Rectoprostatic fistulas following surgery pose difficult problems for surgical reconstruction. A variety of surgical techniques has been proposed including a perineal trans-sphincteric approach and a York-Mason procedure. We repaired three patients by this approach; we found the intervention simple, safe and effective; preliminary colostomy is not necessary.

A personal case of enterocutaneous fistula secondary to total emergency colectomy is reported with particular emphasis on an effective form of radiological management, based on the percutaneous sclerosing of the fistulous tract using absolute ethanol and a hyperosmotic contrast medium (Angioconray 80%). By this approach a rapid closure of the fistulous tract was obtained. Moreover the procedure was easy to perform, well tolerated by the patient and devoid of untoward side effects.

Portal vein thrombosis and pancreatico-pleural fistula are unusual complications of chronic pancreatitis. We describe a patient with chronic alcoholic pancreatitis in whom erosion of the splenic vein led to portal vein thrombosis and to the development of a pancreatico-pleural fistula. We suggest that fistula formation may occur over a considerable time period as the portal vein thrombosis was diagnosed three years before the amylase-rich pleural effusions.

At autopsy, multiple gallstones were recovered from the right pleural space of an elderly patient who presented with a massive right pleural effusion and septic shock. The mechanisms of gallstone migration and fistula formation between the gallbladder and right pleural space are described. Despite atypical presentations, gallbladder disease remains an important differential consideration of right pleural effusion in the elderly.

A twenty year old man, operated successfully when 7 years old for congenital right coronary artery to right ventricule fistula, requiring a fitness certificate for competitive sport (karatè), has been subjected to test of cardiovascular and respiratory function: a resting 12 lead ECG. stress testing, Holter monitoring, expiratory flow-volume curve. The good results allowed him to practice competitive sport, but only under very frequent medical controls.

Enterovesical fistulas affect mainly male patients, due to the interposition of the uterus in women. They can be caused by neoplastic, inflammatory or traumatic processes. A case of ilovesical fistula due to migration of a silastic plate used to repair an epigastric infissional hernia is presented. This case has two distinctive features: first, the cause is extraurological, causing a fistula between both systems due to an inflammatory process. Secondly, we report the long distance travelled by the synthetic mesh, causing a peculiar clinical picture five years after its implantation.

A 3-year prospective study of 244 children (aged 5 months to 17 years) with sensorineural hearing loss of unknown cause revealed that 57 children (23%) had radiographic evidence of abnormalities of the temporal bone, detected by computed tomography scan, and/or progression of sensorineural hearing loss. A man aged 33 years developed an arterio-venous fistula following a gunshot wound from behind in the right calf. The quality of the fistula could be demonstrated employing digital subtraction angiographic technique while the quantity was demonstrated employing 99Tc macroaggregated albumin. The localization of the fistula is important in view of the surgical procedure and the haemodynamic amount is important to determine cardiac involvement.

In a 43-year-old woman, a large, contaminated fistula, initially with vesicointestinal communication, was successfully closed by application of fibrin seal. The course of three sealing attempts indicated that local pretreatment with antibiotics may have been important for the final result. At 4-year follow-up examination there was no recurrence of the fistula.

A 51-year-old woman underwent orthotopic liver transplantation because of a small-duct primary sclerosing cholangitis associated with chronic ulcerative colitis and a hepatoportal arteriovenous fistula.
