Proposal of Brain Plasticity Index Based on Navigated Transcranial Magnetic Stimulation: Metric of Functional Displacement for Language Function.

OBJECTIVE: To identify a brain plasticity index (BPI) that accounts for the cortical variation in language function on the basis of preoperative/postoperative navigated transcranial magnetic stimulation (nTMS) mapping. METHODS: This was a prospective, monocentric study of preoperative/postoperative nTMS mapping. A Python-based open-source pipeline was created to objectively quantify BPI maps, calculated with 2-dimensional nearest-neighbor interpolation of nTMS points for the 2 hemispheres (right/left). BPI maps registered on the Montreal Neurological Institute neuro-anatomy-atlas. Tumoral seg

The Utilization of Superficial Femoral Artery Access for Transcatheter Aortic Valve Replacement and Peripheral Vascular Interventions: A Systematic Review and Meta-Analysis.

The superficial femoral artery (SFA) is a vascular access alternative for various cardiovascular interventions, especially when the common femoral artery CFA is deep, calcified or in high bifurcation. SFA use, however, was associated with pseudoaneurysms in older literature, and its utilization for large-bore access is not well established. We conducted a systematic review, including randomized and observational studies that reported on SFA vascular access for peripheral arterial disease (PAD), and transcatheter aortic valve replacement (TAVR) from inception until October 2024. The pooled weig

Accuracy of a novel mixed reality surgical platform for total knee arthroplasty.

BACKGROUND: Computer-assisted navigation has improved surgeons' ability to achieve accurate implant placement in total knee arthroplasty (TKA). As technology evolves and new systems are introduced, it is imperative to evaluate their accuracy for achieving desired goals. METHODS: This work evaluated the accuracy of a novel mixed reality surgical guidance platform that uses a head-mounted device to measure patient bony anatomy, quantify soft tissue balance, and provide quantitative resection guidance overlaid on the surgical field. Accuracy is evaluated in a cadaveric simulated use study and in

Lymphatic drainage of the trunk: Radiation induced no flow zone of mastectomy skin flaps-A cross-sectional study.

Changes in the lymphatic flow from the trunk following mastectomy, sentinel lymph node biopsy, axillary lymph node dissection, and radiotherapy remain understudied. We aimed to illustrate the altered lymphatic drainage pathways of the trunk following breast cancer treatment. We conducted a retrospective review of patients with breast cancer who underwent truncal lymphography between December 2014 and January 2024. Using a standardized imaging protocol, we recorded dermal backflow patterns, zones of absent lymphatic flow, and drainage pathways to the surrounding lymph node basins. Patients were

Editor's Choice - Sex Specific Differences in Abdominal Aortic Aneurysm Morphology Based on Fully Automated Volume Segmented Imaging: A Multicentre Cohort Study and Propensity Score Matched Analysis.

OBJECTIVE: Sex specific anatomical differences may contribute to observed disparities in outcomes and suitability for endovascular aneurysm repair (EVAR) between men and women with abdominal aortic aneurysms (AAAs). This study aimed to assess these differences using fully automated volume segmentation (FAVS) and explore implications for EVAR suitability. METHODS: This was a retrospective, multicentre cohort study of patients undergoing elective AAA repair between 2013 and 2023 in three tertiary UK centres. Pre-operative computed tomography scans were analysed using FAVS software. Demographic

From 2D to 3D gamma passing rate tolerance and action limits for patient-specific quality assurance in volumetric-modulated arc therapy.

BACKGROUND: Volumetric modulated arc therapy (VMAT) requires an accurate patient-specific quality assurance (PSQA) program. In clinical practice, this is usually performed using the γ-index and the two-dimensional gamma passing rate (2D %GP). A three-dimensional (3D) index incorporating the patient anatomy could be more useful for the 3D dose distribution verification. PURPOSE: The current study demonstrates a thorough investigation of VMAT PSQA treatment plans by examining the correlation between 3D Gamma passing rate (%GP) and 2D %GP. The aim was to establish the tolerance limits (TL) and a

Enhancing CBCT-based CT synthesis using planning MRI in adaptive proton therapy for head cancer: A deep learning approach.

BACKGROUND: Proton therapy (PT) is recognized as a superior treatment for head cancer (HC) due to its precision and minimal damage to surrounding healthy tissues, relying on computed tomography (CT) data for dose calculations. Adaptive proton therapy (APT) is crucial to address changes in patient anatomy during treatment and update dose accuracy. However, in-room cone-beam CT (CBCT) assistance is limited to assessing patient setup, with occasional constraints due to artifacts and/or lower image quality and resolution compared to a CT scan. PURPOSE: Although deep learning (DL) techniques can s

Comparison of Cook Zenith and Relay plus Physician-Modified Endografts for Complex Abdominal and Thoracoabdominal Aortic Aneurysms.

BACKGROUND: Complex abdominal aortic aneurysms (CAAAs) or thoracoabdominal aneurysms (TAAs) often require urgent repair, but custom-made devices are unavailable in such settings. Physician-modified endografts (PMEGs) allow for rapid, patient-specific fenestrated or branched endografts. This study assessed clinical outcomes of CAAA and TAA PMEGs performed at a single Swedish center and compared results using the Cook Zenith and Relay Plus platforms. METHODS: A retrospective single-center review was conducted on all CAAA and TAA PMEG procedures at Linköping University Hospital between January 2

Casting techniques of equine hand and foot synovial cavities for the development of teaching models.

Horse joints are common sites of injury, orthopedic issues, and surgical and clinical interventions. For this reason, a thorough understanding of joint anatomy, including the boundaries of their recesses and their relationships with other structures of the locomotor apparatus, is essential. This study aimed to develop cast anatomical models of the synovial structures of the equine hand and foot, compare different casting materials and visualization methods, and identify the most suitable technique for enhancing the understanding of equine limb arthrology. Additionally, an anatomical descriptio

Multimodal Cardiac Imaging of Immune-Mediated Complications Post Heart Transplantation: A Contemporary Review.

Heart transplantation significantly enhances survival and quality of life for patients with end-stage heart failure. Despite advances in surgical and postoperative care, immune-mediated complications, acute graft rejection and cardiac allograft vasculopathy (CAV), remain major barriers to long-term success. Acute rejection predominantly affects early posttransplant survival, whereas CAV becomes a leading cause of mortality in later years. The gold standards for diagnosis, endomyocardial biopsy for rejection and coronary angiography for CAV, are invasive and imperfect. Noninvasive multimodality

Does residual foraminal stenosis at levels not covered by anterior cervical discectomy and fusion aggravate postoperative outcomes in cervical radiculopathy?

STUDY DESIGN: Retrospective cohort study. PURPOSE: To evaluate whether untreated foraminal stenosis (FS) at levels not included in anterior cervical discectomy and fusion (ACDF) impacts postoperative outcomes in patients with cervical radiculopathy and assess if residual FS increases the risk of adjacent segment disease (ASD). OVERVIEW OF LITERATURE: Level selection for ACDF for cervical radiculopathy is complex, considering variable patient anatomy and symptoms. It is unclear whether treating only potentially symptomatic levels could provide equivalent results to managing all pathologic lev

Advancing Understanding of Anorectal Malformations Through Microfocus Computed Tomography Imaging of Resected Material.

BACKGROUND AND AIMS: Patients born with anorectal malformations (ARMs) might experience constipation and fecal incontinence. During ARM surgery (anterior and posterior sagittal anorectoplasty procedure), the distal part of the bowel (fistula) is usually resected. Microfocus computed tomography (micro-CT) imaging, capable of imaging samples in ultra-high 3-dimensional resolution, can be used to learn from this resected material. Through this technique, we aim to investigate whether or not structures, such as the internal anal sphincter (IAS), are present within this fistula. METHODS: Pediatric

High versus low take-off rectal prolapse: Outcomes of tailored surgery.

BACKGROUND: Full-thickness rectal prolapse (FTRP) is heterogeneous, with outcomes varying despite similar anatomy. Cooper et al. proposed a radiological classification based on prolapse 'take-off' level on defecating proctography (DPG), hypothesising that high take-off (HTO) and low take-off (LTO) phenotypes may respond differently to abdominal versus perineal repair. This hypothesis has not previously been tested in a clinical cohort. METHODS: A retrospective observational cohort study was performed across three hospitals over 12 years (2012-2024). Patients undergoing laparoscopic ventral me
