An outcome study of an inpatient treatment program for pathological gamblers.

In 1972 the Brecksville Unit of the Cleveland Veterans Administration Medical Center began the first inpatient treatment program for pathological gambling in the United States. The 30-day, highly structured gambling treatment program aims for abstinence from gambling, reduction of the urge to gamble, and restoration of a maximum level of social functioning. The authors report the results of a preliminary outcome study of 60 former patients who completed a survey form rating various aspects of their lives one year postdischarge. Fifty-five percent of the respondents reported complete abstinence

The Impact of Homelessness on Mortality of Individuals Living in the United States: A Systematic Review of the Literature.

This systematic review assessed peer-reviewed research studies on mortality rates of the homeless population within the United States. Extrapolated data included definitions of homelessness, mortality data sources, findings on mortality rates, and causes of premature mortality. Results demonstrate that individuals experiencing homelessness die earlier than comparison groups not experiencing homelessness. Methodology and findings varied across studies. Subpopulations included veterans, families, youth, and unsheltered. Causes of death varied across subpopulations and changed over time. Top caus

Small airways disease in an Operation Desert Storm Deployer: Case report and review of the literature on respiratory health and inhalational exposures from Gulf War I.

Constrictive Bronchiolitis (CB) has been reported in US Operation Iraqi Freedom/Enduring Freedom (OIF/OEF) deployers but not in those from prior US conflicts. A 62-year old presented with progressive dyspnea 13 years after deployment to the Persian Gulf in 1991-1992, where he was exposed to burning oil well fire emissions, dust storms, and other potential airborne hazards. In 2014, after a chest computed tomography (CT) scan demonstrated diffuse mosaic attenuation, he underwent surgical lung biopsy, which revealed CB. Deployers from both GWI and OIF/OEF share many exposures. As respiratory sym

Coronary angiography findings in lung injured patients with sulfur mustard compared to a control group.

AIMS AND OBJECTIVES: We evaluated the incidence of coronary artery disease (CAD) in Sulfur mustard (SM) exposed veterans. We also evaluated the relationship between exposure to SM and angiography findings and compared angiography findings of SM exposed individuals with unexposed ones after two decades from the time of exposure to SM. MATERIALS AND METHODS: A case-control study was conducted on 200 consecutive patients (100 SM exposed vs. 100 unexposed) undergoing angiographic assessments due to CAD. RESULTS: The coronary angiography findings between two groups were significantly different (

Pulmonary health effects in Gulf War I service members exposed to depleted uranium.

OBJECTIVE: In a population of Gulf War I veterans who sustained inhalational exposure to depleted uranium during friendly fire incidents in 1991, we evaluated whether those with high body burdens of uranium were more likely to have pulmonary health abnormalities than those with low body burdens. METHODS: We compared self-reported respiratory symptoms, mean pulmonary function values, and prevalence of low-dose chest computed tomography abnormalities between high and low urine uranium groups. RESULTS: We found no significant differences in respiratory symptoms, abnormal pulmonary function valu

Effect of transition home from combat on risk-taking and health-related behaviors.

Transition home following a combat deployment involves a period of adjustment. Exploratory and confirmatory factor analyses of a new 16-item transition scale were conducted with 2 samples and resulted in 4 factors (Benefit, Appreciation, Anger/Alienation, and Guilt/Remorse). In Study 1 (N = 1,651), the number of combat events was positively related to Anger/Alienation 4 months later even after controlling for posttraumatic stress disorder (PTSD) symptoms, partial r = .18, p < .001. In Study 2 (N = 647), after controlling for PTSD symptoms, Anger/Alienation assessed at 4 months postdeployment p

Effects of posttraumatic stress disorder status and covert hostility on cardiovascular responses to relived anger in women with and without PTSD.

Previous literature has found greater heart rate (HR) and blood pressure (BP) responses during relived anger, and a positive association between covert hostility and relived anger, in male veterans with posttraumatic stress disorder (PTSD). This study investigated hostility and cardiovascular responses to a relived anger task in 120 women (70 with PTSD and 50 without PTSD). Women with PTSD reported greater hostile beliefs and covert hostility than non-PTSD controls, reported greater anger and anxiety during the anger recall task, and had higher resting HR. In general, the relationship between

Managements of placenta accreta.

From January 1986 to June 1990 at Veterans General Hospital-Taipei, 12 cases of placenta accreta were reviewed. The incidence were 1 per 1,101 deliveries. Eight cases were confirmed by pathological examination, and the incidence was 0.06% (1/1,652). Placenta previa was found in 7 cases (58.3%); 6 of them had history of cesarean section. All the cases had history of uterine surgery. Seven cases underwent cesarean section, and the other one received myomectomy. Hysterectomy was performed on 6 patients, and conservative treatments were employed on the other 6 cases. No maternal mortality occurred

Nosocomial infections in long-term facilities.

We studied the incidence and prevalence of hospital-acquired infections in our intermediate-care units and the Nursing Home Care Unit at the Veterans Administration Medical Center, Johnson City, TN over a 4-year period (1980 through 1983). The global infection rate was 3.86 per 1,000 patient care days. The lower respiratory tract was the most common site of infection, followed by urinary tract infections, skin infections, bacteremia, wound infections, and infections at other sites. The prevalence study conducted by monthly visits over a 1-year period showed similar results. All of our patients

Pioglitazone for the treatment of alcohol use disorder: A randomized controlled trial.

BACKGROUND AND AIMS: Current medications for alcohol use disorders (AUD) are under-used and of moderate benefit; new or improved treatments are much needed. Preclinical and small clinical studies indicate that pioglitazone, a thiazolidinedione agonist of the peroxisome proliferator-activated receptor gamma, may reduce alcohol use. The primary purpose of this study was to evaluate, in a randomized trial, the effect of pioglitazone on alcohol use and craving in adults with AUD. DESIGN: A double-blind, placebo-controlled randomized trial. SETTING: Enrollment occurred in the United States at two

Cross-Ancestry Associations of Spontaneous Coronary Artery Dissection Genetic Risk With Coronary Atherosclerosis and Migraine Headache.

BACKGROUND: Research studies of spontaneous coronary artery dissection (SCAD) have been primarily focused on European-ancestry individuals, with limited recognition and investigation in non-European-ancestry individuals. While SCAD has not been well ascertained in non-European-ancestry groups, pleiotropic associated traits identified in those of European ancestry have been assessed in individuals of other ancestries. Whether these traits are associated with the complex genetic architecture of SCAD in those of non-European ancestry has not been previously investigated. METHODS: We investigated

Evaluation of Anti-Agitation Medication Prescribing Patterns by Age in the Emergency Department.

BACKGROUND: Acute agitation frequently occurs in the emergency department. Appropriate management is critical for the safety of all parties involved. Benzodiazepines and antipsychotics are commonly used for agitation, but safety concerns exist with these medications in older adults, even with acute use. The purpose of this study was to compare prescribing practices of anti-agitation medications between adults aged 18 to 64 years and those aged ≥ 65 years. METHODS: This study was a retrospective chart review of patients who presented to the Veteran Affairs Southern Nevada Healthcare System eme

Outcomes of Percutaneous Coronary Intervention for Protected versus Unprotected Left Main Coronary Artery Disease: Insights from the VA CART Program.

BACKGROUND: Practice patterns and outcomes of protected left main (PLM) and unprotected left main (ULM) percutaneous coronary intervention (PCI), as well as the differences between these types of PCI, are not well defined in real-world clinical practice. METHODS: Data collected from all Veteran Affairs (VA) catheterization laboratories participating in the Clinical Assessment Reporting and Tracking Program between 2009 and 2019. The analysis included 4,351 patients undergoing left main PCI, of which 1,306 pairs of PLM and ULM PCI were included in a propensity matched cohort. Patients and proc

Innovative Approaches to Engaging Homeless and Marginally Housed Patients in Care: a Case Study of Hepatitis C.

BACKGROUND: Homeless and marginally housed (HAMH) individuals experience significant health disparities compared to housed counterparts, including higher hepatitis C virus (HCV) rates. New direct-acting antiviral (DAA) medications dramatically increased screening and treatment rates for HCV overall, but inequities persist for HAMH populations. OBJECTIVE: This study examines the range of policies, practices, adaptations, and innovations implemented by Veteran Affairs Medical Centers (VAMCs) in response to Veterans Health Administration (VHA)'s 2016 HCV funding allocation to expand provision of

Differences in postinjury auditory system pathophysiology after mild blast and nonblast acute acoustic trauma.

Hearing difficulties are the most commonly reported disabilities among veterans. Blast exposures during explosive events likely play a role, given their propensity to directly damage both peripheral (PAS) and central auditory system (CAS) components. Postblast PAS pathophysiology has been well documented in both clinical case reports and laboratory investigations. In contrast, blast-induced CAS dysfunction remains understudied but has been hypothesized to contribute to an array of common veteran behavioral complaints, including learning, memory, communication, and emotional regulation. This in

Outcomes of ceftriaxone use compared to standard of therapy in methicillin susceptible staphylococcal aureus (MSSA) bloodstream infections.

BACKGROUND: Standard of care therapy (SOCT) for the treatment of methicillin susceptible staphylococcal aureus (MSSA) infections requires multiple daily infusions. Despite questionable efficacy due to high protein binding, ceftriaxone (CTX) is frequently used for treatment of MSSA at Hines VA Hospital. OBJECTIVE: The objective of this study was to determine clinical and microbiological outcomes in patients with MSSA bacteremia treated with CTX compared to SOCT. SETTING: This retrospective study was conducted at the Edward Hines, Jr. VA Hospital which is a comprehensive health care center ser

Is the length of newly diagnosed Barrett's esophagus decreasing? The experience of a VA Health Care System.

BACKGROUND & AIMS: Secular trends in the length of newly diagnosed Barrett's esophagus (BE) are unknown. We have anecdotally noticed less frequent new diagnoses of long segments of BE. METHODS: This is a retrospective analysis of prospectively collected information on a well-characterized large cohort of patients with documented BE that was diagnosed between 1981 and 2000 at Southern Arizona Department of Veterans Affairs Health Care System. We examined temporal changes in the length of BE at the time of diagnosis (frequency and proportions). We conducted correlation analyses, as well as mult

Modeling the symptom connections between posttraumatic stress disorder and comorbid severe mental illness.

INTRODUCTION: Although comorbidity between PTSD and severe mental illness (SMI; schizophrenia, bipolar disorder) is common, the underlying connections between disorders remain unclear. Network theory, a data-analytic technique for modeling complex relationships between several variables, examines relationships between individual symptoms of distinct disorders. This study used network analysis to explore symptom-level connections between PTSD and SMI to clarify pathways outlined in the interactive model between the disorders. METHOD: Participants were 126 Veterans with SMI (75.4 % male; 50.8 %
