Comparative studies of different types of osteosynthesis on the human corpse preparation in bilateral antero-lateral flail chest.

INTRODUCTION: Bilateral flail chest injuries are challenging in treatment and comparatively often require an operative stabilization of the anterior chest wall to re-establish normal physiological conditions of the chest wall in shape and statics. Various procedures have been described which are technically sophisticated for the surgeon. Consequently there is an increasing interest in potentials of operative care and their effectiveness on the anterolateral chest wall. MATERIALS AND METHODS: 12 Human cadavers were prepared and the natural Sternum Position (NP) was marked. A digital probe was

Reduced invasive and muscle-sparing operative approaches to the posterolateral chest wall provide an excellent accessibility for the operative stabilization! : Minimized approaches to the posterolateral chest wall.

PURPOSE: Stabilizing techniques for flail chest injuries are described through wide surgical approaches to the chest wall, especially in the most affected posterior and lateral regions. Severe morbidity due to these invasive approaches needs to be considered due to dissection of the scapular guiding muscles and the risk of injuries to neurovascular bundles. This study discusses possibilities for minimized approaches to the posterior and lateral regions. METHOD: Ten fresh-frozen cadavers in lateral decubitus position were observed on both sides. Each surgical arm was kept mobile during the pro

Dual Bone Fixation: A Biomechanical Comparison of 3 Implant Constructs in a Mid-Diaphyseal Fracture Model of the Feline Radius and Ulna.

OBJECTIVE: To compare the biomechanical properties of dual bone fixation (DBF) constructs to radial locking compression plating (LCP) in an ex vivo feline antebrachial fracture gap model. STUDY DESIGN: Ex vivo study. SAMPLE POPULATION: Cadaveric feline antebrachii (n=12 pairs). METHODS: Antebrachii were radiographed to confirm normal skeletal appearance and maturity. After creation of a 5 mm radial and ulnar ostectomy, each antebrachium received 1 of 3 constructs using an incomplete randomized block design (n=8 per group). All groups received a 10 hole 2.0 mm radial LCP. DBF groups received

[The Extended Deltoid-Split Approach for Plating Four-Part Proximal Humeral Fractures].

The deltoideopectoral approach is established as the gold standard in the surgical treatment of proximal humeral fractures. As an alternative, we demonstrate the extended deltoid approach with an intraoperative video. A direct lateral incision is performed and the anterior parts of the axillary nerve are identified and preserved. In our experience, this approach allows improved visualisation of the greater tuberosity and easier positioning of locking plates. Clinically relevant neurological injuries cannot be seen in our patients or in the literature.

Surgical anatomy of medial open-wedge high tibial osteotomy: crucial steps and pitfalls.

PURPOSE: To give an overview of the basic knowledge of the functional surgical anatomy of the proximal lower leg and the popliteal region relevant to medial high tibial osteotomy (HTO) as key anatomical structures in spatial relation to the popliteal region and the proximal tibiofibular joint are usually not directly visible and thus escape a direct inspection. METHODS: The surgical anatomy of the human proximal lower leg and its relevance for HTO are illustrated with a special emphasis on the individual steps of the operation involving creation of the osteotomy planes and plate fixation. Dislocated Posterior Tibial Tendon Treated With Plate Buttress Fixation in a Collegiate Gymnast: A Case Report and Review of the Literature.

UNLABELLED: Dislocation of the posterior tibial tendon (PTT) is a rare pathological process that occurs most often as a result of acute trauma. The injury involves forced dorsiflexion and eversion of the foot against a contracted posterior tibialis. Diagnosis of the injury is often difficult secondary to the rarity of the injury and its similarity with other benign injuries of the medial ankle. Routine diagnostic imaging often does not reveal the injury, and advanced imaging with magnetic resonance imaging or ultrasound to confirm the diagnosis is often required. The injury can be a result of

Spontaneous sternal fracture due to multiple myeloma requiring extensive surgical repair.

Spontaneous sternal fracture is a well-known complication of multiple myeloma due to osteolytic bone lesions. The possibility of a multiple myeloma should be thoroughly investigated in patients presenting with a spontaneous sternal fracture. This work up should go beyond protein electrophoresis alone as a monoclonal paraprotein is not always present. In some cases, the myeloma plasma cell clone produces only the free light chain (κ or λ) or may even be non-secretory. The underlying plasma cell dyscrasia is treated with chemotherapy and, if needed, local radiotherapy. However, for patients with

Post-traumatic osteonecrosis of the proximal humerus.

Post-traumatic osteonecrosis of the proximal humerus represents a challenging problem to the surgeon. It is commonly seen following multi-fragmentary fractures of the proximal humerus which may affect the long-term functional recovery after such injuries. This review summarises the current evidence on risk factors, reasons why estimating its epidemiology is difficult, the vascular supply of the humeral head, classification, and management options.

Mathematical modelling for the comparison of plate and intramedullary osteosynthesis stability in intraarticular distal radius fractures.

BACKGROUND: A comparison between plate and intramedullary osteosynthesis techniques in terms of the biomechanical stability of distal radius fracture fixation. METHODS: Mathematical modelling was used to simulate distal radius fractures, type C1 and type C2, and to compare the stability of osteosynthesis in both techniques under several wrist joint load conditions: axial compressive loading of the radius, bending loads in volar and dorsal flexion and radial and ulnar deviation, and axial torsional loading of the forearm. RESULTS: For both type C1 and type C2 fractures, the stability of intra

Short-term complications associated with TPLO in dogs using 2.0 and 2.7 mm plates.

The purpose of this study was to evaluate short-term complications associated with the tibial plateau leveling osteotomy (TPLO) procedure in dogs of small and medium sizes. These study findings could then be compared with similar studies in dogs of all sizes. Ninety-eight procedures (82 dogs) using 2 or 2.7 mm TPLO plates were included spanning a 6 yr period with a minimum of an 8 wk follow-up period. The overall complication rate was 36%. Complications were grouped into three categories: intraoperative (2%), those occurring up to 2 wk postoperatively (11%), and those occurring >2 wk postopera

Rotational osteotomy with submuscular plating in skeletally immature patients with cerebral palsy.

BACKGROUND: In cerebral palsy, intoeing gait with increased femoral anteversion is not uncommon and often requires surgical intervention. Although several conventional methods have been used, complications are common. We applied a new technique of rotational osteotomy with submuscular plating in skeletally immature patients with cerebral palsy. METHODS: Eighteen patients (26 femora, 8 bilateral) with a mean age of 8.7 years (range, 6-16) were prospectively treated with this technique. The anatomic distribution of patients was hemiplegia (n = 7), diplegia (n = 8), and asymmetric diplegia (n =

Avoiding pitfalls in craniofacial reconstruction using absorbable plates.

Rigid internal fixation devices are a mainstay in craniofacial surgery. The advent of absorbable plates has been a monumental advancement and has proven to be a safe and effective method of osteofixation. It has been proposed that many problems of the permanent metallic implants can be avoided by the use of resorbable plates. Exposure is thought to be secondary to tissue tension over the plates and breakdown of the overlying tissue envelope. This a report of 2 cases of craniosynostosis reconstruction with absorbable plate exposure through overlying skin incisions necessitating plate removal an

Use of orthogonal or parallel plating techniques to treat distal humerus fractures.

Distal humerus fractures continue to be a complex fracture to treat. This article describes two surgical techniques that can be used to tackle these difficult fractures: Parallel plating and orthogonal plating. Both techniques have yielded excellent outcomes after open reduction and internal fixation; yet each has its own set of unique considerations. However, the key to successful treatment of these difficult fractures regardless of technique remains obtaining anatomic reduction with stable fixation and the implementation of early motion.
