Nontraumatic intracranial internal carotid artery dissection: a case report.

The case presented offers a demonstration of a rare yet devastating condition that may go unrecognized and incompletely worked up by the emergency physician. Internal carotid artery dissection is seen most often in previously healthy, young patients and thus all efforts toward diagnosing this condition and providing proper stabilization must be made. Unfortunately, little advancement in the therapeutic progress of this frequently fatal condition has been made over the past decades. To date, both medical management and surgical techniques have been utilized with variable success. This case shou

Depression screening in adolescents with somatic complaints presenting to the emergency department.

STUDY OBJECTIVE: To determine the frequency of documented depression screening for adolescents presenting with somatic chief complaints to a pediatric emergency department. METHODS: We conducted a retrospective chart review of 408 consecutive patients aged 11 to 17 years who presented to the ED with a chief complaint of chest pain, abdominal pain, headache, weakness/fatigue, dizziness/fainting, or hyperventilation. RESULTS: Documentation of depression screening was noted in 4.2% of cases (17 of 408). For patients charts listing both a somatic chief complaint and a similar nonspecific dischar

Calcitonin gene-related peptide (CGRP) and leg vascular resistance during head-up tilt induced hypovolaemic shock in man.

Calcitonin gene-related peptide is a potent vasodilator and its distribution in perivascular nerves suggests a role in the regulation of vascular tone. We evaluated leg vascular resistance together with total peripheral resistance and the arterial plasma concentrations of calcitonin gene-related peptide and catecholamines during 50 degrees head-up tilt induced hypotension in 7 males. During tilt mean arterial pressure, heart rate, total peripheral resistance, leg vascular resistance and plasma noradrenaline increased, while cardiac output and leg blood flow decreased. After 45 +/- 9 min (mean

[Properties of drug complications in elderly patients].

Adverse reactions to drugs are characteristically more frequent and more severe in elderly people as a result of factors that are found exclusively in this category of patients, such as multiple diseases, multiple medications, multiple prescribers, self-medication, age-related changes in pharmacokinetics and patient's compliance. Clinically, these reactions are atypical, often mixed, and they may take their symptoms from some other disease decompensated by the adverse reaction. The frequency of this iatropathology in elderly patients is such that it must systematically be suspected in the pres

The neurologic complications of cardiovascular disease.

Many cardiac disorders masquerade as neurologic problems. Differential diagnosis is discussed, as well as common neurologic presentations of cardiovascular disease, particularly stroke syndromes arising from emboli, the multitude of disorders that results in loss of consciousness, and neurologic symptoms and signs caused by cardiac drugs.

Early follow-up of lead performance in atrioventricular sequential systems.

Recent developments in pacemaker technology led us to report our initial and follow-up assessment of atrioventricular (A-V) sequential systems in 50 consecutive patients. Primary indications for pacing were sinus node dysfunction or A-V block. Leads were introduced through the subclavian vein. Atrial J-tined leads (27 silicone, 19 urethane) were positioned in the atrial appendage or stump in 46 patients, and coronary sinus or Bisping leads were placed in 4 patients; all 50 patients had tined ventricular leads. A Medtronic 5992 generator was placed in 35 patients and an Intermedics 259-01 gener

Sudden-onset severe presyncope in a 67-year-old man.

A 67-year-old man presented to the emergency department with sudden onset of severe presyncope. He reported that he had a permanent pacemaker implanted in 2006 following atrioventricular node ablation for persistent atrial fibrillation (AF). After suffering increasing shortness of breath, he underwent upgrade to cardiac resynchronisation therapy (CRT) in 2016. He denied any recent falls, interventions or changes in medication. ECG monitoring showed AF with a broad ventricular escape rhythm at around 25 bpm with pauses of up to 3 s. Placement of a magnet over the device resulted in pacing (figu…

Increase in Medical Emergency Calls and Calls for Central Nervous System Symptoms During a Severe Air Pollution Event, January 2013, Jinan City, China.

BACKGROUND: In January 2013, a continuous and severe haze event affected one third of the major cities in China, including Jinan. OBJECTIVES: We investigated whether medical emergency calls (for nonaccidental emergencies) and calls for Central Nervous System (CNS) syndromes increased during this episode compared with the previous winter months (January, February, and December) in 2011 and 2012. METHODS: Daily emergency calls were obtained from Jinan Medical Emergency Center. Sentinel CNS syndromes from nonaccidental emergency calls included seven symptoms: headache, dizziness, syncope, coma,…

Giant left atrial myxoma causing drop attacks by prolapsing into the mitral valve.

Atrial myxoma is the most common primary cardiac tumor. Its clinical presentation varies greatly from asymptomatic incidental mass to serious life-threatening cardiovascular complications. We herein describe the clinicopathological and imaging features of a huge left atrial myxoma protruding into the left ventricle during diastole and obstructing diastolic filling of the left ventricle thus causing drop attacks by prolapsing into the mitral valve. The patient (a 56-year-old female) underwent emergency surgery with complete removal of a 74 g weighing myxoma from the left atrium. She recovered w

Practical recommendations and perspectives on cardiac screening for healthy pediatric athletes.

Primary care providers often are asked to perform the preparticipation physical exam and cardiac screening of pediatric athletes. It can be challenging to evaluate which athletes may require further cardiac evaluation and specialist referral based on a focused history and physical. There is considerable controversy surrounding mass electrocardiogram (ECG) screening of athletes in the United States. The ECG is one of several diagnostic tests used by pediatric cardiologists in the evaluation of underlying heart disease in young athletes. This article reviews cardiac conditions associated with su

Correlations between urinary excretion of catecholamines and electrocardiographic parameters of vagal hyperreactivity in infants with fainting spells. Implication of sympathetic hypotonia?

BACKGROUND: Vagal hyperreactivity (VHR) is a frequent etiology of infant fainting spells; but it is sometimes difficult to diagnose. A biochemical test would therefore be useful, especially as the oculocardiac reflex (OCR) test innocuity is not absolute. AIMS: To evaluate urinary excretions of norepinephrine, epinephrine and dopamine as markers for vagal hyperreactivity. METHODS: During check-up of 55 infants from 0.5 to 11 months of age, for discomfort episodes, including OCR and Holter recording, 24h urinary assays of total norepinephrine, epinephrine and dopamine were carried out to evalu
