Understanding the sleep problems of people with dementia and their family caregivers.

Sleep disturbances are common with dementia and can adversely affect waking function. However, the perspectives of people with dementia and their family caregivers concerning their sleep are under-researched. We conducted three focus groups with 12 community-dwelling pairs (a person with dementia and their family caregiver). Discussions addressed sleep disturbances, coping strategies, and beliefs and attitudes surrounding sleep. Thematic analysis indicated that dementia-related sleep disturbances were common, including confused awakenings and dementia-related behaviors at night, changes to sle

Exploring the effect of foot massage on agitated behaviours in older people with dementia: a pilot study.

AIM: To explore the effects of foot massage on agitated behaviours in older people with dementia living in long-term care. METHODS: Seventeen men and 5 women (mean age 84.7 years), with a diagnosis of dementia and a history of clinically significant agitation, received a 10-minute foot massage each day for 14 days. The short form of the Cohen-Mansfield Agitation Inventory (CMAI-SF) and the Revised Memory and Behavior Problems Checklist (RMBPC) were completed at baseline, post-test and 2-weeks follow up. RESULTS: CMAI-SF and RMBPC scores were significantly reduced at post-test and remained si

Alexithymia, human relationships, and mobile phone use.

Alexithymia is a personality trait associated with difficulties in identifying feelings, difficulties in describing feelings to other people, constricted imaginal processes, and an externally oriented cognitive style. It has been found to be associated with personality features that may cause interpersonally avoidant behavior and other interpersonal problems. The present study explored, in a sample of primary care patients (N = 491), whether alexithymia is associated with mobile phone usage, and whether the perceived quality and quantity of human relationships mediate its effect. Even controll

Common factors of cognitive therapy and interpersonal psychotherapy in the treatment of social phobia.

The comparison of active ingredients across different psychotherapeutic approaches is impeded by the specificity of the theoretical concepts explaining change mechanisms. This study aims to shed light on effective components of the therapeutic process in a trial comparing cognitive therapy (CT) and interpersonal psychotherapy (IPT) in the treatment of social anxiety disorder (SAD). Sixty-two outpatients with SAD were included in the study. Patients and therapists rated common efficacy factors after each session according to the generic model developed by Grawe (2004). Results show significant

Multi-informant reports of psychiatric symptoms among high-functioning adolescents with Asperger syndrome or autism.

The aim of the study was to examine psychiatric symptoms in high-functioning adolescents with autism spectrum disorders reported by multiple informants. Forty-three 11- to 17-year-old adolescents with Asperger syndrome (AS) or high-functioning autism (HFA) and 217 typically developed adolescents completed the Youth Self-Report (YSR), while their parents completed the Child Behavior Checklist (CBCL). Teachers of adolescents with AS/HFA completed the Teacher Report Form (TRF). The informants reported significantly more psychiatric symptoms, especially withdrawn, anxious/depressed, social and att

Loneliness as the cause and the effect of problematic Internet use: the relationship between Internet use and psychological well-being.

The current research started from the assumption that one of the major motives driving individuals' Internet use is to relieve psychosocial problems (e.g., loneliness, depression). This study showed that individuals who were lonely or did not have good social skills could develop strong compulsive Internet use behaviors resulting in negative life outcomes (e.g., harming other significant activities such as work, school, or significant relationships) instead of relieving their original problems. Such augmented negative outcomes were expected to isolate individuals from healthy social activities

An overview of genetic influences in alcoholism.

This review summarizes recent findings from human research regarding genetic influences in alcohol abuse and dependence. Genes explain about 50% of the vulnerabilities leading to heavy drinking and associated problems. Most genetic influences appear to impact at least four prominent intermediate characteristics (phenotypes) that interact with environmental events to produce the alcoholism risk: a flushing response to alcohol; a low level of response to alcohol; personality characteristics that include impulsivity, sensation seeking, and neuronal and behavioral disinhibition; and through psychi

Weekly problems scales: instruments for sexually abused youth and their nonoffending parents in treatment.

This study's purpose was to determine if efficient measures could be created to assess multiple problematic behaviors identified in youth who were sexually abused and in treatment. Because of the lack of easily administered brief instruments that assess multiple domains of interest in this population, complementary parent and child assessment measures were developed. The Weekly Problems Scale-Child Version (WPSC) and the Weekly Problems Scale-Parent Version (WPS-P) were created to monitor the weekly progress of the child and family in treatment and focus specifically on common areas of difficu

Predictors of use of services among dementia caregivers.

Caregivers of persons with dementia do not use community resources until late in the disease process, despite the fact that judicious use of community resources can delay nursing home admission. Data from the National Caregiver Training Project, based on Hall and Buckwalter's (1987) progressively lowered stress threshold (PLST) model, were used to examine variables related to use of community resources. Spouse and adult child caregivers were divided into two groups based on amount of community resources used per week. Within this geographically diverse sample of caregivers, 64% did not use pro

Do street youths' perceptions of their caregivers predict HIV-risk behavior?

This study examined street youths' perceptions of their caregivers and the association between these perceptions and HIV-risk behavior in a random probability sample of 715 12- to 23-year-old street youths from Los Angeles and San Diego, CA (mean age, 18.7 years). All participants had been homeless at some point during the past 12 months, with 70% recruited from nonshelter sites. Although youths reported high rates of hostility, unavailability, substance use, and legal problems among their caregivers, 86% reported that their caregivers had at least one attribute associated with support. Falls associated with dementia: how can you tell?

In any practice setting, professional nurses must be skilled to assess and manage medical problems rapidly. Post-fall assessment among those with dementia is a daunting task. Emergent medical problems after a fall are difficult to detect among those with dementia, because impaired communication portrays a false reality that the older adult is uninjured. Furthermore, early detection of potential post-fall injuries may not occur within a health care system that relies on verbal communication and eyewitness accounts as the main source of fall occurrence information. Professional nurses must turn

Abuse, addiction, and depression as pathways to sexual risk in women and men with a history of substance abuse.

PURPOSE: In this paper, we develop and test a model for predicting sexual risk for HIV and other STDs. METHODS: Researchers interviewed 528 women and men with a history of substance abuse about their past experiences of physical and sexual abuse, symptoms, and sexual risk behavior (number of partners, trading sex for drugs or money, unprotected intercourse, and sexual orientation). The model explores direct and indirect associations of physical abuse and sexual abuse with sexual risk. RESULTS: Depression and severity of problems with drugs and alcohol were expected to mediate associations be

Prediction of intravenous drug use.

Information about lifetime intravenous drug use (IVDU) was obtained from 1,062 relatives of hospitalized alcoholics, felons, and control subjects, of whom 92 had a history of IVDU and an additional 230 had a history of substantial, but non-IV drug use. The IVDUs were significantly more likely than all other subjects to have reported experiencing numerous behavioral and social difficulties relating to conduct difficulties, early initiation of substance use, and disrupted living situation before the age of 15. They were also more likely to have exhibited conduct problems than other drug users. I

Factors related to seroconversion among homo- and bisexual men after attending a risk-reduction educational session.

Thirteen homosexual men, volunteers in a study of the natural history of HIV, who seroconverted to HIV after participating in an educational program on HIV prevention, were interviewed about the circumstances leading to their seroconversion. Six men had participated in unprotected anal intercourse with at least one partner whom they believed was HIV-negative. Four men attributed their conversion to mental health problems or to drug and alcohol use. Two men's seroconversions could not be ascertained and one man attributed seroconversion to a condom break. Most men who had learned how to avoid i

Practical management of urinary incontinence.

A comprehensive evaluation of incontinence using office-based techniques may discover reversible problems such as delirium, urinary infections, vaginitis, depression, drug effects, endocrinologic causes, immobility, or impaction. Treatment of these causes may alleviate incontinence. Bedside urodynamics are a sensitive, specific, and useful office-based technique that can identify detrusor instability, overflow incontinence, or stress incontinence. Low urinary flow rate, difficulty with catheterization, slow bladder filling or severe urgency, bladder capacity over 600 ml, and post-void residual

Disclosure of traumas and psychosomatic processes.

Results from a series of studies are summarized in support of a general theory of inhibition and psychosomatics. According to this view, to inhibit thoughts, feelings, or behaviors is associated with physiological work. In the short term, inhibition results in increased autonomic nervous system activity. Over time, inhibition serves as a cumulative stressor that increases the probability of psychosomatic disease. Actively avoiding thoughts and feelings surrounding a trauma and/or not discussing a trauma is a particularly insidious form of inhibition. The results from recent surveys and experim
