Balloon Anchor Technique to Aid Closure of Coronary Fistula Using Vascular Plug.

Coronary arteriovenous fistulas (CAFs) are abnormal communications between coronary arteries and cardiac chambers or great vessels.1 They are usually congenital but may be acquired after trauma, surgery, or percutaneous procedures. Although often incidental in childhood, hemodynamically significant CAFs in adults can cause myocardial ischemia, arrhythmia, heart failure, or endarteritis.2 Evidence to guide management is limited, and multidisciplinary evaluation at centers experienced in both percutaneous and surgical approaches is recommended.3 A 60-year-old man with hypertension and prior left

Measuring patient satisfaction with education using extended reality in orthognathic surgery: Development, validation, and reliability of the 3D orthognathic surgery questionnaire (3DOS-Q).

OBJECTIVES: The effectiveness of extended reality technologies in improving patient understanding and satisfaction, particularly in orthognathic surgery, remains underexplored. No existing questionnaire specifically assesses patient satisfaction with different 3D model display techniques in this context. Therefore, this research aimed at developing and validating a questionnaire to assess patient satisfaction with preoperative education using 3D models for orthognathic surgery. METHODS: Building on a previous questionnaire for trauma patients, an initial questionnaire was developed and adapte

Immediate Tendon Transfer for Functional Reconstruction in Upper Limb Sarcoma Surgery: An Observational Study.

INTRODUCTION: Upper limb sarcoma surgeries often require sacrificing major nerves to achieve oncologic control, leading to significant functional loss. In low-resource settings where microsurgical reconstruction is limited, immediate tendon transfer offers a practical solution for restoring function. This study evaluates functional outcomes following immediate tendon transfer in patients undergoing nerve-sacrificing sarcoma excision. METHODS: This multicentric observational study was conducted at four tertiary centers in Nepal: KIST Medical College and Teaching Hospital, National Trauma Centr

Internal joint stabilizer vs. stand-alone ligament repair in treatment of traumatic elbow instability: an age-matched analysis.

BACKGROUND: Traumatic elbow instability is a complex problem, and treatment often involves addressing bony injury followed by ligament and capsular repair with or without external fixation. The internal joint stabilizer (IJS) is a dynamic internal fixator that is an alternative treatment for addressing soft tissue injuries that has gained popularity among treating surgeons. The purpose of this study is to compare the outcome of an IJS with conventional primary repair approaches in the treatment of traumatic elbow instability. METHODS: Patient demographics, injury characteristics, surgical fin

Delayed diagnosis of femoral neck fracture: A case report and literature review.

INTRODUCTION: Femoral neck fractures are common in elderly resulting from minor trauma. In resource limited settings, these fractures are sometimes missed in radiographic interpretation which delays the treatment and risk of morbidity and mortality increases. CASE PRESENTATION: We report the case of a 72-year-old male who presented to a primary healthcare facility with complaints of pain in the left hip and lower back following a fall. He was misdiagnosed with a soft tissue injury and managed conservatively with pelvic traction. Due to persistent symptoms and inability to bear weight, he soug

Causes of Death and Screening for Toxicants and Hemopathogens of European Hedgehogs (Erinaceus europaeus) from a Wildlife Rehabilitation Center in Northern France.

The European hedgehog (Erinaceus europaeus), common in rural and suburban environments, is perceived as declining within its native range, prompting conservation concerns. This study of hedgehogs admitted to a French rehabilitation center aimed to identify causes of death, detect potential emerging diseases, and systematically screen dead hedgehogs for toxicants and hemopathogens. Using clinical information, necropsy examination, and ancillary testing in 159 dead hedgehogs, we identified the primary cause of death of 92% of them. The leading causes of death were impaired general condition (30%

Multidisciplinary management of high-grade pediatric liver injuries.

OBJECTIVE: To present our experience of multidisciplinary management of high-grade pediatric liver injuries. INTRODUCTION: Pediatric high-grade liver injuries pose significant challenge to management due to associated morbidity and mortality. Emergency surgical intervention to control hemorrhage and biliary leak in these patients is usually suboptimal. Conservative management in selected high-grade liver injuries is now becoming standard of care. Management of hemobilia due to pseudoaneurysm formation and traumatic bile leaks requires multidisciplinary management. METHODS: A retrospective re

A bipartite medial cuneiform bone: a rare variant with symptomatic presentation and the classic "E-sign".

BACKGROUND: Bipartite medial cuneiform bone (BMC) is located at the Lisfranc joint of the midfoot, and it represents a rare variant involving two separate ossification centers in the medial cuneiform bone. Although BMC is typically asymptomatic, it can become clinically relevant under conditions of trauma or chronic stress, affecting foot stability. CASE REPORT: The current imaging report describes a 48-year-old female presenting with chronic dorsal midfoot pain, worsened by extended standing and ambulation. Physical examination indicated mild tenderness without swelling or ecchymosis. Magnet

Disparities in pancreatic cancer care and research in Native Americans: Righting a history of wrongs.

Disparities in pancreatic cancer incidence and outcomes exist in Native American populations. These disparities are multifactorial, difficult to quantify, and are influenced by historical, socioeconomic, and health care structural factors. The objective of this article was to assess these factors and offer a call to action to overcome them. The authors reviewed published data on pancreatic cancer in Native American populations with a focus on disparities in incidence, outcomes, and research efforts. The historical context of the interactions between Native Americans and the United States healt

Is pin-plate overlap in tibial plateau fractures associated with increased infection rates?

INTRODUCTION: Despite advances in the treatment of high energy proximal tibia fractures, including the utilization of staged management with external fixation, the infection rate remains high. Overlap between external fixator pin sites and definitive internal fixation has been proposed as a risk factor for infection. METHODS: This retrospective study reviews 244 patients with staged knee-spanning external fixation followed by delayed definitive internal fixation at two separate level one trauma centers. Presence of pin-plate overlap as well as several other known risk factors for infection we

Patient blood management in India - Review of current practices and feasibility of applying appropriate standard of care guidelines. A position paper by an interdisciplinary expert group.

In a developing country like India, with limited resources and access to healthcare facilities, dealing with massive hemorrhage is a major challenge. This challenge gets compounded by pre-existing anemia, hemostatic disorders, and logistic issues of timely transfer of such patients from peripheral hospitals to centers with adequate resources and management expertise. Despite the awareness amongst healthcare providers regarding management modalities of bleeding patients, no uniform Patient Blood Management (PBM) or perioperative bleeding management protocols have been implemented in India, yet.

Deep Learning Assisted Detection of Abdominal Free Fluid in Morison's Pouch During Focused Assessment With Sonography in Trauma.

Background: The use of focused assessment with sonography in trauma (FAST) enables clinicians to rapidly screen for injury at the bedsides of patients. Pre-hospital FAST improves diagnostic accuracy and streamlines patient care, leading to dispositions to appropriate treatment centers. In this study, we determine the accuracy of artificial intelligence model-assisted free-fluid detection in FAST examinations, and subsequently establish an automated feedback system, which can help inexperienced sonographers improve their interpretation ability and image acquisition skills. Methods: This is a si

Self-care of Frontline Health Care Workers: During COVID-19 Pandemic.

The world is amidst the COVID-19 pandemic that has used social distancing as a tool for containing the virus from spreading exponentially among other individuals. Previous literature suggests that human contact and attachment is a key for well-being which is why punishments like solitary confinement in detention centers like jail has always been debated as being torturous (Wolfendal 2020). With this notion, anxiety and stress may become more prevalent in individuals who experience self-isolation or are under a forced lockdown. For health-care workers like doctors and psychologists, who advocat
