Death due to low-energy chest wall trauma, commotio cordis, may occur in young athletes. Death is sudden and usually refractory to even immediate resuscitation efforts. Herein are described the clinical, angiographic, and hemodynamic data of a rare survivor. These observations suggest that commotio cordis not only may be secondary to ventricular fibrillation, but also may be associated with coronary vasospasm or segmental changes in myocardial contractility.

After a retrobulbar block, a patient developed transient apnoea and cranial nerve paralysis. All of the symptoms resolved within hours. According to the literature, apnoea is a rare complication of retrobulbar blockade and usually develops ten minutes after administration of the blockade at the latest. The outcome is excellent if skilled staff, and facilities for resuscitation are available. This complication does not occur after peribulbar anaesthesia.

A review of the world literature revealed only five reported cases of azygos vein disruption from blunt chest trauma. Four of these were isolated injuries. We report a case of isolated complete azygos vein transection as a result of blunt motor vehicular trauma. Emergency thoracotomy and venous ligation resulted in successful resuscitation. Shearing forces as a result of deceleration were felt to be a possible etiology.

A lightning strike involving 47 children is described. Four cases demonstrate the most common and serious resulting pathology: burns, myocardial infarction, and neurological symptoms varying from feelings of fear and nightmares to brain death. The pathophysiology of lightning injury is described. The importance of immediate resuscitation of the victim who appears dead after a lightning strike is emphasized, and procedures to prevent lighting injury are presented.

The patient with a cerebral gunshot wound has a very unstable condition. In Belfast emphasis has been laid on rapid evacuation and on starting resuscitation within a few minutes of injury. Early and adequate transfusion combats shock; controlled ventilation helps limit the rise in intracranial pressure. Intracranial haematomata should be sought by early operation. Operation seldom improves neurological function in missile wounds of the spine.

We report a case of type I tracheal agenesis with vertebral defects, anal atresia, tracheoesophageal fistula and/or esophageal atresia, radial dysplasia, renal defects, and cardiovascular defects association in which computed tomography was helpful in categorizing the diagnosis. This rare and fatal condition should always be considered when confronted with a neonate impossible to resuscitate at birth.

A patient developed neck swelling following removal of an internal jugular vein cannula. Although tracheal extubation was delayed, severe airway obstruction occurred resulting in a cardiac arrest. He was successfully resuscitated. Doppler ultrasonography revealed a huge cervical haematoma surrounding a carotid artery pseudoaneurysm. The carotid artery pseudoaneurysm was obliterated using radiological intervention.

A patient reported symptoms of upper airway obstruction for 4 years. His symptoms worsened with a viral upper respiratory infection and he suffered acute respiratory arrest. He was resuscitated and a large goitre with intrathoracic extension which was compressing the trachea was resected. The mechanisms of upper airway compromise by goitre are discussed. While pneumatic antishock trousers are a useful adjunct for the initial management of hypotension, they do not replace, but can augment, fluid replacement and other resuscitative procedures. They can be applied quickly and are relatively safe when used in the manner recommended. They must not be used in a patient with congestive heart failure. Finally, it is imperative that they are not removed suddenly from the hypovolemic, hypotensive patient.
