Bottlenecks in cardiovascular prevention: Can artificial intelligence close the gap?

Cardiovascular diseases are a leading cause of morbidity and mortality in France, despite current preventive strategies and evidence-based therapies. Artificial intelligence (AI) has emerged as a promising approach to overcoming several of these barriers, with the potential to improve cardiovascular prevention across the entire spectrum of care. Upstream, AI can enhance risk stratification through the integration of clinical, biological, imaging, environmental, behavioural and longitudinal data, thereby moving cardiovascular prevention toward an AI-augmented precision medicine paradigm, in whi

Lifestyle and BrainAGE in adult depression.

This study investigates how heterogeneity in lifestyle and physical fitness shapes patterns of brain ageing in depression. The study sample included medically healthy adults from the UK Biobank (44-82 years) with a history of depression (n = 896) and non-psychiatric controls (n = 36,206). Heterogeneity through Discriminative Analysis (HYDRA) was used to derive data-driven subgroups within the depression sample based on 224 behavioural and physical health features. Global and voxel-based brain-age-gap-estimates (brainAGE) computed from structural neuroimaging data were used for personalized ass

The intratumoral microbiome in breast cancer: an actionable component of the tumor immune microenvironment.

Breast cancer remains one of the leading causes of cancer-related deaths in women. The discovery of a breast tumor microbiota has deeply changed our understanding of the breast tumor microenvironment, identifying microorganisms as active players in tumor progression. Increasing evidence suggests that intratumoral bacteria may influence multiple hallmarks of breast cancer, including chronic inflammation, immune modulation, metabolic reprogramming, epithelial plasticity, metastatic dissemination, and therapeutic response. Distinct microbial communities have been associated with specific breast c

Psychological Process-Based Profiles in the Binge-Eating Spectrum: Clinical Implications Beyond DSM-5 Subtypes.

BACKGROUND AND OBJECTIVES: Binge-spectrum eating disorders (BSEDs) are clinically impactful, yet the psychological mechanisms underlying their persistence and heterogeneity remain insufficiently understood. Clarifying these mechanisms is essential for refining diagnostic models and improving treatment. This study focused on young to early-middle adults, a developmental period with elevated risk for the onset and aggravation of BSED, to identify empirical subgroups based on eating- and psychological-related variables. METHODS: A two-step cluster analysis was performed in 187 patients (men and

"Attention to the person who prepared the questionnaire": Queer men's survey marginalia in HIV behavioural surveillance.

Survey marginalia are the unsolicited annotations left by participants in the margins of questionnaires. Marginalia can offer valuable insight into how participants perceive study instruments and researchers, the meanings behind their responses, and add nuance to lived experiences that are otherwise obscured or unaccounted for in survey design. This paper presents a qualitative analysis of survey marginalia left by participants of a pen-and-paper HIV behavioural surveillance survey administered in person to sexual minority men at 2S/LGBTQ+ pride events across Canada. Using interpretive descrip

Vidas en Contexto: Unpacking Social Determinants of Health and Related Health Outcomes among Puerto Ricans through a Person-Centered Approach.

This study used a person-centered approach to uncover patterns of Social Determinants of Health (SDOH) among adults aged 18 and older residing in Puerto Rico. This research addresses a knowledge/population gap relating to the co-occurrence of SDOH among Puerto Ricans who may experience unique patterns of unmet social needs associated with the island's colonial status. Data were drawn from the 2023 Behavioral Risk Factor Surveillance System survey, which included respondents residing in Puerto Rico (n = 4,594), of whom the largest age group was aged 65 and older (26.9%). Latent class analyses u

Interindividual variability in platelet aggregation among aspirin-treated patients: a large retrospective analysis.

Interindividual variability in response to aspirin remains a major challenge in antiplatelet therapy. Identification of patients with high platelet reactivity (HPR) and optimal or low platelet reactivity remains challenging and may be influenced by both biological and methodological factors, including interlaboratory variability. We report results of a large cohort of aspirin-treated patients to assess the gold standard light transmission aggregometry (LTA). We retrospectively analyzed 3065 patients treated with aspirin (ASA; 75-150 mg/d) who sequentially underwent LTA assessment using multipl

When two worlds collide - the experiences of academic and Local Authority teams in implementing a cluster randomised control trial to test the impact of the MapMe tool on child healthy weight maintenance: a qualitative study.

AIMS: Little is known about Local Authority (LA) teams' involvement in and delivery of research trials. The National Childhood Measurement Programme (NCMP) measures the weight status of 4- to 5- and 10- to 11-year-olds and communicates the results to parents, via letter. However, parents can be surprised by these results. The MapMe2 intervention is a web-based tool delivered by LAs to improve parental acknowledgement and understanding of childhood overweight and obesity. This article focuses on the practicalities and challenges of the research team and LA stakeholders working together. METHOD

A wearable accelerometry landscape of human health and disease.

Digital wearable devices provide objective characterization of human behaviors in real-world settings, yet their implications for health and diseases remain incompletely understood. Here, we analyzed a large-scale accelerometry-based cohort, including 70,473 participants with up to 10 years of follow-up, yielding a total of 11.8 million person-hours of data. From this cohort, we constructed a comprehensive atlas of 210 accelerometer-derived phenotypes (ADPs), encompassing physical activity, sedentary behavior, sleep, circadian rhythm, step counts, and step intensity. The study identified 8149

Flexible minds, better decisions: a cross-sectional study on cognitive and emotional correlates of decision-making in law school.

BACKGROUND: Although decision-making styles (DMSs) are fundamentally complex, multidimensional, and contextual phenomena, they are systematically filtered through individuals' cognitive and emotional antecedents. In the high-stakes and analytical environment of legal education, identifying these structural predictors is critical to mitigating maladaptive judgment patterns. While existing literature globally links Cognitive Flexibility (CF) and Emotional Intelligence (EI) to adaptive reasoning, there remains a significant empirical gap regarding how the sub-dimensions of CF (alternatives, contr

Global economic impacts of Highly Pathogenic Avian Influenza: A systematic review and impact framework.

Highly Pathogenic Avian Influenza (HPAI) has risen globally since the early 2000s, causing economic impacts on industries, government, ecosystems, and society. A systematic literature review (SLR) following PRISMA guidelines was conducted to document and quantify the economic and financial impacts of HPAI. Data on economic methods, impact parameters and control strategy assessments were extracted. Impact estimates were converted into 2024 values and expressed as impact per agricultural GDP, per capita and average annual income. A framework categorising impact types was developed and quality wa

Bacterial clusters are associated with the risk of severe disease progression in inflammatory bowel disease irrespective of conventional disease categories.

Background: Inflammatory bowel diseases (IBDs) are complex conditions marked by chronic inflammation in the gastrointestinal tract. Traditional classification separates IBD into Crohn's disease and ulcerative colitis, but this division may not fully capture disease heterogeneity. Here, we examine whether microbiome-driven subtyping can describe novel clinical IBD phenotypes. To achieve this, we applied unsupervised clustering to fecal microbiota profiles from the population-based Inflammatory Bowel Disease in South-Eastern Norway III (IBSEN III) cohort. Methods: A Gaussian Mixture Model (GMM)

Pulmonary fibrosis through patients' eyes: an interpretative phenomenological analysis of illness perceptions and acceptance.

BACKGROUND: Idiopathic pulmonary fibrosis (IPF) is a progressive, life-limiting lung disease that causes substantial morbidity. While there is growing recognition of the importance of symptom management and quality of life, little is known about how individuals with IPF perceive their illness, manage emotional responses, and form beliefs about their condition. OBJECTIVES: The main aim was to explore how people with IPF perceive and anticipate their illness. A secondary aim was to identify key factors-both facilitators and barriers-that affect these perceptions and influence expectations about
