Severe peptic oesophagitis.

The incidence, clinical and investigative features, treatment, and course of severe oesophagitis in 200 patients seen and followed up in the Thoracic Surgical Department for north east Scotland from 1951 to 1967 are reviewed. The male/female ratio was 1/1.9. The incidence of severe oesophagitis (grades III and IV) approximated to 4.5 per 100,000; there was a dramatic increase from the age of 50 years onwards.Reflux, with or without hiatal hernia, precedes oesophagitis and has an incidence in excess of 86 per 100,000. It is difficult to assess the extent to which reflux produces mild oesophagit

Assessing Public Awareness and Understanding of Dysphagia: A Representative Survey of US Adults.

PURPOSE: Dysphagia is a commonly occurring medical condition estimated to occur in between and 10% adults in the US. Despite this relatively high prevalence, the general population's understanding of this condition is currently unknown. Our aims were to (a) conduct a large-scale survey to determine the public's awareness and understanding of dysphagia in comparison to other three other health conditions and (b) compare this knowledge to relative prevalence rates of the conditions. METHODS: The survey was designed to measure four constructs of interest comparing dysphagia with insomnia, vertig

Upper Gastrointestinal Functional and Motility Disorders in Children.

Children with disorders affecting the sensory and motor functions of the esophagus will present primarily with swallowing dysfunction, dysphagia, and chest pain, and those with disorders affecting the normal function of the stomach will present with symptoms like abdominal pain, nausea, and vomiting. Recent advances in the mechanisms of disease and technology have increased our understanding of gastrointestinal physiology and that knowledge has been applied to develop new diagnostic studies and therapeutic interventions. We present an overview of the clinical presentation, diagnosis, and treat

Diaphragmatic CMAP amplitude from phrenic nerve stimulation predicts functional decline in ALS.

OBJECTIVE: To evaluate phrenic nerve motor amplitude (PhrenicAmp) as an independent predictor of functional decline in amyotrophic lateral sclerosis (ALS). We also assessed both PhrenicAmp and forced vital capacity (FVC) as predictors of functional loss in patients with bulbar dysfunction. METHODS: We included consecutive ALS patients with PhrenicAmp and FVC at baseline. Participants were evaluated with the revised ALS Functional Rating Scale (ALSFRS-R) at inclusion and at, at least, one subsequent follow-up visit. The outcome measure of functional decline was the percentage reduction in ALSF

Roles of High-resolution Manometry in Predicting Incomplete Bolus Transit in Patients With Dysphagia.

BACKGROUND: High-resolution manometry (HRM) is used to assess esophageal motility diseases. Abnormalities in a number of HRM parameters have been reported in patients with dysphagia. However, it is unclear whether some of abnormal HRM parameters are predictive of dysphagia. The aim of this retrospective study was to investigate the roles of HRM parameters in predicting incomplete bolus clearance (IBC) in patients with dysphagia using high-resolution impedance manometry. METHODS: A total of 644 wet swallows were reviewed and analyzed in 63 patients with symptoms of dysphagia or reflux who unde

A comprehensive intervention following the clinical pathway of eating and swallowing disorder in the elderly with dementia: historically controlled study.

BACKGROUND: Eating problems in patients with advanced dementia are strongly associated with their deteriorating survival. Food and drink intake in people with dementia may be supported by specific interventions, but the effectiveness of such interventions is backed by almost no evidence. However, comprehensive geriatric assessment (CGA) might potentially clarify the etiology of decreased oral intake in people with dementia; thus improving their clinical outcomes. METHODS: This study was a single-arm, non-randomized trial that included historically controlled patients for comparison. We define

Motor neurone disease - caring for the patient in general practice.

BACKGROUND: Motor neurone disease is a neurodegenerative disease that leads to progressive disability - and eventually death - within 2-3 years. OBJECTIVE: This article describes the role of the general practitioner in caring for patients with motor neurone disease. DISCUSSION: The diagnosis of motor neurone disease relies on the presence of upper and lower motor neurone features. There is currently no pathognomic test for motor neurone disease and it largely remains a diagnosis of exclusion following an accurate clinical history, combined with basic screening blood investigations and struct

Novel TOR1A mutation p.Arg288Gln in early-onset dystonia (DYT1).

BACKGROUND: The three-nucleotide deletion, triangle upGAG (within the gene TOR1A), is the only proven cause of childhood-onset dystonia (DYT1). A potentially pathogenic role of additional sequence changes within TOR1A has not been conclusively shown. METHODS: DNA sequencing of exon 5 of TOR1A in a patient with DYT1. RESULTS: Detection of sequence change c.863G>A in exon 5 of TOR1A in the patient. The G>A transition results in an exchange of an arginine for glutamine (p.Arg288Gln) in subdomain alpha5 of TOR1A. Several findings point to a potentially pathogenic role of the sequence change in t

Feeding problems and malnutrition in spinal muscular atrophy type II.

The aim of the study was to conduct a survey using a dedicated questionnaire to assess feeding difficulties and weight gain in a population of 122 Spinal Muscular Atrophy (SMA) type II patients, aged between 1 and 47 years. All the answers were entered in a database and were analysed subdividing the cohort into age groups (1-5, 6-10, 11-14, 15-19, 20-29, and 30-50 years). Six out of our 122 patients (5%), all younger than 11 years, had weights more than 2SD above the median for age matched controls, whilst 45 (37%) had weights less than 2SD below the median. Chewing difficulties were reported
