Information needs in parents of children with a rheumatic disease.

BACKGROUND AND OBJECTIVE: Incorporating the patient's perspective and expectations into the delivery of health care has become an important indicator of today's quality of medical care. Our aim was consequently to explore the information needs of parents of children with juvenile idiopathic arthritis and other rheumatic diseases. MATERIALS AND METHODS: Cross-sectional, anonymous survey using a purpose-designed questionnaire, which separately assessed sources of information and topics. With respect to sources, we also asked about their degree of helpfulness, and regarding topics, we also asked

Hypnosis for treatment of insomnia in school-age children: a retrospective chart review.

BACKGROUND: The purposes of this study are to document psychosocial stressors and medical conditions associated with development of insomnia in school-age children and to report use of hypnosis for this condition. METHODS: A retrospective chart review was performed for 84 children and adolescents with insomnia, excluding those with central or obstructive sleep apnea. All patients were offered and accepted instruction in self-hypnosis for treatment of insomnia, and for other symptoms if it was felt that these were amenable to therapy with hypnosis. Seventy-five patients returned for follow-up

Family members serving the gestational carrier needs of women lacking a uterus: a report of two cases.

CASE REPORT: In two instances, a designated relative served as a gestational carrier for a woman who no longer had a uterus. In the first case, a 38-year-old woman underwent an emergency cesarean hysterectomy due to worsening fatty degeneration of the liver in the third trimester, culminating in massive hemorrhage, consumptive coagulopathy and the loss of twins at 28 weeks gestation following successful in vitro fertilization. In the second case, a 28-year-old had congenital absence of the uterus and vagina. Both surrogates volunteered for service and were carefully screened medically, reprodu

Work status as a predictor of surgical outcome of discogenic low back pain.

STUDY DESIGN: Prospective observational study. OBJECTIVES: To evaluate the role of work status as a predictor of outcome from anterior lumbar fusion. SUMMARY OF BACKGROUND DATA: Many psychosocial factors have been identified as predictors of chronic disability and of outcomes of surgery. Workers' Compensation and job satisfaction are two of the strongest and most evaluated factors. Work status at the time of intervention may also be relevant but has rarely been studied independently in patients having lumbar fusion. METHODS: A total of 106 patients with discogenic low back pain were treated

Optimizing the assessment of quality of life after laryngeal cancer treatment.

Reports of the impact of larynx cancer treatment modality on quality of life are conflicting, in part due to varying study methodology. The aims of this study were to (1) provide preliminary comparisons of quality of life following radiotherapy or combination therapy; (2) evaluate a number of measures of quality of life and thereby (3) inform future prospective studies. Thirty-six laryngeal cancer patients, 24 following radiotherapy, 12 following radiotherapy and laryngectomy completed the Functional Assessment of Cancer Therapy (FACT) - General/Head and Neck subscale; Nottingham Health Profil

Menopause.

Elevating women from the nadir of ovarian hypofunction has been a major driving force in developing hormonal strategies for the management of menopause. As indicated by recent evidence, however, this may have resulted in unacceptable morbidity in several women. Likewise, the use of menstrual cessation as the hallmark of menopause may have served the counterproductive effect of delaying the onset of appropriate preventive pharmacologic and non-pharmacologic strategies until the later years of life. Preventive and therapeutic strategies that target the menopausal phase of life exclusively are gr

Cancer patients in pain: considerations for assessing the whole person.

Pain is a subjective perception that is influenced by psychosocial and behavioral factors and physical pathology. In cancer, the source of the pain may be the disease itself, the treatment, or co-occurring pain syndromes. Often, cancer is a progressive disease, and pain may be marked by exacerbations, additional treatment, and remissions. Thus, pain assessment must become part of routine care. Ratings of pain should be performed on a regular basis, just as vital signs are taken on a regular basis. Unlike the other vital signs, however, pain can only be assessed by the patients' verbal and nonv

Which factors determine the long-term outcome among patients with a very small or unconfirmed AMI.

AIM: To describe various factors associated with the very long-term prognosis for patients with a very small or an unconfirmed acute myocardial infarction (AMI). METHODS: Patients below 76 years of age, hospitalized due to suspected AMI who either developed a very small AMI (enzyme elevation<twice upper normal limit and maximum serum (S) aspartate aminotransferase (S-ASAT)<1.4 ukat/l) or an unconfirmed AMI (a suspected ischemic event with no signs of myocardial necrosis) were evaluated at our out-patient clinic. The 10-year mortality was related to the clinical history, age and sex, metabolic

Age-related differences in the quality of life of breast carcinoma patients after treatment.

BACKGROUND: The objective of this study was to compare the quality of life (QOL) of younger (< or =50 years) versus older (>50 years) women on recent completion of treatment of breast carcinoma. METHODS: Data reported herein were obtained from a baseline assessment of 304 breast carcinoma patients. These patients were enrolled in a multiinstitutional, randomized trial testing a psychosocial telephone counseling intervention for breast carcinoma patients immediately after treatment. The assessment was made using a self-administered (mail) questionnaire, with an overall response rate of 86%. In

Repetitive strain injury. 2. Diagnostic and treatment tips on six common problems. The Goff Group.

Repetitive strain injury is caused by recurrent overuse, resulting in microtrauma to tissues. Local pain and tenderness, weakness, inflammation, and limited function are common findings. Some of the strain injuries seen most often are carpal tunnel syndrome, trigger finger, shoulder impingement syndrome, tennis elbow, thoracic outlet syndrome, and myofascial pain disorders. Often, treatment can be started at the initial visit, after systemic disorders have been ruled out. A vital step is elimination of aggravating factors, such as improper posture, inadequate attention to ergonomic factors at

Disadvantage as a measure of handicap: a paired sibling study of disabled adults in Lebanon.

In 1980 WHO defined disability as a functional limitation due to impairment, and handicap as the psychosocial disadvantage consequent to disability. This study was designed to investigate the advent of handicap in a group of adults physically disabled by poliomyelitis in childhood by comparing them to their age- and sex-matched siblings. An area survey was conducted in West Beirut and its Southern Suburb and 240 such disabled people and their siblings were identified and interviewed. Handicap was defined as disadvantage in six areas, namely, education, work, income, marital status, housing, an

Stress and hypertension--a critical review.

Recurrent psychosomatic stress has been popularly thought to cause permanent hypertension by repeated reactive blood pressure elevations. These are considered by some to be caused by various mechanisms, including an increased sympathetic outflow and a decreased parasympathetic inhibition, or possibly an inborn functional abnormality in the walls of the resistance vessels, which becomes a structural abnormality as a result of the elevated pressure. Recent work, however, has questioned some of these concepts. Many of the assumptions in the past concerning the ill-effects of elevated blood pressu

Chronic arthritis in children. Juvenile rheumatoid arthritis.

Juvenile rheumatoid arthritis (JRA) is best defined as the condition of chronic synovitis in children. Such chronic childhood arthritis probably includes several distinct disease processes. Recognizable subgroups are systemic-onset disease (20%), rheumatoid factor-negative polyarthritis (25%), rheumatoid factor-positive polyarthritis (5%), pauciarthritis associated with antinuclear antibodies and chronic iridocyclitis (30%-35%), and pauciarthritis associated with sacroiliitis and HLA-B27 (10%-15%). Rheumatoid factor-positive polyarthritis appears to be the childhood equivalent of classic adult

Social isolation, loneliness, disability progression, and mortality among older adults with cardiac disease: a longitudinal NHATS study.

BACKGROUND: Cardiac disease is the leading cause of morbidity and mortality among older adults, yet the psychosocial pathways through which it accelerates functional decline remain poorly characterized. Social isolation and loneliness are plausible mediators, but their longitudinal roles in the cardiac disease-disability-mortality cascade have not been examined in an integrated framework. METHODS: We analyzed 5,654 US Medicare beneficiaries aged ≥65 from the National Health and Aging Trends Study (NHATS) followed across Rounds 7-14 (2017-2024). Cardiac disease was ascertained at baseline via

Parental Aesthetic Anxiety, Disability Aesthetics, and the Ethics of Surgical Normalization in Congenital Hand Differences.

Children born with congenital hand differences (syndactyly, fusion of adjacent digits; polydactyly, duplication of digits; radial club hand, partial or complete absence of the radius; and symbrachydactyly, short, webbed, or absent fingers) occupy a unique and underexamined position at the intersection of pediatric surgery and disability studies. While surgical literature has meticulously documented operative techniques, functional outcomes, and complication profiles, it has almost entirely neglected the aesthetic and cultural dimensions of the decision to operate. This narrative review address

Low intensity extracorporeal shockwave therapy for erectile dysfunction: a study in an Indian population.

INTRODUCTION: Erectile dysfunction (ED) has been shown to be associated with a number of physical conditions and affects not only physical but also psychosocial health. Currently oral, on-demand phosphodiesterase type 5 inhibitors (PDE5i) are preferred first line treatment. Though effective, these drugs have limitations and are associated with significant non-compliance, side effects and do not reverse the underlying pathology. Non-invasive low intensity shockwave therapy (LISWT) has been shown to significantly improve erectile function in men previously PDE5i dependent. We describe our experi

Therapeutic Improvement in People With Schizophrenia Undergoing tACS/CBTp (Transcranial Alternating Current Stimulation/Cognitive Behavioral Therapy for Psychosis) Associated With Usual Medication Regimen: Protocol for a Pilot, Randomized, Single-Blind Trial.

BACKGROUND: Transcranial alternating current stimulation (tACS) applies a low-intensity sinusoidal electrical current through electrodes placed on the scalp to boost the brain's own oscillation by way of entrainment. When a single frequency is applied, this exogenous oscillation synchronizes with the brain's endogenous frequency. Gamma frequency synchrony stands out as a binding mechanism for integrating disparate brain networks, mediating perception, cognition, and memory, typically disturbed in schizophrenia. The treatment of schizophrenia includes medication and cognitive behavioral therapy

Efficacy of transcranial magnetic stimulation in the treatment of combat-related PTSD: a systematic review and meta-analysis.

INTRODUCTION: Combat-related post-traumatic stress disorder (PTSD) remains highly prevalent among military personnel and veterans and is frequently chronic, disabling, and only partially responsive to first-line pharmacological and psychotherapeutic interventions. Given the central role of fronto-limbic circuit dysfunction in PTSD, transcranial magnetic stimulation (TMS) has emerged as a biologically plausible neuromodulatory strategy, yet its protocol-level efficacy in combat-exposed populations is not well established. Clarifying whether specific TMS modalities offer clinically meaningful be
