Surgical outcomes of middle fossa approach in intracanalicular vestibular schwannoma.

CONCLUSION: Middle fossa approach (MFA) shows a hearing preservation rate of 86% and facial nerve function was preserved with HB grade I or … 3 years. At initial presentation, 57% of the patients had vertigo, 43% hearing disturbance, and 64% tinnitus. The mean tumor size was 9.7 mm. The tumors were completely resected in 86% of the patients. Hearing was post-operatively preserved in 12 patients and two patients lost their hearing following surgery. Facial nerve function post-operatively remained …

Noise-Induced hearing loss among professional musicians.

OBJECTIVES: After presbycusis, noise exposure is considered the second cause of sensorineural hearing loss. Due to exposure to high-intensit … teral hearing loss were present in respectively 42.4% and 19.2% of the participants. The history of tinnitus after performance and ear pain during performance was reported by 64 (51%) and 35 (28%) individuals, respectively. Less than 2% of the participants used hearing protection devices. CONCLUSIONS: Long-term exposure to loud sounds puts musicians at ri …

Ocular findings may be accompanied by lymphocytic meningitis, hearing impairment and/or tinnitus in a variable proportion of patients. Prompt diagnosis followed by early, aggressive and long-term treatment with high-dose corticosteroids is most often ensued by good visual outcomes. Atypical teratoid/rhabdoid tumor (AT/RT) arising from the acoustic nerve in a young adult: a case report and a review of literature.

Atypical teratoid/rhabdoid tumors (AT/RTs) are rare, highly malignant central nervous system tumors that predominantly occur in young children. A 22-year-old woman presented with a 4-year history of relapsing tinnitus and gradual hearing loss. Neuroimaging revealed an enhanced intrinsic left internal auditory canal mass. The patient underwent radiotherapy treatment. Three years later, the tumor size continued to increase, as observed by imaging, and ultimately evolved into the left cerebellopontine angle. As a consequence, a total tumor resection was performed, and a pathological diagnosis of …

DNA copy gains of tumor-related genes in vestibular schwannoma.

DNA copy gains are a common event in tumor growth. This study determines the gene dosage/amplification of seven tumor-related genes in patie … nd volume (p = 0.005). Copy gains of EGFR, ERBB2, ERBB4, and MDM4 were associated with preoperative tinnitus. Contrary to other tumors of the central nervous system, development of VS does not appear to involve gene amplification. However, copy gains of certain tumor-related genes may play a role in the biological behavior of these neoplasms. Our findings …

Giant cell tumor of the temporal bone with direct invasion into the middle ear and skull base: a case report.

Giant cell tumor (GCT) is classified as a benign bone tumor, and it is frequently identified at the epiphysis of long bones and relatively rare in the temporal bone. For orthopedists expert at recognizing bone and soft tissue tumors, the diagnosis of GCT is relatively easy; however, since head and neck surgeons experience few cases of GCT, it may be difficult to diagnose when it occurs in the temporal bone. A 32-year-old man complained of left hearing loss, aural fullness, and tinnitus. Examination of the ear revealed a bulging tumor. Audiologic examination demonstrated conductive hearing loss…

Infantile onset neurofibromatosis type 2 presenting with peripheral facial palsy, skin patches, retinal hamartoma and foot drop.

CONCLUSIONS: NF2 can already become evident in infancy. While in adulthood tinnitus, hearing loss and vestibular symptoms are the classical signs, these are often absent in the paediatric group. Children rather have ocular symptoms, neurological problems such as cranial nerve palsies other than eighth nerve, limb weakness and skin …

Analysis of causes for late presentation of Indian patients with vestibular schwannoma.

OBJECTIVE: To determine the causes of delay in diagnosis and treatment of Indian patients with vestibular schwannomas. METHODS: In a prospective study from 2003 to 2005, 50 patients with a confirmed diagnosis of vestibular schwannoma were interviewed to determine the causes for (1) the delay between the patient noting the initial symptom and the definitive diagnosis, and (2) the reasons for delayed diagnosis. RESULTS: In 90 per cent of patients, the initial symptom was either hearing loss (62 per cent), vertigo (24 per cent) or tinnitus (4 per cent). However, most patients had been diagnosed…

Comparison of FSE T2W and 3D FIESTA sequences in the evaluation of posterior fossa cranial nerves with MR cisternography.

PURPOSE: The aim of this study was to compare 3D fast imaging with steady state acquisition (3D FIESTA) to fast spin echo T2-weighted (FSE T2W) MRI sequences in the imaging of cisternal parts of cranial nerves V-XII. MATERIALS AND METHODS: We retrospectively evaluated the temporal MRI sequences of 50 patients (F:M ratio, 27:23; mean age, 44.5 +/- 15.9 years) who were admitted to our hospital with vertigo, tinnitus, and hearing loss. In all, we evaluated 800 nerves. Two radiologists, working independently, divided the imaging findings into 3 groups: 0 (not visualized), 1 (partially visualized)…

Aberrant internal carotid artery in the middle ear.

This study was undertaken to evaluate the incidence, typical clinical signs, and proper management of patients with an aberrant internal carotid artery in the middle ear as presented in a case report and review of the relevant literature. A total of 86 cases were reviewed concerning gender, affected side, presenting symptoms, findings on clinical and radiologic examination, and further treatment. About two thirds of the patients (68.6%) were female. The right side was affected in 40 patients, and both sides were affected in 13 patients. Hearing loss (41 patients) and pulsatile tinnitus (26 pat…

[Bilateral vertebral artery dissection and its chronological changes detected by MR angiography: a case report].

A case of intracranial bilateral vertebral artery (VA) dissection presenting with ischemic symptoms which following unilateral dissection is presented. A 42-year-old male with an occipital headache was pointed out right vertebral artery stenosis with magnetic resonance (MR) angiography 8 day before admission. He admitted to our hospital complaining of severe vertigo and tinnitus. MR images and cerebral angiograms revealed bilateral VA dissection with infarcts in light lower surface of cerebellum perfused by posterior inferior cerebellar artery and right hypothalamus. Conservative therapy was a…

Enlargement of a vestibular schwannoma after stereotactic radiotherapy.

BACKGROUND: Radiosurgery has been increasingly advocated as a primary treatment for vestibular schwannoma (VS), and recently fractionation of the dose has been proposed as a method to decrease the risk of radiation injury when treating larger tumors. METHOD: The authors describe a 48-year-old woman who presented with right-sided hearing loss and new-onset tinnitus, with a progressive decrease in facial sensation. The diagnosis of a large right cerebellopontine angle VS was made on magnetic resonance imaging (MRI). The patient was treated with a course of fractionated stereotactic radiotherapy…

Symptomatic unruptured capillary telangiectasia of the brain stem: report of three cases and review of the literature.

Three young patients with transient or intermittent focal neurological signs suggesting brain stem involvement are described, in whom high field MRI showed focal areas of hyperintensity in T2 weighted spin echo images, hypointensity in T2* weighted gradient echo images, and enhancement in postcontrast T1 weighted images consistent with unruptured capillary telangiectasia of the brain stem. The first patient was a 28 year old woman who complained of recurrent left ear tinnitus, exacerbated during the menstrual period; MRI demonstrated that the vascular anomaly involved the left acoustic pathway…
