[Surgical treatment of purulent corneal ulcers in eyes with neurotrophic keratitis and paralytic lagophthalmos].

AIM: to develop a surgical approach to the treatment of purulent corneal ulcer in eyes with neurotrophic keratitis with lagophthalmos after auditory neuroma removal or stroke. MATERIAL AND METHODS: A total of 12 patients (13 eyes) have been managed. All received combined surgery that included simultaneous keratoplasty, autoconjunctivoplasty, and permanent partial tarsorrhaphy. RESULTS: Ocular integrity was preserved in all cases, and also vision - in half of them. In 29% of cases, visual acuity increased from 0.09±0.05 up tо 0.21±0.13. Lagophthalmos decreased from 5.86±1.35 mm down to 3.01±0

Isolated bitemporal hemianopsia due to traumatic chiasmal syndrome.

A 20-year-old man presented with complaints of inability to see the outer half of objects and blurred near vision while reading. His complaints began one year ago after a motor vehicle accident that caused cranio-orbital fractures. Ocular examination revealed complete bitemporal hemianopsia and slight exotropia. Central visual acuity was 20/20 in both eyes. Radiologic studies demonstrated fractures in the fronto-ethmoid and sphenoid bones and thinning of the optic chiasm. No hypothalamic-pituitary abnormality was detected. Clinical findings remained unchanged during follow-up. Although rare, b

A case of vasculitis, retinitis and macular neurosensory detachment presenting post typhoid fever.

BACKGROUND: Ocular and extraocular immune-mediated phenomena are known to occur following febrile illness. Vasculitis, retinitis and neurosensory detachment are not well-recognized sequelae of typhoid fever. FINDINGS: We report a case of vasculitis, retinitis and macular neurosensory detachment presenting post typhoid fever. A 27-year-old female presented with decreased vision in right eye with history of typhoid fever (treated adequately 6 weeks prior). Her best corrected visual acuity in right eye was 20/125, N36. Fundus showed a patch of vasculitis and retinitis superior to the disc associ

Isolated oculomotor nerve palsy: a rare manifestation of internal carotid artery dissection.

A 58-year-old man reported sudden-onset binocular double vision that appeared 3 days earlier. He denied history of headache/cervical pain or trauma. He had a medical history of well-controlled diabetes, hypertension and dyslipidaemia. Neurological examination revealed a left-sided ptosis and binocular horizontal diplopia in dextroversion without apparent extraocular-muscle paresis or pupillary involvement. Other cranial nerves were spared as well as motor, sensory and coordination systems. There were no signs of ocular erythema, proptosis or palpable orbital mass. Brain MR angiography revealed

Magnetic resonance imaging of pachymeningeal enhancement in Vogt-Koyanagi-Harada disease.

Vogt-Koyanagi-Harada (VKH) disease is a systemic disease consisting of bilateral granulomatous panuveitis combined with cutaneous and neurologic manifestations. However, there have been few reports of brain magnetic resonance imaging (MRI) in VKH disease. A 54-year-old Korean woman presented with severe periorbital pain, blurred vision and meningismus. Ophthalmologic examination disclosed bilateral optic disc edema with peripapillary nerve fiber hemorrhages. Lumbar puncture revealed monocytic pleocytosis. After a diagnosis of VKH disease was made, the patient was treated with high-dose cortico

Cardiac dysrhythmias associated with ophthalmic atropine.

Atropine sulfate, a mydriatic and cycloplegic agent, is frequently used in patients undergoing glaucoma surgery. Trabeculectomy with peripheral iridectomy is the most common glaucoma surgery performed to decrease intraocular pressure and preserve vision. Systemic absorption of ophthalmic atropine does occur and may result in toxic and adverse side effects. Cardiac dysrhythmias are one of the major adverse reactions. This case study reviews three patients who had a trabeculectomy for glaucoma and received ophthalmic atropine. One patient received both systemic and ocular atropine. Two patients

Ocular motility problems after orbital decompression for dysthyroid ophthalmopathy.

Ocular motility problems of 50 consecutive patients following orbital decompression for dysthyroid (Graves') ophthalmopathy were analyzed retrospectively. No significant relationship to the development of postoperative diplopia was seen in the amount of retrodisplacement of the globe, the anatomical approach for orbital decompression, or the indication for decompression. Several clinical observations were made. Patients whose indication for orbital decompression was a vision-threatening ophthalmopathy were more likely (although not statistically significantly) to develop postoperative strabism

When Silicone Oil Goes Anterior: A Case of Anterior Segment Neovascularization After Complex Retinal Detachment Repair.

Silicone oil is widely used for intraocular tamponade in complicated retinal detachment surgery. Despite the efficacy of this treatment, complications may occur, including emulsification of the silicone oil in the anterior chamber. Chronic ischemia from recurrent retinal detachment or the development of proliferative vitreoretinopathy places an eye at risk for the development of iris rubeosis and/or intralenticular neovascularization. An 81-year-old pseudophakic female presented with a chief complaint of blurry vision in the left eye (OS) with no light perception in the right eye (OD) since re

Targeted Delivery of Avacopan via Sortase A-Modified Extracellular Vesicles Attenuates Endotoxin-Induced Retinal Inflammation.

Uveitis is a prevalent ocular inflammatory condition and a major contributor to global vision impairment. The inflammatory cascade activates the complement system, resulting in the production of C5a, which interacts with its receptor C5aR1 on microglia, promoting excessive neutrophil recruitment and reactive oxygen species (ROS) bursts. In this study, we develop a targeted drug delivery system by conjugating small extracellular vesicles with the microglia-binding peptide MG1 using Sortase A. The C5aR1 antagonist Avacopan was loaded into the modified EVs via extrusion. Using a lipopolysaccharid
