Pulmonary veno-occlusive disease: an uncommon cause of pulmonary hypertension.

Pulmonary veno-occlusive disease (PVOD) is a rare and challenging cause of pulmonary hypertension. Clinical presentation is non-specific, including dyspnoea, cough and fatigue. Diagnosis of PVOD is typically based on high clinical suspicion with a definitive diagnosis confirmed by histology. Our case involves a healthy 21-year-old man who developed dyspnoea on exertion at an elevated altitude during deployment to Afghanistan. His work-up included an echocardiogram, a high-resolution CT scan, V/Q scan, pulmonary function tests with diffusion capacity, and a cardiac catheterisation with vasodila

Aortic valve tethering: embryonic remnant of a common origin?

A 14-year-old asymptomatic girl without relevant medical history was referred to our department for heart murmur evaluation. The echocardiogram showed cardiac chambers with normal size and function. Noteworthy was the presence of an apparently fibrous tissue joining the ventricular surfaces of the aortic non-coronary and right coronary leaflets with the anterior mitral leaflet. Both valves were slightly thickened and there was a mild anterior mitral valve 'billowing' causing an eccentric mild-to-moderate regurgitant jet. During systole, tethering of this tissue caused the incomplete opening of

Acute myocardial infarction in a patient with essential thrombocythemia: successful treatment with percutaneous transluminal coronary recanalization.

A 65-year-old woman with essential thrombocythemia (ET) was admitted to hospital where she was diagnosed as acute myocardial infarction (AMI). Because of abundant thrombus of right coronary arteries, percutaneous transluminal coronary recanalization by administration of urokinase was selected as the reperfusion therapy, resulting in successful revascularization with Thrombolysis in Myocardial Infarction grade III coronary flow. The maximum creatine kinase reached 507 IU/L, and left ventriculography performed at 1 month after initiation of both anticoagulant and antiplatelet therapies revealed

Current therapy of infants with congenital diaphragmatic hernia.

Congenital diaphragmatic hernia (CDH) is a life-threatening anomaly with a significant mortality rate. Despite widespread prenatal diagnosis, few parameters have been well defined to aid in prediction of outcome of these infants. Antenatal maternal steroid administration and foetal surgery are not proven interventions. Postnatal treatment has changed over the last 10 years, with avoidance of hyperventilation and ventilator-induced lung injury resulting in improved survival. Therapies such as inhaled nitric oxide, exogenous surfactant administration and extracorporeal membrane oxygenation (ECMO

Long-term follow up of children with the Wilson-Mikity syndrome.

Nine children who survived the development of Wilson-Mikity (W-M) syndrome were studied at a mean age of 7.6 +/- 0.3 years. Persistent respiratory symptoms during the year preceding the study were present in one child. Pulmonary function studies demonstrated a significant degree of airway obstruction in 3 children. Positive methacholine challenge was observed in 6 children indicating bronchial hyperreactivity. Abnormal chest radiographs were present in 3 children. One showed the characteristic appearances of asthma with air trapping, bronchial wall thickening and redistribution of pulmonary ve

Survivors of Cyanotic Congenital Heart Disease: A Review.

IMPORTANCE: Cyanotic congenital heart diseases, which occur in approximately 0.2% of live births in the United States, have high mortality rates if untreated, but survival to adulthood is common with current surgical interventions. OBSERVATIONS: Approximately 80% of all cyanotic congenital heart diseases are caused by tetralogy of Fallot (TOF), D-loop transposition of the great arteries (TGA), and congenital heart disease with single-ventricle circulation. Survivors of cyanotic congenital heart diseases benefit from multidisciplinary care including pediatric and adult cardiologists, congenita

Genetic regulation of right ventricular trabeculation identifies developmental and cardiopulmonary pathways.

Right ventricular (RV) trabeculation may reflect adaptation to loading conditions and imprinting of early developmental processes, yet its genetic determinants and clinical relevance in adults remain poorly understood. In contrast to the left ventricle, the RV has distinct developmental origins, geometry and loading conditions, suggesting chamber-specific mechanisms of trabecular remodelling. Using deep learning-based image segmentation and fractal dimension analysis, we quantified RV trabecular complexity in diastole and systole in 48,118 UK Biobank participants with genetic data. RV trabecul

Hemodynamic Effects of Awake Prone Positioning With COVID-19 Acute Respiratory Failure.

INTRODUCTION: Awake prone positioning (PP) reduces need for intubation for patients with COVID-19 with acute respiratory failure. We investigated the hemodynamic effects of awake PP in non-ventilated subjects with COVID-19 acute respiratory failure. METHODS: We conducted a single-center prospective cohort study. Adult hypoxemic subjects with COVID-19 not requiring invasive mechanical ventilation receiving at least one PP session were included. Hemodynamic assessment was done with transthoracic echocardiography before, during, and after a PP session. RESULTS: Twenty-six subjects were included

The heart in congenital diaphragmatic hernia: Knowns, unknowns, and future priorities.

There is growing recognition that the heart is a key contributor to the pathophysiology of congenital diaphragmatic hernia (CDH), in conjunction with developmental abnormalities of the lung and pulmonary vasculature. Investigations to date have demonstrated altered fetal cardiac morphology, notably relative hypoplasia of the fetal left heart, as well as early postnatal right and left ventricular dysfunction which appears to be independently associated with adverse outcomes. However, many more unknowns remain, not least an understanding of the genetic and cellular basis for cardiac dysplasia an

Electrocardiographic prediction of late gadolinium enhancement on cardiac magnetic resonance in Becker muscular dystrophy.

Dystrophin deficiency results in cardiomyopathy and fibrosis with variable onset. Little is known about electrocardiographic abnormalities in Becker muscular dystrophy and their relationship to underlying cardiac pathology. We hypothesized QRS fragmentation is associated with myocardial fibrosis on cardiac magnetic resonance imaging in Becker muscular dystrophy patients. We retrospectively evaluated 44 patients, and extracted data from clinically obtained electrocardiogram and cardiac magnetic resonance. Ventricular function and presence or absence late gadolinium enhancement representing myoc

Acute phase proteins altered in the plasma of patients with congenital ventricular septal defect.

PURPOSE: Ventricular septal defect (VSD) has intracardiac left-to-right shunt and increased pulmonary flow that may affect the acute phase response (APR). We examined the hypothesis that plasma proteins of VSD patients may be altered. EXPERIMENTAL DESIGN: 2DE and MS were used to detect differential plasma proteins in VSD patients (n = 55) and controls (n = 70). Candidate APR proteins were confirmed by ELISA in new samples. RESULTS: Among three differentially expressed APR proteins from 322 protein spots detected, haptoglobin (0.4 ± 0.04 versus 0.6 ± 0.07 mg/mL; p = 0.016) and serum amyloid P

[Severe hydrogen sulfide intoxication: a pediatric case of survival].

We report a paediatric case of survival following severe hydrogen sulfide (H2S) gas intoxication. A 13-year-old boy was found submerged to the neck in a manure tank. He was hypothermic, unresponsive with bilateral mydriasis, and had poor oxygen saturation. After intubation, he was transferred to the paediatric intensive care unit of a tertiary care children's hospital. He developed acute respiratory distress syndrome (ARDS) requiring high frequency percussive ventilation. Cardiac evaluation was significant for myocardial infarction and left ventricular function impairment. He completely recove

Echocardiographic abnormalities and their correlation with bronchiectasis score in primary antibody deficiencies.

BACKGROUND: Primary antibody deficiencies are characterized by defective antibody production and recurrent infections. Patients usually present with recurrent respiratory tract infections with consequent chronic pulmonary damage and bronchiectasis, which could potentially influence cardiac function. Our aim was to assess noninvasively the cardiac complications due to pulmonary disease in patients with primary antibody deficiency. METHODS: A cross-sectional series of patients with primary antibody deficiency syndromes from our referral immunology center were recruited. Individuals undergoing h

Tricuspid annular systolic velocity predicts maximal oxygen consumption during exercise in adult patients with repaired tetralogy of Fallot.

BACKGROUND: Many patients with repaired tetralogy of Fallot (TOF) have clinically important pulmonary regurgitation that can eventually lead to right ventricular (RV) dilatation and dysfunction and associated morbidity and mortality. Unfortunately, standard echocardiographic techniques to evaluate RV size and function can be inaccurate. Newer Doppler modalities such as Doppler tissue imaging (DTI) can detect subtle changes in myocardial velocities and may better identify subclinical RV dysfunction in these patients. METHODS: A total of 25 patients with repaired TOF prospectively underwent com

Potential implications of the helical heart in congenital heart defects.

The anatomic and functional observations made by Francisco Torrent-Guasp, in particular his discovery of the helical ventricular myocardial band (HVMB), have challenged what has been taught to cardiologists and cardiac surgeons over centuries. A literature debate is ongoing, with interdependent articles and comments from supporters and critics. Adequate understanding of heart structure and function is obviously indispensable for the decision-making process in congenital heart defects. The HVMB described by Torrent-Guasp and the potential impact on the understanding and treatment of congenital

[Diabetic cardiopathy. Pathophysiologic concepts and therapeutic approaches].

Epidemiological data reviewed suggest that diabetes itself increases the cardiac risk of diabetics (types I and II), independently from the development of coronary heart disease and in addition to other risk factors (hypertension, hypercholesterolemia, hypertriglyceridemia, smoking and others), presumably by a specific myocardial disease called "diabetic cardiopathy", or according to the recommendations of the WHO, "diabetic heart muscle disease." Disturbances of the left and right ventricular function as well as the autonomic function of the heart can be understood as signs of this specific c

PRIME-HFrEF Trial: a randomized, double-blind, multi-dose umbilical cord-derived mesenchymal stem cell regimen for heart failure.

The safety of multi-dose mesenchymal stem cell (MSC) regimens has seldom been systematically investigated. The PRIME-HFrEF (Prospective Randomized Controlled Study of Multiple Intravenous Infusions of Umbilical Cord-derived MSCs in Patients with Heart Failure and Reduced Ejection Fraction) trial was a single-center, randomized, placebo-controlled, investigator-initiated study (ClinicalTrials.gov identifier: NCT04992832) that enrolled 40 patients. The trial aimed to evaluate the safety of three intravenous infusions of Umbilical Cord-derived MSCs (UC-MSCs) administered at six-week intervals in

Associations between cardiometabolic traits and diabetic cardiomyopathy: an imaging-based analysis.

INTRODUCTION: Diabetic cardiomyopathy (DCM) often evades diagnosis before manifestation of clinical symptoms. In this study we explored how cardiometabolic traits influence early cardiac structure and function in asymptomatic people living with diabetes (PwD), using advanced imaging. METHODS: We conducted a cross-sectional study of 88 participants: 57 people living with type 2 diabetes (PwT2D), 16 people living with type 1 diabetes (PwT1D) and 15 controls. All subjects underwent transthoracic echocardiography and/or cardiac magnetic resonance (CMR) imaging. Strain analysis, perfusion indices,

Echocardiographic Indices of Cardiac Remodeling Following Transcatheter Edge-to-Edge Repair for Secondary Mitral Regurgitation: A Systematic Review and Meta-Analysis.

BACKGROUND: Secondary mitral regurgitation is associated with adverse clinical outcomes and cardiac remodeling. While transcatheter edge-to-edge repair (TEER) improves symptoms and prognosis in selected patients, its effects on cardiac remodeling remain debated. This systematic review and meta-analysis aimed to evaluate the impact of TEER on echocardiographic indices of cardiac remodeling. METHODS: PubMed, Scopus, and Cochrane Library were used to identify relevant studies assessing echocardiographic changes before and after TEER. Outcomes of interest were left ventricular (LV) ejection fract
