Distal Symmetric Polyneuropathy: A Review.

IMPORTANCE: Peripheral neuropathy is a highly prevalent and morbid condition affecting 2% to 7% of the population. Patients frequently experience pain and are at risk of falls, ulcerations, and amputations. We aimed to review recent diagnostic and therapeutic advances in distal symmetric polyneuropathy, the most common subtype of peripheral neuropathy. OBSERVATIONS: Current evidence supports limited routine laboratory testing in patients with distal symmetric polyneuropathy. Patients without a known cause should undergo a complete blood cell count, comprehensive metabolic panel, vitamin B12 m

A rapid, novel method of volumetric assessment of MRI-detected subchondral bone marrow lesions in knee osteoarthritis.

PURPOSE: To assess reliability and validity of a semi-automated quantitative method for osteoarthritis (OA)-related bone marrow lesion (BML) assessment in the femur and tibia. METHODS: In a cross-sectional study of subjects with knee OA, we examined concurrent criterion and clinical validation of a novel method of semi-automated quantitative BML measurement. The primary outcome was total segmented BML volume in femoral and tibial medial and lateral knee compartments. Criterion validation was examined through comparison of BML volumes with Whole-Organ Magnetic Resonance Imaging Score (WORMS) s

Pressure-induced pain on the tibia: an indicator of low bone mineral density?

Previous literature investigating bone pain in osteoporosis has prominently focused on painful conditions following osteoporotic fractures. "Is osteoporosis really a silent disease without bone pain and tenderness unless a fracture occurs?" Our aim in this study was to answer the question by assessing the questionable tenderness on tibia bones of fracture-free patients with low bone density and to compare the findings with a normal population. One-hundred-thirty-three consecutive postmenopausal female patients with the mean age of 56 years admitted to our clinic for bone mass measurement were

Validation of the 10-Item Lower Extremity Functional Scale (LEFS-10) for Individuals With Knee Osteoarthritis.

BACKGROUND: Functional capacity assessment in knee osteoarthritis (KOA) is essential because it relates to pain, disability, and quality of life. Reliable, sensitive, and validated tools are needed to measure lower limb function. PURPOSE: To validate the Brazilian Portuguese version of the 10-item Lower Extremity Functional Scale (LEFS-10) for people with KOA. METHODS: A cross-sectional validation study involved 100 participants with KOA. The agreement between the 10-item (LEFS-10) and 20-item (LEFS-20) scales was examined using Spearman's correlation coefficient (rho) between the LEFS-10 an

Guide to lower extremity radiologic measurements: part 1 hip.

Potential etiologies of hip pain, especially in younger patients with acetabular dysplasia and various impingement syndromes, or in older patients following arthroplasty can be subtle radiographically, and quantitative assessment on standardized imaging is often performed to confirm the diagnosis and help clinicians choose between conservative or surgical management. As hip preservation and joint replacement surgical techniques have become more sophisticated and widespread, the number of measurements reported in the literature to detect subtle abnormalities has greatly increased, and measureme

Psychometric Evidence on the Tampa Scale for Kinesiophobia for Temporomandibular Disorders: A Scoping Review.

Temporomandibular disorders are frequently associated with orofacial pain, functional limitations, and psychosocial factors that may contribute to persistent symptoms. Kinesiophobia, or fear of movement, may influence mandibular function and rehabilitation outcomes. This scoping review aimed to map the available evidence on the psychometric properties and cross-cultural adaptations of the Tampa Scale for Kinesiophobia for Temporomandibular Disorders (TSK-TMD) and to identify gaps in the current literature. Literature searches were conducted in PubMed, Scopus, and the Web of Science Core Collec

Validity of IMUs in Comparison to a Marker-Based-Motion Capture System for Spatio-Temporal Parameters During Wheelchair Propulsion.

BACKGROUND: Manual wheelchair propulsion is often associated with pain in the upper extremities. Recording spatio-temporal parameters can optimize movement patterns and prevent injuries. This study compares a marker-based camera system with inertial measurement units to validate their use in wheelchair propulsion on a test stand. METHODS: Spatio-temporal parameters of 27 manual wheelchair users propelling at three self-selected speeds (slow, normal, fast) were simultaneously recorded using a marker-based camera system and inertial measurement units, and subsequently compared between both syst

Congenital Chloride Diarrhea: Diagnosis by Easy-Accessible Chloride Measurement in Feces.

Background. Congenital chloride diarrhea (CCD) is an autosomal recessive disorder caused by mutations in the genes encoding the intestinal Cl(-)/HCO3 (-) exchanger and is clinically characterized by watery, profound diarrhea, electrolyte disturbances, and metabolic alkalosis. The CCD diagnosis is based on the clinical symptoms and measurement of high chloride concentration in feces (>90 mmol/L) and is confirmed by DNA testing. Untreated CCD is lethal, while long-term clinical outcome improves when treated correctly. Case Presentation. A 27-year-old woman had an emergency caesarian due to pain

Validity of clinical measures of shoulder subluxation in adults with poststroke hemiplegia.

OBJECTIVES: Shoulder subluxation after cerebrovascular accident has been associated with chronic pain, orthopedic complications, peripheral nerve damage, and autonomic dysfunction. Clinical diagnosis and gradation of subluxation is problematic due to the lack of precision in frequently used measures. The objective of this study was to assess the validity of clinical techniques used to assess inferior subluxation of the hemiplegic shoulder by comparing these techniques with radiographic measurement. METHOD: In 20 male subjects with hemiplegia, the presence, type, and degree of subluxation was

Relationship Between Patient-reported Outcome Measures and Accelerometer-measured Physical Activity and Sleep in Patients With Postherpetic Neuralgia.

OBJECTIVE: Available treatment for chronic neuropathic pain is still limited, and the positive effects are modest. Thus, clinicians aim to improve activity and quality of life despite pain. The objective monitoring of activity is attracting attention in chronic pain assessments. Therefore, we objectively evaluated daytime activity and sleep in patients with postherpetic neuralgia (PHN), using actigraphy to determine risk factors for decreased activity. METHODS: Participants with PHN wore an actigraph (a wristwatch-like accelerometer) on the nondominant hand. The actigraph measured day-time ac

Lateral center-edge angle in femoroacetabular impingement: from the sourcil or the rim of the acetabulum?

The correlation between clinical outcomes and preoperative/postoperative measures of the lateral center-edge angle (LCEA) will help establish the cutoff values for this measurement and determine whether to obtain it from the lateral acetabular rim (LCEAR) or the lateral end of the sourcil (LCEAS). The hypothesis was that the LCEAS would be more sensitive than the LCEAR. An upper cutoff value of LCEA could predict better functional outcomes in FAI patients. 106 patients (103 unilateral, 3 bilateral) who underwent hip arthroscopy surgery due to FAI and had a minimum 2-year follow-up were include

Effect of Age on Complication Rates and Outcomes Following First Metatarsophalangeal Arthrodesis for Hallux Rigidus.

BACKGROUND: Hallux rigidus is one of the most common toe pathologies in patients greater than 50 years old. Although metatarsophalangeal (MTP) arthrodesis is used to improve pain and function, older patients are often considered to be higher-risk operative candidates. There are minimal data on outcomes of MTP fusion and no studies comparing outcomes between geriatric and younger patients. METHODS: This is a single-center, prospective study of all patients who underwent MTP fusion between August 1, 2015, and July 1, 2018. Patient-reported outcomes were assessed using the Short Form Health Surv
