Musculoskeletal Diseases Role in the Frailty Syndrome: A Case-Control Study.

Frailty syndrome severely burdens older age, and musculoskeletal diseases are of paramount importance in its development. The aim of this study is to unravel the contribution of musculoskeletal diseases to frailty syndrome. This is a case-control study, and we enrolled 55 robust community-dwelling age- and gender-matched patients, with 58 frail and pre-frail subjects. Frailty was diagnosed according to the Fried criteria (FP), and the Fragility Index (FI) was calculated. In all the subjects, a comprehensive geriatric assessment was carried out. Their nutritional status was evaluated by the Min

Changes in neuromuscular function in elders: Novel techniques for assessment of motor unit loss and motor unit remodeling with aging.

Functional muscle fiber denervation is a major contributor to the decline in physical function observed with aging and is now a recognized cause of sarcopenia, a muscle disorder characterized by progressive and generalized degenerative loss of skeletal muscle mass, quality, and strength. There is an interrelationship between muscle strength, motor unit (MU) number, and aging, which suggests that a portion of muscle weakness in seniors may be attributable to the loss of functional MUs. During normal aging, there is a time-related progression of MU loss, an adaptive sprouting followed by a malad

Bioimpedance spectroscopy: Is a picture worth a thousand words?

Volume status can be difficult to assess in dialysis patients. Peripheral edema, elevated venous pressure, lung crackles, and hypertension are taught as signs of fluid overload, but sensitivity and specificity are poor. Bioimpedance technology has evolved from early single frequency to multifrequency machines which apply spectroscopic analysis (BIS), modeling data to physics-based mixture theory. Bioimpedance plots can aid the evaluation of hydration status and body composition. The challenge remains how to use this information to manage dialysis populations, particularly as interventions to i

Nutritional and Exercise Interventions in Cancer-Related Cachexia: An Extensive Narrative Review.

One of the common traits found in cancer patients is malnutrition and cachexia, which affects between 25% to 60% of the patients, depending on the type of cancer, diagnosis, and treatment. Given the lack of current effective pharmacological solutions for low muscle mass and sarcopenia, holistic interventions are essential to patient care, as well as exercise and nutrition. Thus, the present narrative review aimed to analyze the nutritional, pharmacological, ergonutritional, and physical exercise strategies in cancer-related cachexia. The integration of multidisciplinary interventions could hel

Binge-eating disorder is associated with an unfavorable body mass composition in patients with non-alcoholic fatty liver disease.

The interaction between eating disorders and non-alcoholic fatty liver disease (NAFLD) remains unexplored, especially with regards to binge-eating disorder (BED). Our team conducted a service evaluation project in order to assess risk factors for the presence of BED among patients with NAFLD and the impact of BED on body mass composition. The overall prevalence of patients screening positive to BED Screener-7 (BEDS-7) was 28.4%, while a previous diagnosis of depression and marital status (as single or separated) were independently associated with positive BED. Furthermore, patients with positi

Radiation-Induced Myopathy Developing in a Hodgkin Lymphoma Patient: An Autopsy Case with Systemic Muscle Sampling.

Radiation-induced myopathy (RIM) is a rare complication occurring years after radiotherapy. RIM basically occurs within the irradiation field, but some cases have been reported to be accommodated by myopathy outside the irradiation field, and the actual extent of RIM is obscure. The presented case also showed decreased MMT scores and abnormal needle electromyography results in the muscles outside the irradiated field, and the patient was initially thought to have RIM both within and outside the irradiated field. However, while systemic postmortem muscle sampling revealed prominent myopathy in

Time gap between the onset and diagnosis in Werner syndrome: a nationwide survey and the 2020 registry in Japan.

Patients with Werner syndrome present with diverse signs of aging that begin in adolescence. A Japanese nationwide survey was conducted to establish a registry that could clarify the disease profile of patients with Werner syndrome. The questionnaires were sent to 7888 doctors. The survey identified 116 patients diagnosed with Werner syndrome based on the diagnosis criteria. Forty patients were enrolled in the registry. Data on clinical symptoms, treatment information, and laboratory examination from patients who provided informed consent were collected. The data at enrollment were analyzed. T

Maldigestion Versus Malabsorption in the Elderly.

PURPOSE OF REVIEW: To evaluate recently published information about the frequency of maldigestion and malabsorption in older individuals, likely diagnoses causing these problems, and the diagnostic scheme when these diagnoses are being considered. RECENT FINDINGS: Although the prevalence of malnourishment and frank malnutrition may be increasing among older adults admitted to the hospital, this appears to be due to reduced food intake rather than maldigestion or malabsorption. The mechanisms of food digestion and absorption seem to be resilient, even in old age, but concurrent illness may pro

Glycerol-induced injury as a new model of muscle regeneration.

Skeletal muscle regenerates efficiently following injuries and diseases. However, muscle regeneration is compromised in several conditions by adipocyte infiltration and excessive collagen deposition. Adipocyte infiltration is a characteristic feature of sarcopenia, diabetes, cachexia, muscular dystrophies and advanced cases of Duchenne muscular dystrophy (DMD), while excessive collagen deposition is a hallmark of muscular dystrophies and severe muscle injuries, such as lacerations, contusions and strains. Muscle adipogenesis and fibrosis are major causes of muscle weakness that impairs muscle

Nutritional Considerations for Healthy Aging and Reduction in Age-Related Chronic Disease.

A projected doubling in the global population of people aged ≥60 y by the year 2050 has major health and economic implications, especially in developing regions. Burdens of unhealthy aging associated with chronic noncommunicable and other age-related diseases may be largely preventable with lifestyle modification, including diet. However, as adults age they become at risk of "nutritional frailty," which can compromise their ability to meet nutritional requirements at a time when specific nutrient needs may be high. This review highlights the role of nutrition science in promoting healthy aging

Citrulline in health and disease. Review on human studies.

The amino acid L-citrulline (CIT) is safely used from the neonatal period onwards in those with urea cycle defects and carbamyl phosphate synthetase or ornithine transcarbamylase deficiencies, but several lines of enquiry indicate that it might have a much wider therapeutic role. When protein intake is low and there is a catabolic state, endogenous arginine (ARG) synthesis cannot fully be met and its supplementation can prove challenging, particularly in patients with critical and multisystem illness. Supplementary CIT could constitute a safer but still focused means of delivering ARG to endot

Identification of skeletal muscle mass depletion across age and BMI groups in health and disease--there is need for a unified definition.

Although reduced skeletal muscle mass is a major predictor of impaired physical function and survival, it remains inconsistently diagnosed to a lack of standardized diagnostic approaches that is reflected by the variable combination of body composition indices and cutoffs. In this review, we summarized basic determinants of a normal lean mass (age, gender, fat mass, body region) and demonstrate limitations of different lean mass parameters as indices for skeletal muscle mass. A unique definition of lean mass depletion should be based on an indirect or direct measure of skeletal muscle mass nor

Muscle type and fiber type specificity in muscle wasting.

Muscle wasting occurs in a variety of conditions, including both genetic diseases, such as muscular dystrophies, and acquired disorders, ranging from muscle disuse to cancer cachexia, from heart failure to aging sarcopenia. In most of these conditions, the loss of muscle tissue is not homogeneous, but involves specific muscle groups, for example Duchenne muscular dystrophy affects most body muscles but spares extraocular muscles, and other dystrophies affect selectively proximal or distal limb muscles. In addition, muscle atrophy can affect specific fiber types, involving predominantly slow ty

Regulation of muscle protein synthesis and the effects of catabolic states.

Protein synthesis and degradation are dynamically regulated processes that act in concert to control the accretion or loss of muscle mass. The present article focuses on the mechanisms involved in the impairment of protein synthesis that are associated with skeletal muscle atrophy. The vast majority of mechanisms known to regulate protein synthesis involve modulation of the initiation phase of mRNA translation, which comprises a series of reactions that result in the binding of initiator methionyl-tRNAi and mRNA to the 40S ribosomal subunit. The function of the proteins involved in both events

The intensity and effects of strength training in the elderly.

BACKGROUND: The elderly need strength training more and more as they grow older to stay mobile for their everyday activities. The goal of training is to reduce the loss of muscle mass and the resulting loss of motor function. The dose-response relationship of training intensity to training effect has not yet been fully elucidated. METHODS: PubMed was selectively searched for articles that appeared in the past 5 years about the effects and dose-response relationship of strength training in the elderly. RESULTS: Strength training in the elderly (>60 years) increases muscle strength by increasi

Beyond reduced intake: tumor metabolism and immune dysregulation drive cancer-related malnutrition in head and neck squamous cell carcinoma (HNSCC).

Cancer-related malnutrition is highly prevalent in patients with head and neck squamous cell carcinoma (HNSCC) and represents a major determinant of treatment tolerance, quality of life, and survival. Although traditionally attributed to reduced oral intake caused by dysphagia, mucositis, and treatment-related toxicity, increasing evidence indicates that malnutrition in HNSCC is largely driven by tumor-induced metabolic and inflammatory alterations. Enhanced aerobic glycolysis, systemic inflammation, and tumor-derived factors promote hypermetabolism, substrate redistribution, and progressive s

Weighted vest use or resistance exercise to offset muscle loss in older adults: secondary findings from the INVEST in bone health RCT.

BACKGROUND: The INVEST in Bone Health Trial examined the effects of weight loss (WL), WL plus resistance training (WL + RT), or WL plus weighted vest use (WL + VEST) on musculoskeletal health. This secondary analysis evaluated changes in muscle area and density using computed tomography (CT) and lean and fat mass using dual-energy x-ray absorptiometry (DXA). METHODS: One hundred fifty participants (50/group) were randomized to 12-months of WL, WL + RT, or WL + VEST, undergoing CT and DXA scans at baseline, six- and 12-months. DXA measured lean and fat mass, while CT assessed muscle and interm

Novel myostatin inhibition by Acacia farnesiana: A network pharmacology workflow decoding its multi-target mechanism in sarcopenic obesity.

Sarcopenic obesity (SO) represents a multifaceted metabolic disorder characterized by the pathological coexistence of skeletal muscle atrophy and dysfunctional adiposity, leading to severe metabolic and functional impairments. Myostatin, a key negative regulator of myogenesis, has emerged as a critical therapeutic target for counteracting muscle atrophy associated with SO. In this study, we deployed an integrative network pharmacology strategy to elucidate the pharmacokinetic profiles and therapeutic efficacy of Acacia farnesiana-derived bioactive agents, specifically palmitic acid and α-terpi

The lung-muscle metabolic axis: a proposed mechanism for post-resection systemic frailty.

Long-term non-cancer mortality following lung cancer surgery remains an under-recognized challenge in clinical practice. This review proposes a novel "pulmonary hypertensive phenotype" hypothesis, suggesting that the permanent reduction of the pulmonary vascular bed after anatomical resection acts as the primary upstream driver of systemic vulnerability. While the ventilatory capacity may partially recover through enlargement of the airspace and compensatory remodeling, restoration of the pulmonary vascular bed is often incomplete. This deficit leads to a sustained right ventricular afterload

Efficacy of combined nutrition and exercise interventions on muscle health and performance in patients after a stroke: protocol for a systematic review and meta-analysis.

INTRODUCTION: Stroke survivors experience profound skeletal muscle deterioration, typically losing 15%-20% of skeletal muscle mass in the affected limb within 4-6 months post-stroke. This deterioration accelerates functional decline, falls and secondary sarcopenia. While combined nutrition and exercise interventions have shown synergistic effects in other muscle-wasting conditions, the evidence for stroke remains fragmented. This systematic review and meta-analysis aims to evaluate the efficacy of combined nutrition and exercise interventions on muscle health and performance in patients after
