Metastatic coccidioidal endophthalmitis.

A severe granulomatous iridocyclitis developed in association with a cavitary pulmonary lesion in a 29-year-old man. The initial diagnosis and treatment was for pulmonary tuberculosis with tuberculous uveitis. Although the pulmonary lesion improved with antituberculous therapy, the condition of the eye deteriorated. An anterior-chamber tap was positive for Coccidioides immitis, and the patient was treated with intravenous and two intracameral injections of amphotericin B. The eye was ultimately enucleated three weeks after the initial intracameral injection, and yet was culture-positive for th

An unusual presentation of miliary tuberculosis with medullary nephrocalcinosis in an infant.

Miliary tuberculosis (MTB) is a rare and potentially fatal manifestation of Mycobacterium tuberculosis infection, resulting from hematogenous dissemination of the primary infection. Although more commonly seen in immunocompromised individuals, infants are also susceptible due to their immature immune systems. We present the case of an 88-day-old female with MTB and extrapulmonary disease involving the liver, spleen, kidneys, and retina. Imaging demonstrated diffuse pulmonary nodules, hepatic and splenic granulomas, and nephrocalcinosis. This case highlights the hallmark radiologic features see

Extrapulmonary Tuberculosis with Seminal Vesicle Involvement in an Immunocompetent Patient.

Extrapulmonary tuberculosis often presents diagnostic challenges due to its involvement in multiple organ systems. We report a case of a 54-year-old, immunocompetent man who presented with prolonged fever and a generalized seizure. Neuroimaging revealed ring-enhancing lesions suggestive of tuberculomas. Further evaluation demonstrated a seminal vesicle abscess and cervical lymphadenitis. Histopathology and microbiological analysis confirmed Tuberculosis. The patient was initiated on antitubercular therapy, along with anti-convulsants and steroids. This case highlights the importance of conside

Disseminated sporotrichosis masquerading as tuberculosis in a patient without overt immunodeficiency: challenges in tuberculosis-endemic area.

Sporotrichosis is a mycosis with multitudinous clinical manifestation based on varying factors. We report a 78-year-old man who came to an out-patient clinic for progressive ulcerative cutaneous lesions for 6 months with concurrent pulmonary symptoms and osteomyelitis. Initial investigations were negative for infectious organisms and therapeutic trial of standard anti-tuberculous regimen failed to improve clinical symptoms. Subsequent fungal culture from the ulcerative skin showed dark colonies of Sporothrix schenckii complex. A review of sporotrichosis cases mimicking tuberculosis is provided

From Triumph to Trial: A Case Study of Non-Tuberculous Mycobacterium After Tetralogy of Fallot (TOF) Correction in an Immunocompetent Child.

Mycobacterium simiae, a slow-growing non-tuberculous mycobacterium (NTM), presents diagnostic challenges due to its resemblance to Mycobacterium tuberculosis and other NTMs. While NTM infections and tuberculosis share clinical and radiological features, their management strategies differ. Accurate differentiation is pivotal, as misdiagnosing NTM infections such as MDR-TB can lead to ineffective treatments. A case involving an 11-year-old female with tetralogy of Fallot (TOF) and a pulmonary M. simiae infection underscores the importance of precise diagnosis. Enhancing diagnostic methods is imp

Fever and an abnormal chest X-ray during the COVID-19 pandemic.

During the COVID-19 pandemic, a 56-year-old man presented at our emergency department with fever and shortness of breath; Diffuse pulmonary nodular vascular spread lesions were found. Detailed history taking showed a four-week history of fever and night sweats. The man had been under treatment for 2 years with Adalimumab, a tumor-necrosis-factor (TNF) inhibitor, for ulcerative colitis. Before start, screening by tuberculin skin test was negative. Cultures en PCR on BAL and urine were positive for mycobacterium tuberculosis also ocular findings were present. The diagnosis of military tuberculos

Toward novel vaccines against tuberculosis: current hopes and obstacles.

Approximately 2 million people die of tuberculosis (TB) each year. The current vaccine, Bacille Calmette-Guérin (BCG), albeit widely employed, does not protect against adult pulmonary disease, and new vaccines are urgently needed to reduce the incidence of TB worldwide. New insights into the cellular and molecular mechanisms that underlie the interactions between Mycobacterium tuberculosis and its host have been exploited to develop novel vaccine candidates that recently have entered clinical trials. This review provides a brief overview of different approaches toward a new vaccination strateg

Comparative analysis of lymphocyte subsets in tuberculosis, NTM infections, and other respiratory diseases.

OBJECTIVE: This study aimed to analyze the differences in lymphocyte subsets and immune function between tuberculosis (TB) and nontuberculous mycobacterial pulmonary disease (NTM-PD), thereby deepening the understanding of the pathogenesis of these diseases and providing important insights for clinical diagnosis, treatment, and prognosis evaluation. METHODS: Patients with pulmonary imaging abnormalities admitted to the Tuberculosis Department of Shanghai Pulmonary Hospital from January 2023 to December 2023 were included. Based on diagnostic assessments, they were categorized into active tube

[Respiratory function rehabilitation: a component of treatment for tuberculosis].

The present views of pathophysiological processes in tuberculosis make it necessary not only to suppress the vital activity of Mycobacterium tuberculosis, but also to restore biophysical immune and other protective functions of an organism. The acutely progressive forms of tuberculosis isolating drug-resistant mycobacteria require that the exposure techniques improving circulation and metabolic and bioenergetic processes in the cellular and tissue structures at the site of a specific tuberculous process be compulsorily included. Rehabilitative measures considerably accelerate reparative proces

Transmission of Mycobacterium tuberculosis from an infant.

SETTING: This report investigates the unusual transmission of Mycobacterium tuberculosis from a 12-week-old infant with nosocomially acquired tuberculosis (TB). Compliance with recommendations on the post-exposure management of young children is described. DESIGN: Contacts of an infant case of TB were identified and recommended to undergo baseline and post-exposure tuberculin skin tests (TST) as per Canadian TB standards. TST conversion was measured at least 8 weeks post exposure. Children aged <6 years were recommended to initiate preventive treatment with isoniazid (INH) until their post-ex

Unilateral post-tuberculous lung destruction: the left bronchus syndrome.

In a prospective study of 13 patients requiring pneumonectomy for unilateral post-tuberculous lung destruction the left side was found to be affected in 12. Review of a further 172 cases showed the left lung to have been destroyed in 109 (63%). It is suggested that this predominance of the left side is due to the anatomical characteristics of the left main bronchus and that disordered haemodynamics also appear to play a part.

[The risk of grave evolution of tuberculosis in young children (under 2 years of age)].

Between 1971 and 1972, 22% of all children under two years with tuberculosis presented with severe forms of the disease (meningitis, miliaria, caseous forms). In the period 1974-1975, although the number of cases in the group was reduced by half, the severe forms still represented 20,4% of the total. The risk of severe evolution of tuberculosis decreases rapidly, from 55--66% in the first months of life. to 14,2--15,5% at 1--2 years. The risk of death in children aged between 0 and 2 years with tuberculosis (all forms) was of 26,6--58,3% between 0 and 5 months but only 4,5--9,3% in the second
