Evolving insights on the role of microglia in neuroinflammation, plasticity, and regeneration of the injured spinal cord.

Microglia have emerged as central players in the pathophysiology of traumatic spinal cord injury (SCI). The purpose of this brief review is to highlight the evolution of knowledge on the role of microglia in SCI. We explore the initial discovery of macrophages and their role in SCI lesions, followed by how microglia were examined and distinguished from monocyte-derived macrophages. We then discuss findings from studies that mapped and manipulated microglia in experimental SCI, made possible through technological advances in genetic, pharmacological, and bioinformatic approaches. We also highli

Riluzole as a pharmacological therapy for spinal cord injury: where does this therapy stand?

PURPOSE OF REVIEW: Spinal cord injury (SCI) remains a disabling condition associated with long term neurological impairment, functional disability, and reduced quality of life. Despite decades of research, pharmacological interventions with proven clinical efficacy remain limited. This review critically evaluates the current evidence supporting riluzole as a neuroprotective agent for acute traumatic and nontraumatic SCI. We synthesize findings from preclinical and clinical studies, assess the progress towards clinical translation, and outline key challenges and research opportunities for futur

Spinal Cord Injury in Enterovirus D68 Infection: Mechanisms and Pathophysiology in a Mouse Model.

Enterovirus D68 (EV-D68) is a significant global pathogen associated with severe respiratory infections and acute flaccid myelitis in children. Currently, there are no vaccines or antiviral drugs available for EV-D68, and a robust model to elucidate the pathogenesis of EV-D68 and evaluate treatment methods is lacking. We developed a mouse-adapted EV-D68 strain that caused progressive limb paralysis after intramuscular inoculation in 7-day-old mice. Viral load analysis showed that the skeletal muscle and spinal cord had the highest titers and most severe injuries. RNA sequencing of the infected

Spinal cord-protective effect of resorcimoline for ischemia-reperfusion injury in a rabbit model.

OBJECTIVES: Spinal cord injury can occur after aortic surgery as a result of reactive oxygen species (ROS) generated during ischemia-reperfusion. Resorcimoline is a novel and potent antioxidant that has demonstrated ROS-scavenging activity in vitro. We evaluated the spinal cord-protective effects of resorcimoline and compared its efficacy with that of edaravone, a free-radical scavenger currently used clinically in Japan. METHODS: A spinal cord injury model was established via infrarenal aortic occlusion for 20 minutes in 30 Japanese white rabbits. The animals received saline (control), resor

Spinal cord injury in abusive and accidental head injury in children, a neuropathological investigation.

The diagnosis of abusive head trauma (AbHT) in children is a challenging one that needs to be differentiated from natural disease and accidental head injury (AcHT). There is increasing evidence from the Neuroradiology field showing spinal cord injury in children subject to AbHT, which has, so far, been poorly investigated pathologically. In this study we retrospectively reviewed the forensic records of 110 paediatric head injury cases over an eight-year-period. The records included detailed circumstances of death and clinical history alongside neuropathology, ophthalmic pathology and osteo-art

Biomimetic Nanoplatform Based on the Neutrophil Membrane for Targeted Therapy of Spinal Cord Injury.

INTRODUCTION: Spinal cord injury (SCI) triggers a cascade of secondary damage, including oxidative stress and neuroinflammation, for which effective treatments remain limited. To address this, we developed a biomimetic nanoplatform, NEU@MPBNPs-HED, consisting of mesoporous Prussian blue nanoparticles (MPBNPs) loaded with hederagenin (HED) and cloaked with neutrophil membranes (NEUm) to enhance lesion targeting and immune evasion. METHODS: The physicochemical properties, drug release characteristics, and cellular uptake of NEU@MPBNPs-HED were characterized. Therapeutic efficacy was evaluated i

Acute Spinal Cord Syndrome As the Initial Manifestation of Multiple Sclerosis: A Case Report.

Multiple sclerosis (MS) typically presents with optic neuritis, brainstem syndromes, or sensory disturbances. Isolated spinal cord involvement as the initial manifestation occurs in fewer than 30% of cases, making early recognition challenging but essential for timely diagnosis and treatment initiation. A previously healthy 22-year-old Caucasian male presented with an acute onset of mid-thoracic pain followed by progressive sensory deficits below the T10 dermatome and lower limb weakness developing over five days. Neurological examination revealed spastic paraparesis (Medical Research Council

The Clinical Management of Electrical Stimulation Therapies in the Rehabilitation of Individuals with Spinal Cord Injuries.

Background: People with spinal cord injuries (SCIs) often have trouble remaining active because of paralysis. In the past, exercise recommendations focused on the non-paralyzed muscles in the arms, which provides limited benefits. However, recent studies show that electrical stimulation can help engage the paralyzed extremities, expanding the available muscle mass for exercise. Methods: The authors provide an evidence-based approach using expertise from diverse fields, supplemented by evidence from key studies toward the management of electrical stimulation therapies in individuals with SCIs.

Influence of sports on cortical excitability in patients with spinal cord injury: a TMS study.

BACKGROUND: Patients with spinal cord injury (SCI) show abnormal cortical excitability that might be caused by deafferentation. We hypothesize a reduced short-interval intracortical inhibition preceding movement in patients with SCI compared with healthy participants. In addition, we expect that neuroplasticity induced by different types of sports can modulate intracortical inhibition during movement preparation in patients with SCI. METHODS: We used a reaction test and paired-pulse transcranial magnetic stimulation to record cortical excitability, assessed by measuring amplitudes of motor-ev

Elevation of inositol pyrophosphate IP7 in the mammalian spinal cord of amyotrophic lateral sclerosis.

BACKGROUND: Amyotrophic lateral sclerosis (ALS) is a fatal neurodegenerative disorder associated with progressive impairment of spinal motor neurons. Continuous research endeavor is underway to fully understand the molecular mechanisms associating with this disorder. Although several studies have implied the involvement of inositol pyrophosphate IP7 in ALS, there is no direct experimental evidence proving this notion. In this study, we analyzed inositol pyrophosphate IP7 and its precursor IP6 in the mouse and human ALS biological samples to directly assess whether IP7 level and/or its metaboli
