the gastrointestinal tract of mammals, may play a role in healthcare burden neurodegeneration, which affects many of the body's activities. Lately, it has been reported that some mediators produced by changes in the gut microbiota are associated with neurodegenerative disease

Leveraging the Electronic Medical Record for Functional Neurological Disorder: A Scoping Review.

BACKGROUND: Functional neurological disorder (FND) is a neuropsychiatric condition that causes disabling neurological symptoms. Despite its frequent occurrence, FND has a long history of misclassification, and this is reflected in inconsistencies in how healthcare providers code for FND patients in the medical record. OBJECTIVES: We aim to (1) assess if an electronic medical record (EMR) case definition(s) for FND or its subtypes has been created or validated, (2) explore what yet-to-be-validated definitions and codes have been used and (3) understand how the EMR is currently being used to st

Long-read genome sequencing enhances diagnostics of pediatric neurological disorders.

BACKGROUND: Singleton short-read genome sequencing (GS) is increasingly used as a first-line genetic test for childhood neurological disorders (such as intellectual disability, neurodevelopmental delay, motor delay, and hypotonia) with diagnostic yields from 26 to 35%, typically involving a mix of single nucleotide variants and small insertions/deletions (SNV/INDELs), structural variants (SVs), and short tandem repeats (STRs). Long-read GS is emerging as an attractive alternative, offering a more comprehensive assessment of the genome, but its utility still needs to be systematically evaluated

Bisphenol S and Neurological Health: An Integrated Overview of Neurotoxicity and Underlying Mechanisms.

Bisphenol S (BPS), a pervasive contaminant used in consumer and industrial products, has been widely detected in human serum, urine, hair, placenta, and breast milk. Although initially studied mainly as an endocrine disruptor, accumulating evidence indicates that BPS also exerts neurotoxicity by perturbing neuroinflammatory responses, neuroendocrine regulation, and neuronal development. However, its neurotoxic profile and mechanistic basis remain incompletely defined. This review systematically retrieved and synthesized epidemiological, animal, and cellular studies published between 2000 and F
