[Emerging from depression and panic disorder into a normal life: a case report].

This study documents the nursing experience of caring for a patient with depression and panic disorder, using cognitive therapy. The subject had experienced chronic depression and repeated panic attacks for an extended period of time, which had a negative impact on both her and her family. In addition to applying relaxation techniques to alleviate discomfort resulting from repeated hyperventilation, the author also noted that negative emotion was the common cause of the subject's depression and anxiety. Such patients habitually experience negative cognition, which influences their emotions and

Guided and unguided CBT for social anxiety disorder and/or panic disorder via the Internet and a smartphone application: study protocol for a randomised controlled trial.

BACKGROUND: Smartphone technology presents a novel and promising opportunity to extend the reach of psychotherapeutic interventions by moving selected parts of the therapy into the real-life situations causing distress. This randomised controlled trial will investigate the effects of a transdiagnostic, Internet-administered cognitive behavioural (iCBT) self-help program for anxiety, supplemented with a smartphone application. The effect of added therapist support will also be studied. METHODS/DESIGN: One hundred and fifty participants meeting diagnostic criteria for social anxiety disorder an

Trajectories of symptom reduction and engagement during treatment for childhood behavior disorders: differences across settings.

In this study, we examined trajectories of symptom reduction and family engagement during the modular treatment phase of a clinical trial for early-onset disruptive behavior disorders that was applied either in community settings or a clinic. Participants (N=139) were 6-11 year-old children with diagnoses of Oppositional Defiant Disorder (ODD) or Conduct Disorder (CD). Symptoms of ODD/CD and level of engagement were assessed at every session during the course of treatment. Overall, symptom reduction was characterized by a gradual decline in symptoms over the first 11 sessions followed by a fla

3-year follow-up of the NIMH MTA study.

OBJECTIVE: In the intent-to-treat analysis of the Multimodal Treatment Study of Children With ADHD (MTA), the effects of medication management (MedMgt), behavior therapy (Beh), their combination (Comb), and usual community care (CC) differed at 14 and 24 months due to superiority of treatments that used the MTA medication algorithm (Comb+MedMgt) over those that did not (Beh+CC). This report examines 36-month outcomes, 2 years after treatment by the study ended. METHOD: For primary outcome measures (attention-deficit/hyperactivity disorder [ADHD] and oppositional defiant disorder [ODD] symptom

Glutamatergic augmentation strategies in obsessive-compulsive disorder.

Proven treatment strategies for obsessive-compulsive disorder (OCD) include pharmacotherapy with serotonin reuptake inhibitors and cognitive behavior therapy (CBT). A significant proportion of patients (25%-30%) fail to respond to these treatment options, necessitating the need for additional treatment options to improve treatment outcomes and quality of life in patients with OCD. Augmentation strategies using various glutamatergic agents have been explored, with diverse outcomes. The aim of this review is to give an overview of the glutamatergic system in the brain with a focus on glutamaterg

[Hysteria I. Histrionic personality disorder. A psychotherapeutic challenge].

What is left of Freud's hysteria in modern diagnostics is the histrionic personality. Psychological and somatic functional disorders, such as dissociative and somatoform disorders are freed from the label of being hysterical, but even the histrionic personality disorder does not enjoy professional agreement as far as diagnostics and therapy are concerned. This disorder is characterized by dramatization, suggestibility, superficial changing affects, impressionist cognitive style, preoccupation with outward appearance, seductive behavior and the wish to take centre stage, a compensatory attitude

Behavior in subcortical vascular dementia with sight pathologies: visual hallucinations as a consequence of precocious gait imbalance and institutionalization.

BACKGROUND: Subcortical vascular dementia (sVAD) is considered the most frequent dementia in old population, and it is due to a small vessel disease. It has a very specific nosography, where the dominant factors are dysexecutive functions, depression, and apathy. Very few studies described visual hallucinations in sVAD, apart from in the final stages of it. METHODS: This study recruited 577 patients with a diagnosis of sVAD associated with major ocular pathologies and 1118 patients with sVAD without any significant ocular pathology: Patients were followed up for 24 months. We studied the infl

Selective IGT decision-making impairment in a patient with juvenile Parkinson's disease and pathological gambling: a role for dopaminergic therapy?

The orbitofrontal cortex and the dopaminergic system are structures involved in managing impulsivity and sensibility to reinforcements, and both are typically impaired in Parkinson's disease (PD). Also, l-DOPA treatment can contribute to the development of the 'Dopamine Dysregulation Syndrome', a syndrome that can influence the patients' personality and lead to risk-taking behaviors. In this study, we describe the case of a 42-year-old woman (LT) affected by juvenile PD, treated with both l-DOPA and dopamine agonists, who showed a sudden onset of pathological gambling (PG), as the only neurops

A Narrative Review of Neurodevelopmental, Psychiatric, and Behavioral Consequences of Preterm Birth.

Preterm infants survive into adulthood. However, this survival often comes with increased health and social challenges during adolescence and adulthood, including medical disabilities, learning difficulties, and behavioral and psychological issues. Understanding the long-term outcomes of preterm birth is crucial for developing prevention and treatment strategies, as well as setting future research priorities. Research in this area can be complex due to the need for long follow-up periods and changes in the classification of diseases and outcomes over time, which make it challenging to establis

Clinical markers of HIV predict redox-regulated neural and behavioral function in the sensorimotor system.

Even in the modern era of combination antiretroviral therapy, aberrations in motor control remain a predominant symptom contributing to age-related functional dependencies (e.g., neurocognitive impairment) in people with HIV (PWH). While recent evidence implicates aberrant mitochondrial redox environments in the modulation of neural oscillatory activity serving motor control in PWH, the contribution of important clinical and demographic factors on this bioenergetic-neural-behavioral pathway is unknown. Herein, we evaluate the predictive capacity of clinical metrics pertinent to HIV (e.g., CD4

Introduction to Parkinson disease (PD) and its complications.

Parkinson disease (PD) is a common neurodegenerative disorder in older adults characterized by motor and nonmotor symptoms and complications. Impulse control disorders (ICDs), such as pathological gambling, compulsive shopping, compulsive sexual behavior (hypersexuality), and binge eating disorder, affect 13.6% of the PD population. Use of dopamine receptor agonists (DRAs) is considered a major risk factor for ICD development. Amantadine and a high dose of levodopa were linked to ICDs to a lesser extent than DRAs. Based on the severity of behavior(s), ICDs can negatively impact social, profess

Treatment of avoidant-restrictive food intake disorder in an older adult: A proof-of-concept case study.

OBJECTIVE: Cognitive behavioral therapy for Avoidant Restrictive Food Intake Disorder (ARFID; CBT-AR) is an emerging treatment for ARFID. However, this treatment modality has yet to be examined among older adults (e.g., older than 50 years) or with adults presenting with feeding tubes. To inform future versions of CBT-AR, we present the results of a singular case study (G) of an older male with the sensory sensitivity phenotype of ARFID who presented for treatment with a gastrostomy tube. METHODS: G was a 71-year-old male who completed eight sessions of CBT-AR in a doctoral training clinic. A…

Electroconvulsive therapy for pediatric malignant catatonia with cerebellar dysgenesis.

Electroconvulsive therapy was successfully used to treat malignant catatonia in a 15-year-old male patient with congenital dysgenesis of the left hemisphere of the cerebellum and hypoplasia of the vermis and left pons due to a presumed cerebral vascular accident in utero. The patient experienced significant motor and communication delays with mild cognitive impairment, but was otherwise in good health until age 15 years, when he developed rigidity, posturing, stupor, unresponsiveness, repetitive self-injurious behaviors, and negativism, as well as autonomic abnormalities including profuse diap

Progressive supranuclear palsy, multiple system atrophy and corticobasal degeneration.

Progressive supranuclear palsy (PSP), multiple system atrophy (MSA), and corticobasal degeneration (CBD) are forms of parkinsonism. PSP and CBD are 4R tauopathies and clinicopathologic overlaps exist between these two disorders. Neuropsychiatric symptoms including apathy, depression, anxiety are common features in patients with PSP and CBD. Disinhibition and impulsive behavior are also frequently observed in PSP patients, whereas hallucinations are seen only occasionally. Severe derangement in several neurotransmitter systems may account for behavioral symptoms observed in PSP and CBD, but sub

Immersive virtual reality for reducing public speaking anxiety in students accessing a university psychological counseling service: protocol for a randomized controlled trial.

BACKGROUND: Public Speaking Anxiety (PSA) is a form of social anxiety spread among university students, negatively affecting both academic performance and overall psychological well-being. Virtual Reality (VR)–based interventions combined with Cognitive Behavioral Therapy (CBT) represent a promising approach for treating PSA, as they enable gradual and controlled exposure to feared social situations by simulating realistic audiences and environments. The primary aim of this study is to evaluate whether the use of VR as a support to standard psychotherapy is effective in managing anxiety and PS…
