Unusual features in a case of snakebite, presumably due to a Cape cobra (Naja nigricollis).

A case of snakebite, thought to be due to a Cape cobra, in a young girl is described. Unusual features were a protracted period of paralysis and severe neurological residua in the form of hemiparesis and impaired vision. The ophthalmological features started as a bilateral optic neuritis and progressed to bilateral optic atrophy. It is difficult to ascribe all these effects to the neurotoxins of the Cape cobra as the neurotoxic features usually resolve rapidly if the patient survives. The actions of the neurotoxins, the quantity of antivenom to be administered in cases of cobra bite and the po

Chronic sixth nerve palsies. Are they really harbingers of serious intracranial disease?

Abducens palsies are encountered frequently in ophthalmologic practice and are usually benign transient deficits. When a sixth nerve palsy lasts longer than three months, conventional wisdom dictates an extensive investigation. In this report, we describe 38 patients with chronic, isolated sixth nerve palsies. The patient who is seen initially with an abducens palsy should undergo thorough medical and neurologic examinations to determine if the palsy is isolated. If it is isolated, the condition is not likely due to serious intracranial disease despite a duration of greater than three months.

Abnormal immune responses in the ocular presentation of Wiskott-Aldrich syndrome.

Wiskott-Aldrich syndrome is an x-linked disorder characterized by a triad of thrombocytopenia, eczema, and recurrent infections. Until recently, survival into the second decade was rare. In the last few years, a better understanding of the syndrome has been achieved, and earlier recognition and newer modes of treatment have improved life. These patients are living longer and ophthalmologic presentation reflects this change. We present three case histories of patients followed for the past 15 years who have had, in association with this syndrome, eczema of the eyelids, episcleritis, marginal ke

Follow-up study of phototreated fullterm newborns.

A follow-up study including examinations made by a paediatrician, an ophthalmologist and a psychologist and complemented with EEG-recordings and a number of laboratory determinations was performed at the age of 3 years on 41 children who had received phototherapy during the first week of life and on 42 control children of the same age. No differences were found in growth, development, social maturity, neurological or ophthalmologic disorders, EEG-recordings or laboratory determinations between both groups. It seems unlikely that phototherapy causes any harmful longterm effects provided that th

Parasellar tumors in children. I. Clinical presentation, preoperative assessment, and differential diagnosis.

This paper reviews the clinical presentation, neuro-ophthalmologic, endocrinologic, and radiographic findings of 74 children (0-20 years) harboring parasellar lesions who were treated on our service between 1968 and 1977. Our data support the conclusion that earlier diagnosis is occurring, although there is still a significant lag between onset of symptoms and treatment, especially in histologically benign lesions. The differential diagnosis is facilitated by recognition of key features of four major tumor types: craniopharyngioma, parasellar glioma, germinoma, and pituitary adenoma.

Combined nuclear and supranuclear defects in ocular motility. A clinicopathologic study.

A 65-year-old man suffered a midbrain infarct after coronary artery bypass surgery. He was left with a severe neuro-ophthalmologic deficit consisting of paralysis of upward and downward vertical gaze, weakness of adduction of the left eye, a dilated fixed left pupil, and partial right Horner's syndrome. He died 31 months after the episode. Postmortem examination disclosed an infarct involving parts of both oculomotor nuclei as well as supranuclear structures thought to be involved in the mediation of vertical eye movements.

[Vision against the light as an aid to indication for cataract operation (author's transl)].

The examination of visual acuity against the light may help the decision, whether or not a senile cataract should be operated on. We examined 84 eyes with different lens opacities, the visual acuity was analysed in relation to the axial, cortical and capsular cataract. We found in all cases a marked reduction of the visual acuity against the light. This has been general ophthalmological experience especially in the capsular cataract. This additional examination, we propose, is easily performed, and was given helpful information for all kinds of cataracts, where the indication for surgery was n

Biological cleansing: Toward improving ocular surface surgical outcomes.

Postoperative complications such as dry eye, ocular surface pain, and abnormal wound healing responses, including fibrosis or ectasia, remain a significant concern in corneal, refractive, and cataract surgeries as they occur even in those patients without the classical clinical risk factors. Persistent low-grade or subclinical ocular surface inflammation in these patients may have contributed to these suboptimal outcomes. Inflammatory factors such as IL-1β, IL-6, IL-17A, TNFα, IFNγ, and/or MMP-9 are reported to be higher on the ocular surface of patients with either overt or subclinical inflam

Surgical Correction for Pediatric Epiblepharon and Trichiasis.

Lower eyelid epiblepharon is a condition that is common in Asian children and improves with age in most patients; however, the problem can persist until school age or even adulthood. No difference has been observed in terms of incidence in association with sex or the affected eye, and the condition mainly affects the lower eyelid, usually in both eyes. The condition typically features horizontal semilunar skin folds lying parallel to the palpebral margin that accumulate and push the eyelashes towards the ocular surface, which rub against the cornea, resulting in corneal injury and irritation s

Comparison of Cochet-Bonnet, Dental Floss, and Cotton Wisp With Applications to Corneal Sensation Testing.

PURPOSE: The measurement of corneal sensation can be useful in the diagnosis of corneal neuropathies. Although the Cochet-Bonnet (CB) esthesiometer is the gold standard, cotton wisp (CW) and dental floss are commonly used in clinical practice. METHODS: We assessed the steady-state force of thick dental floss (tkDF), thin dental floss (tnDF), and CW compared with that of CB. We surveyed cornea/refractive specialists (CRS) and noncornea/refractive specialists (NCRS) for frequency and method by which corneal sensation was checked. RESULTS: Compared with CB, the maximum force applied by tkDF was
