Magnetic resonance imaging of injuries in the child athlete.

Sports related injuries in children and adolescents are an increasingly frequent clinical problem. Soft tissue injury is relatively less common in children than in adults and radiographs suffice in most situations. However there are specific clinical circumstances where MR imaging is of value. We discuss and demonstrate a variety of acute and chronic sports related conditions where MR imaging has contributed to the diagnosis and management. Some conditions such as apophysitis, osteochondritis and chronic physeal injuries are unique to childhood while others including spondylysis and stress fra

Effusion criteria and clinical importance of glenohumeral joint fluid: MR imaging evaluation.

PURPOSE: To investigate the clinical importance of glenohumeral joint (GHJ) fluid. MATERIALS AND METHODS: The amount of GHJ fluid in 17 volunteers and 208 consecutive patients was graded at magnetic resonance imaging with T2-weighted fat-suppressed coronal oblique images by two blinded observers. Thorough historical data and physical examination results were available for 108 patients. Presence and grade of GHJ fluid were correlated with age, sex, presence of osteophytes activity scale, supraspinatus tenderness, clinical impingement, prior subacromial injections, rotator cuff tears (RCTs), jo

Spectral frequency analysis of dynamic balance in healthy and injured athletes.

Different neurophysiological circuits underlie the various frequencies of the body sway for the regulation of human upright posture. Servo-controlled platforms allowed experimental studies about latency responses after a sudden external disturbance but power frequency repartition during spontaneous dynamic equilibrium has rarely been examined. The purpose of the present study was to examine the zone of sway frequency assigned to proprioceptive afferences from muscular, articular and tendinous receptors in healthy and injured subjects standing on an unstable and minimal area. Dynamic balance co

Distal Forearm Replantation in a Child: A Case Report with a 30-year Follow-up.

BACKGROUND: Amputation in the upper extremities influenced the quality of life a lot adversely. So, replantation was tried in many cases of amputation. Especially, due to good plasticity and healing capacity, replantation in children should be actively attempted. On the contrary, owing to growth potential in children, there are several late complications to happen like shortening and synostosis. There are only a few longterm follow-up reports of paediatric patients after replantation of upper extremities. We report a case of successful distal forearm replantation in a 2-year-old child who sust

Prediction of quadriceps strength recovery after anterior cruciate ligament reconstruction with a hamstring autograft: Decision tree analysis.

BACKGROUND: The wide variations in patient demographics and concomitant injuries make the prediction of which patients will regain strength quickly following anterior cruciate ligament (ACL) reconstruction challenging. This study aimed to identify the dominant factor affecting quadriceps strength recovery after ACL reconstruction with a hamstring tendon autograft and to construct a predictive model for quadriceps strength recovery using decision tree analysis. METHODS: Three hundred and eighty-six patients who underwent ACL reconstruction with a hamstring tendon autograft were included in thi

Innovative applications and future challenges of shape memory scaffolds for functional reconstruction in diseases of the musculoskeletal system.

Musculoskeletal disorders (MSDs), such as osteoporosis, cartilage degeneration, and tendon injuries, are common worldwide and have a significant impact on patients' quality of life. These conditions are frequently associated with aging, trauma, or chronic diseases. They are typically characterized by slow and limited self-repair, particularly in tissues like cartilage and tendons, which exhibit low regenerative potential. Traditional treatment methods, such as implantable scaffolds, often face challenges related to incomplete tissue integration and inadequate adaptation to dynamic biomechanica…

Stifle arthrodesis in dogs using semi-circular saw: Cadaveric study.

The aim of this study was to develop a novel surgical technique for stifle arthrodesis in dogs using a semicircular saw for tibial and femoral ostectomies through a medial approach. Ten pelvic limbs from five canine cadavers underwent stifle arthrodesis. Prior to the surgical procedure, the limbs were radiographed to rule out musculoskeletal abnormalities. Additionally, the radiographs were used for surgical planning. For the tibial ostectomy, the center of the intercondylar eminences, the cranial limit of the tibial plateau, and the caudal cortex of the tibia were used as landmarks. In the fe

Cadaveric Analysis of Key Anatomic Structures of Athletic Pubalgia.

PURPOSE: This study proposes to establish in-depth inspection of the anatomic structures involved with the pathology of athletic pubalgia in a cadaver model. METHODS: Eight male fresh frozen cadavers were dissected in a layered fashion. The rectus abdominis (RA) and adductor longus (AL) tendon insertions were isolated to quantify the size of the anatomic footprint and distance from the surrounding anatomy. RESULTS: The RA insertional footprint was 1.65 cm (SD, 0.18) in width by 1.02 cm (SD, 0.26) in length, and the AL insertional footprint on the underside of the pubis was 1.95 cm (SD, 0.28)

A Soft Hand Exoskeleton With a Novel Tendon Layout to Improve Stable Wearing in Grasping Assistance.

We present a novel soft exoskeleton providing active support for hand closing and opening. The main novelty is a different tendon routing, folded laterally on both sides of the hand, and adding clenching forces when the exoskeleton is activated. It improves the stability of the glove, diminishing slippage and detachment of tendons from the hand palm toward the grasping workspace. The clenching effect is released when the hand is relaxed, thus enhancing the user's comfort. The alternative routing allowed embedding a single actuator on the hand dorsum, resulting more compact with no remote cable

Bipolar Hip Arthroplasty Using a Conjoined Tendon-preserving Posterior Approach in Geriatric Patients.

In bipolar hemiarthroplasty (BHA), it is important to preserve soft tissue to reduce the risk of postoperative dislocation. A variety of surgical approaches for BHA are available, but extra care is needed with muscle- and tendon-preserving approaches in geriatric patients. We investigated the usefulness of BHA using a conjoined tendon-preserving posterior (CPP) approach, in which only the external obturator muscle is dissected, in geri-atric patients. We retrospectively analyzed the cases of 40 femoral neck fracture patients (10 men, 30 women) aged ≥ 80 years who underwent BHA using the CPP ap

Biofabrication and Signaling Strategies for Tendon/Ligament Interfacial Tissue Engineering.

Tendons and ligaments (TL) have poor healing capability, and for serious injuries like tears or ruptures, surgical intervention employing autografts or allografts is usually required. Current tissue replacements are nonideal and can lead to future problems such as high retear rates, poor tissue integration, or heterotopic ossification. Alternatively, tissue engineering strategies are being pursued using biodegradable scaffolds. As tendons connect muscle and bone and ligaments attach bones, the interface of TL with other tissues represent complex structures, and this intricacy must be considere

Arthroscopic Remnant-Preserving Anterior Talofibular Ligament Reconstruction does not Improve Mid-term Function in Chronic Ankle Instability.

BACKGROUND: Anterior talofibular ligament (ATFL) reconstruction is routinely undertaken to manage chronic lateral ankle instability (CLAI). This study evaluated the effects on clinical outcome of preserving or not preserving the ATFL remnant when reconstructing the ATFL. METHODS: From January 2015 to July 2017, 53 CLAI patients with ATFL injury were randomized to undergo either a remnant-preserving (preservation of ATFL) or a non-preserving (no preservation of ATFL) anatomic reconstruction of the ATFL using an ipsilateral free semitendinosus tendon autograft. Simultaneous bilateral distal biceps tendon repair: case report.

Simultaneous bilateral rupture of the distal biceps tendon is a rare clinical entity, seldom reported in the literature and with unclear therapeutic setting. The authors report the case of a 39-year-old white man who suffered a simultaneous bilateral rupture while working out. When weightlifting with elbows at 90° of flexion, he suddenly felt pain on the anterior aspect of the arms, coming for evaluation after two days. He presented bulging contour of the biceps muscle belly and ecchymosis in the antecubital fossa, extending distally to the medial aspect of the forearm, as well as a marked dec

Potential effects of racket grip size on lateral epicondilalgy risks.

The effects of tennis racket grip size on the forces exerted by muscles affecting lateral epicondylalgia (LE) were assessed in this study. Grip forces and joint moments applied on the wrist were quantified under three different handle size conditions, with and without induced muscle fatigue for intermediate and advanced players. The obtained experimental results were then used as input data of a biomechanical model of the hand. This simulation aimed to quantify the impact of grip strength modulation obtained in the experiment on the wrist extensor muscle forces. Our results show that there is

Bilateral temporoparietal fascial free flaps for reconstruction of bilateral hand defects: a report of two cases.

Bilateral temporoparietal fascial free flaps were used for reconstruction of bilateral hand defects in two male patients. A 42-year-old man sustained crushed injury to both hands with avulsion defects and exposed bones and tendons. The two separate procedures were performed under general anesthesia. The temporoparietal fascial free flap was skin grafted on the ward on the following day after the operation. The other patient was a 61-year-old leprosy patient who had bilateral high ulnar nerve palsy for 28 years. One simultaneous procedure was performed under local anesthesia for harvesting the

An investigation into the depth of penetration of low level laser therapy through the equine tendon in vivo.

Low level laser therapy (LLLT) is frequently used in the treatment of wounds, soft tissue injury and in pain management. The exact penetration depth of LLLT in human tissue remains unspecified. Similar uncertainty regarding penetration depth arises in treating animals. This study was designed to test the hypothesis that transmission of LLLT in horses is increased by clipping the hair and/or by cleaning the area to be treated with alcohol, but is unaffected by coat colour. A LLLT probe (810 nm, 500 mW) was applied to the medial aspect of the superficial flexor tendon of seventeen equine forelim

[Magnetic resonance imaging of glenohumeral instability: new concepts].

The shoulder is the most unstable joint in the human body and is therefore susceptible not only to acute dislocations but also to minimal states of subluxation related to overuse. The latter condition is known as microinstability and is translated clinically into internal impingement syndromes. Both in dislocations due to acute traumatism and in microinstability, a series of injuries to the glenoid lip, glenohumeral capsule, the articular cartilage, the head of the humerus, the tendons of the rotator cuff, the long tendon of the biceps, the glenohumeral ligaments, and coracohumeral ligament ca

The use of latissimus dorsi muscle flap in the aesthetical reconstruction of heat-press injury of the hand.

We present a successful case of aesthetic reconstruction utilizing free latissimus dorsi muscle flap transfer. A large quantity of skin of the dorsum of hand and finger was lost. The dorsum of the index, long and ring fingers was severely damaged, such that extensor tendons were necrotic and all digital bones and the second metatarsal bone were exposed with partial necrosis. In addition, the proximal interphalangeal joints (PIP) were also exposed. To cover exposed bones and the tendons of dorsum of the hand, a free latissimus dorsi muscle flap was transferred, and then meshed skin covered the

Effects of hyperbaric oxygen therapy on bone, muscle, cartilage, tendon and ligaments in animal studies: a systematic review.

BACKGROUND: Hyperbaric oxygen therapy (HBOT) has been investigated for its potential to enhance tissue healing because of its capacity to increase oxygen tension in ischemic and hypoxic tissues. Numerous preclinical studies have explored its role in the repair and regeneration of different musculoskeletal tissues. PURPOSE: The aim of this systematic review was to assess the effects of HBOT on bone, muscle, cartilage, tendon, and ligament tissue in preclinical studies. STUDY DESIGN: Systematic review. METHODS: A systematic review was performed via PubMed and LIVIVO databases. The inclusion c

Reappraisal of nerve repair.

In every case of acute injury involving the nerve, the surgeon must decide whether a primary repair of an elective early secondary repair is the treatment of choice. In a clean-cut nerve without defect, immediate primary repair, using trunk-to-trunk coaptation with epineurial sutures, offers an optimal solution. In the periphery of the median and the ulnar nerves, in which motor and sensory fascicles are already separated, fascicular dissection is performed, and coaptation of fascicle groups should be done. In medical centers with excellent facilities, such nerve repair will give good results
