Great toe metatarsophalangeal joint arthrodesis: a user-friendly technique.

Thirty-four feet (23 patients) were treated with a metatarsophalangeal (MP) joint fusion of the hallux using five threaded 0.062-in K wires for fixation. Operations were done for the following diagnoses: rheumatoid arthritis (26 procedures), hallux rigidus (1), salvage of previous bunionectomies (3), hallux valgus with absent toe, bilateral fusion (2), severe hallux valgus with chronic MP joint synovitis (1), and congenital hallux varus (1). The ages ranged from 17 to 73 years, with an average of 55 years. Follow-up was available on 31 of the fusions by questionnaire and telephone contact, wit…

[Immunopathological study of synovium of rheumatoid arthritis].

46 patients with different rheumatic disorders were subjected to arthroscopic examination and the biopsied synovia were studied immunopathologically. Immunoglobulins, complements and fibrinogen deposition were detected by direct immunofluorescence, and rheumatoid factor (RF) deposition was detected by peroxidase-labelled denatured human IgG. Postoperative diagnosis of these cases were as follows: rheumatoid arthritis (RA) 21; seronegative spondylarthropathy 4; reactive arthritis 2; unidentified synovitis 4; osteoarthritis 8; and nonsynovitis conditions 5. It was shown that the positive rates o…

Collagen arthritis in rats, arthritogenic lymphokines and other aspects.

This review will mainly highlight data from selected, independent studies which collectively implicate a primary role for T cells in the pat … as 10(3) type II collagen specific T line cells were capable of provoking a sustained proliferative synovitis when instilled into the knee joint cavity of syngeneic naive rats. Further analysis of this phenomenon revealed that the arthritogenic capacity of various collagen-reactive line cells correlated with their ability to release a 65-Kd, collagen-bindin …

Evaluation of EVT401, a P2X7 Receptor Antagonist, in Healthy Chinese Participants: a first-in-human, double-blind, randomized, placebo-controlled, dose-escalation trial.

PURPOSE: As a critical hub between extracellular danger signals and intracellular inflammation, the P2X7 receptor centrally drives synovitis and joint destruction in rheumatoid arthritis, making it an attractive therapeutic target. This study reported the first-in-human data for EVT401, an orally administered P2X7 receptor antagonist. METHODS: A phase I trial was conducted in healthy Chinese participants aged 18 to 50 years, encompassing a single ascending dose study (40-1920 mg), a food-effect study (960 mg), and a multiple ascending dose study (480 mg-960 mg). This randomized, double-blind,…

Diagnostic Complexity in Systemic Inflammation: Adult-Onset Still's Disease.

Adult-onset Still's disease (AOSD) is a rare systemic autoinflammatory disorder classically associated with fever, rash, arthritis, and marked inflammatory laboratory abnormalities. Diagnosis is clinical and typically requires exclusion of infection, malignancy, and other rheumatologic diseases. We describe a 33-year-old previously healthy Nigerian male with several weeks of daily fevers by history, diffuse polyarthralgia with synovitis, sore throat, and a transient non-pruritic rash initially treated as an allergic reaction. His symptoms progressed despite antibiotics. Outpatient testing show…

Moringa as a Functional Food for Rheumatoid Arthritis: A Scoping Review of Evidence.

Background/Objectives: Rheumatoid arthritis is a chronic autoimmune disease characterised by persistent synovitis and joint destruction. While conventional pharmacotherapies, such as disease-modifying anti-rheumatic drugs, are effective, they are often limited by significant adverse effects and high costs. Moringa, a medicinal plant rich in bioactive compounds, has emerged as a potential functional food adjunct for managing this condition. This scoping review systematically maps the evidence regarding the efficacy of moringa supplementation in alleviating the pathology of rheumatoid arthritis.…

Single-cell and spatial profiling of peripheral blood and synovium unveils pro-arthritis CNBP+ myeloid cells.

BACKGROUND: Arthritis ranks as a primary contributor to global disability, with synovitis representing a common pathological feature across all types of arthritis. Given that macrophages constitute the predominant inflammatory cells in arthritic synovium, deciphering their heterogeneity facilitates the elucidation of pathogenic mechanisms. METHODS: We integrated single-cell transcriptomic profiles from human peripheral blood and synovial tissue across 173 samples encompassing six types of arthritis to characterize transcriptional remodeling, including molecular signatures, transcriptional regu…

Cross-presentation of citrullinated antigens drives cytotoxic CD8+ T cell responses in rheumatoid arthritis.

Rheumatoid arthritis (RA) is an autoimmune synovitis marked by anti-citrullinated protein antibodies (ACPAs) and infiltration of the synovium by activated immune cells. In ACPA-positive RA, CD8+ T cells are elevated in both the blood and synovium, and can be activated by MHC class I-restricted citrullinated autoantigens to mediate cytotoxic effector function. However, the mechanisms underlying the activation of cytotoxic CD8+ T cells in RA remain poorly understood. Here, single-cell transcriptomic and T cell receptor repertoire analysis of RA blood and synovial T cells revealed shared clonally…

Association of serum pentraxin 3 with rheumatoid arthritis disease activity and synovial blood flow.

Rheumatoid arthritis (RA) is characterized by persistent synovitis. Conventional inflammatory markers such as C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR) inadequately reflect synovial inflammation, particularly under interleukin-6 pathway inhibition. Pentraxin 3 (PTX3), an extrahepatic acute-phase protein produced in inflamed tissues, may better reflect local inflammation and angiogenesis in RA joints. We enrolled 80 RA patients and 80 age-sex-matched healthy controls. Clinical data, inflammatory markers, and disease activity scores (DAS28) were collected. Serum PTX3 leve…
