BACKGROUND: As compared with withholding parenteral nutrition (PN) for one week (late-PN), early supplementation of insufficient enteral nutrition with PN (early-PN) increased infection risk and delayed recovery of critically ill children in the PEPaNIC-RCT, with harm attributed to higher amino acid doses. We hypothesized that this harm could be explained by increased ureagenesis, revealing metabolic intolerance to enhanced feeding.
METHODS: This is a secondary analysis of the PEPaNIC-RCT (ClinicalTrials.gov-NCT01536275), in which patients (newborn-17 years) admitted to pediatric intensive ca

PURPOSE: Periodontitis manifests as a chronic inflammatory condition triggered by pathogenic microbial colonisation, and its pathogenesis involves the interaction of multiple factors such as oxidative stress, immune imbalance and abnormal bone metabolism. In recent years, the role of nutritional factors, especially vitamins, in regulating inflammation, oxidative stress and bone regeneration has gradually attracted attention, but their specific mechanisms and clinical application potential still need to be systematically explored. We hypothesised that vitamins A, C, D, E, and K confer protectio

Perinatal nutrition is vital for better maternal and child health outcomes. Biomedical guidelines recommend increasing caloric intake, avoiding toxins, and consuming a diverse, nutritious diet during pregnancy and postpartum. Yet, a significant gap persists between nutritional recommendations and actual practices in resource-limited settings. This gap is further complicated by cultural beliefs and traditional practices surrounding food and nutrition, which influence dietary habits. This study examines perinatal dietary practices in rural Bihar, India. We focused on three key areas: caloric ade

BACKGROUND: Throughout the world, burn injury is a major cause of death and disability. In resource-limited countries, burn injury is one of the leading causes of permanent disability among children who survive traumatic injuries, and burn injury is the fourth leading cause of disability worldwide. This study applied Andersen's model of health care access to evaluate if patient characteristics (predisposing factors), burn care service availability (enabling factors) and injury characteristics (need) are associated with physical impairment at hospital discharge for patients surviving burn injur

BACKGROUND: Nasogastric tube feeding (NG) has been widely used in patients with bulbar palsy after ischemic stroke but is associated with a significant risk of complications including malnutrition and pneumonia. Intermittent oro-esophageal tube feeding (IOE) can help alleviate these concerns. This study explored the clinical effect of IOE versus NG on nutritional status, swallowing function, stroke-associated pneumonia, and depression in patients with bulbar palsy after ischemic stroke.
METHODS: This randomized controlled study included 148 patients with bulbar palsy after ischemic stroke who

BACKGROUND: Addressing children's tobacco smoke exposure (TSE) remains a public health priority. However, there is low uptake and ineffectiveness of treatment, particularly in low-income populations that face numerous challenges to smoking behavior change. A multilevel intervention combining system-level health messaging and advice about TSE delivered at community clinics that disseminate the Special Supplemental Nutrition Program for Women, Infants and Children (WIC), combined with nicotine replacement and intensive multimodal, individual-level behavioral intervention may improve TSE control

Children with severe acute malnutrition, defined as weight-for-height <70% of the reference median or bilateral pedal oedema or mid-arm circumference <110 mm having complications, were managed in the Nutrition Unit of the Chittagong Medical College Hospital (CMCH) following the guidelines of the World Health Organization, with support from Concern Worldwide Bangladesh and ICDDR,B. In total, 171 children aged less than five years (mean +/- SD age 23.5 +/- 15.3 months) were admitted during June 2005-May 2006. Of them, 66% were aged less than two years, and 84.2% belonged to households with a mon

INTRODUCTION: Multimorbidity - the coexistence of multiple chronic conditions - is common among older adults with cognitive impairment but is rarely addressed in an integrated manner.
METHODS: We analyzed 17,915 adults aged 55 to 84 years (8997 receiving uMETHOD clinical decision-support care plans and 8918 from the National Health and Nutrition Examination Survey). The mean age was 69.0 years (SD 7.65; 95% CI: 68.86 to 69.1). Multimorbidity was quantified from diagnoses, labs, medications, vitals, and platform-inferred conditions. Associations with cognitive impairment were modeled using age

BACKGROUND: Renal impairment is common in patients receiving long-term home parenteral nutrition (HPN) for short bowel syndrome-associated intestinal failure (SBS-IF). While glucagon-like peptide-2 (GLP-2) analogues like teduglutide are effective in reducing parenteral support (PS) requirements, data on their use in advanced stage chronic kidney disease (CKD) or haemodialysis are scarce.
CASE REPORT: We describe a 66-year-old female with SBS-IF on HPN, complicated by hyperoxaluria and nephrocalcinosis contributing to stage 4 CKD. Initial renally-adjusted teduglutide dosing (0.025 mg/kg/day) e

Metabolic theories of various psychiatric disorders are generating growing interest within the medical community. These theories are supported by an emerging scientific literature suggesting a central role of neuronal energy dysfunction in diverse psychopathologies. In parallel, the use of metabolic interventions is gaining credibility in the management of psychiatric disorders such as bipolar disorder. We present here the case of a 34-year-old woman with a known diagnosis of bipolar disorder type II, mixed personality disorder, and alcohol and cannabis use disorder, who exhibits euthymia with

Colonic mucus forms a first line of defense against bacterial invasion while providing nutrition to support coinhabiting microbes in the gut. Mucus is composed of polymeric networks of mucin proteins, which are heavily modified post-translationally. The full compendium of enzymes responsible for these modifications and their roles in health and disease remain incompletely understood. Herein, we determine the biochemical function of NXPE1, a gene implicated in ulcerative colitis (UC), and demonstrate that it encodes an acetyltransferase that modifies mucin glycans. Specifically, NXPE1 utilizes

Progressive familial intrahepatic cholestasis type 2 (PFIC2) is a rare genetic disorder characterized by severe intrahepatic cholestasis, which often manifests in infancy with progressive liver dysfunction. We present the case of a 3-month-old infant with a one-month history of jaundice, vomiting, and bloody stools, presenting a unique set of diagnostic challenges. Initial clinical and laboratory findings indicated significant liver dysfunction, prompting further imaging and genetic analysis. An abdominal ultrasound revealed hepatomegaly with a coarse liver texture, while a hepatobiliary imino

There is growing evidence linking vitamin D deficiency with risk of COVID-19. It is therefore distressing that there is major disagreement about the optimal serum level for 25-hydroxyvitamin D (25(OH)D) and appropriate supplement dose. The UK Scientific Advisory Committee for Nutrition has set the lowest level for defining sufficiency (10 ng/ml or 25 nmol/L) of any national advisory body or scientific society and consequently recommends supplementation with 10 micrograms (400 IU) per day. We have searched for published evidence to support this but not found it. There is considerable evidence t

The European Society for Clinical Nutrition has published recommendations on the 'definition and classification of intestinal failure (IF)'. Two criteria must be present: a 'decreased absorption of macronutrients and/or water and electrolytes due to a loss of gut function' and the 'need for parenteral support'. Home parenteral support (HPS) is the primary treatment for chronic IF but is associated with complications. Areas covered: The principal cause of chronic IF is short bowel syndrome (SBS). The aim of treatment is to maximize intestinal absorption and reduce or eliminate the need for HPS

PURPOSE OF REVIEW: This article discusses the current state of the art in artificial intestine generation in the treatment of short bowel syndrome.
RECENT FINDINGS: Short bowel syndrome defines the condition in which patients lack sufficient intestinal length to allow for adequate absorption of nutrition and fluids, and thus need parenteral support. Advances toward the development of an artificial intestine have improved dramatically since the first attempts in the 1980s, and the last decade has seen significant advances in understanding the intestinal stem cell niche, the growth of complex p
