[Assessment of right ventricular afterload in mitral valve diseases with radionuclide angiography].

Right ventricular function at rest and during exercise was studied in 33 patients with mitral valve disease by equilibrium gated radionuclide angiography using 99mTc in vivo labeled red blood cells. Radionuclide measurements of right ventricular ejection fraction (RVEF) were correlated with mean pulmonary arterial pressure (mPAP). RVEF decreased significantly with exercise. There was no significant correlation between RVEF at rest and mPAP. However, mPAP showed significant negative correlation with RVEF during exercise and with the changes of RVEF from rest to exercise. It is concluded that RV

Prevention of infective endocarditis: the view from the United States.

The American Heart Association's recommendations for the prevention of infective endocarditis were first published in February, 1955, and the most recent (fifth revision) in December, 1984. The somewhat controversial nature of these recommendations reflects several issues, including the degree to which infective endocarditis in man is preventable and the relative safety of alternative regimens. Nevertheless, it is apparent that a reasonable fraction of cases are preventable and that antibiotics for this purpose are appropriate. It is also clear that certain patient groups, i.e. those with pros

Hemodynamic effects of pulmonary regurgitation in one and one-half ventricle repair.

The pulmonary circulation after one and one-half ventricle repair is complex because of the direct connection of two sources of blood flow. Associated pulmonary regurgitation further complicates the pulmonary circulation. We report the complex hemodynamics depicted by phase-contrast magnetic resonance imaging in a patient with one and one-half ventricle circuit and severe pulmonary regurgitation. Antegrade superior vena caval flow occurred almost exclusively during diastole and regurgitated into the right ventricle. Consequently, the entire systemic venous return reached the right ventricle du

Presentation of pulmonary artery intimal sarcoma in an infant with a history of neonatal valvular pulmonic stenosis.

Intimal sarcoma of the pulmonary artery is rare in the adult population. It is usually diagnosed postmortem in patients thought to have pulmonary emboli. We present a case of intimal sarcoma of the pulmonary artery in an infant with a history of neonatal pulmonic stenosis.

Aneurysm of the pulmonary trunk in association with minimal pulmonary stenosis.

We report a rare instance of aneurysm of the pulmonary trunk (maximum diameter 6.8 cm) in association with trivial valvar pulmonary stenosis (peak gradient 18 mm Hg). The relevant literature is briefly reviewed.

[Radiological diagnosis of peripheral pulmonary stenosis (author's transl)].

Peripheral pulmonary stenosis is frequently associated with congenital heart disease and seen in childhood. It is rather rarely discovered in adult patients. In adults, however, it is detected as an isolated congenital deformity or as part of generalized congenital cardiovascular disease and may also be caused by collagen disease. The chest x-ray and nuclear scan demonstrate the condition, but pulmonary angiography is essential for positive confirmation.
