Pendred syndrome.

Pendred Syndrome, first described in 1896, is phenotypically characterized as the coexistence of sensorineural deafness and enlarged goiter with elevated iodine discharge after perchlorate administration. In 1996 the syndrome was mapped to chromosome 7 and the following year, the responsible gene was cloned and mutations were identified. The gene, pds, codes for a 780 amino acid protein, pendrin, which functions as an ion transporter. Located on the apical membrane of thyrocytes, it appears to be responsible for the transport of iodide out of the cell into the colloid where iodination of thyro

Normal and defective neuronal membranes: structure and function: neuronal lesions in peroxisomal disorders.

Neuronal involvement in the peroxisomal disorders is divided into two main groups: developmental and postdevelopmental or degenerative. In the former the major lesions are neuronal migration abnormalities, which vary from severe in the cerebro-hepato-renal (Zellweger) syndrome (ZS) to mild in neonatal adrenoleukodystrophy. More common, but much less severe, are defects in neuronal differentiation or terminal migration, particularly involving the inferior medullary olives. Ultrastructural and neurochemical observations in ZS suggest that the presence of abnormal cytosomes in migrating neurons a

[Dangers of blank firearms--risk of injury and expert assessment].

Starters' pistols have a wounding potential which strongly depends on the shot range and the combination of weapon and ammunition. The detrimental effect consists of five aspects: gas pressure, particles, thermal effect, chemical irritation and shooting noise. Fatal injuries are caused by contact shots in the head, neck or chest. Shots from less than 5 cm distance can lead to loss of the eye. Particles work as small projectiles at distances of less than 50 cm and cause powder tattooing, concerning the eye however severe cornea injuries are possible. The thermal effect can be observed with shoo

Cochlear implant soft surgery: fact or fantasy?

A basic surgical principle is to be as gentle as possible to accomplish the goals of the operation. The concept of "soft surgery" for cochlear implants consisted of a small, localized cochleostomy and gentle electrode insertion, the hope being that by limiting damage to the inner ear, superior hearing results might be obtained. The technique includes deferring the cochleostomy until immediately before electrode insertion, use of a large burr to flatten the promontory, followed by a smaller burr to expose the endosteum, preservation of the endosteum of the scala tympani, smoothing of the bony e

Monitoring cochlear function intraoperatively using distortion product otoacoustic emissions.

Distortion product otoacoustic emissions (DPOAEs) elicited by a bitonal stimulus complex are low-level sounds that are generated within the cochlea, and are easily measured by a miniature microphone system placed in the external auditory canal. Under conditions of rapid measurement, DPOAEs have been shown in an animal model to be exquisitely sensitive to interruption of the blood supply to the inner ear. Currently, there is no real-time procedure available for monitoring hearing function intraoperatively during neurotologic surgery. The goal of the present study was to determine the utility of

Mechanisms of hearing loss in acoustic neuroma: an otoacoustic emission study.

Evoked otoacoustic emissions (EOAE) are active mechanical responses from the cochlea which provide information about the integrity of the preneural cochlear receptor mechanisms. It may be hypothesised, therefore, that if a hearing impairment is neural in origin, normal EOAEs may be obtained from the cochlea, which, although dissociated, is functioning normally. This study examined the status of the cochlea with EOAE in patients with cochlear (Meniere's disease) and neural (surgically proven acoustic neuroma) disease. In patients with presumed cochlear lesions, no emissions were present with me

Motion sickness: a synthesis and evaluation of the sensory conflict theory.

"Motion sickness" is the general term describing a group of common nausea syndromes originally attributed to motion-induced cerebral ischemia, stimulation of abdominal organ afferents, or overstimulation of the vestibular organs of the inner ear. Seasickness, car sickness, and airsickness are commonly experienced examples. However, the identification of other variants such as spectacle sickness and flight simulator sickness in which the physical motion of the head and body is normal or even absent has led to a succession of "sensory conflict" theories that offer a more comprehensive etiologic

Intractable headache after excision of an acoustic neuroma treated by stent revascularisation of the sigmoid sinus.

A 47 year old man developed severe headaches after resection of an acoustic neuroma ipsilateral to non dominant venous drainage. CSF pressures were normal but imaging studies showed acquired, severe narrowing of the sigmoid sinus where it traversed the surgical defect. Stenting the sinus gave a lasting clinical improvement.

Stem cell and peripheral nerve injury and repair.

Peripheral motor nerve injuries are a significant source of morbidity. Neural stem cells (NSCs), a group of relatively primitive cells, possess self-renewal ability and multidifferentiation potential. NSCs may be successfully separated from the human embryo and central nervous system (CNS) and differentiated into mature neurons and gliacytes by in vitro induction or transplantation into the body and may be differentiated into Schwann-like cells under specific conditions. It has been demonstrated that the ability of peripheral nerves to regenerate is mainly attributable to Schwann cells. NSC tr
