Atrial septal defect in Malta.

OBJECTIVE: To study diagnostic and surgical trends in atrial septal defect (ASD) in a population-based study, and estimate birth prevalence and spontaneous closure rates. METHODOLOGY: All patients in Malta diagnosed as having ASD and born between 1990 and 1994 were identified from various sources. This took place in the setting of a regional hospital supplying diagnostic services for the entire population. Echocardiographic follow-up was also undertaken for lesions not requiring intervention. RESULTS: A total of 190 patients born in this period were diagnosed as having ASD. Age at diagnosis

Waterston anastomosis. Comparison of results of operation berfore and after age 6 months.

The results of 40 Waterston aortico-to-pulmonary anastomoses are reviewed from the experience at The Hospital for Sick Children, Toronto, between June 1966 and October 1969. Over the age of 6 months there was one death associated with the procedure in 12 cases. Under the age of 6 months the mortality associated with the operation in cases of tetralogy of Fallot was 50%. This represents the severer degree of defect as well as the increased technical difficulties in the younger age group.

Role of Doppler echocardiography for assessing right ventricular cardiac output in patients with atrial septal defect.

BACKGROUND: Although Doppler echocardiography is routinely used to assess left ventricle cardiac output, there are limited data about the feasibility of Doppler echocardiography for right ventricular (RV) cardiac output assessment in patients with left-to-right shunt. The purpose of the study was to determine the correlation between Doppler-derived and Fick-derived RV cardiac index (CI), and the interobserver correlation in Doppler-derived RV CI assessment. METHODS: Retrospective study of patients (age ≥18 years) with unrepaired atrial septal defect who underwent cardiac catheterization and e

The role of computed tomography angiography in patients undergoing evaluation for minimally invasive cardiac surgery: an early program experience.

OBJECTIVE: An increasing number of patients are undergoing surgical procedures using minimally invasive cardiac surgery (MICS). These techniques use conventional or retrograde arterial perfusion with direct aortic cross-clamping or endoballoon occlusion. Precise knowledge of the arterial tree is required to avoid complications and to plan for the operation. We examined the role of computed tomography angiography (CTA) in evaluating patients for MICS. METHODS: We reviewed all consecutive candidates undergoing CTA during preoperative evaluation for MICS aortic, mitral, tricuspid, Maze, atrial s

Incomplete RV Remodeling After Transcatheter ASD Closure in Pediatric Age.

Published data showing the intermediate effect of transcatheter device closure of atrial septal defect (ASD) in the pediatric age-group are scarce. The objective of the study was to assess the effects of transcatheter ASD closure on right and left ventricular functions by tissue Doppler imaging (TDI). The study included 37 consecutive patients diagnosed as ASD secundum by transthoracic echocardiography and TEE and referred for transcatheter closure at Cairo University Specialized Pediatric Hospital, Egypt, from October 2010 to July 2013. Thirty-seven age- and sex-matched controls were selected

Electroanatomic relationships in patients with primum atrioventricular septal defect.

OBJECTIVES: The objective of this study was to test the hypothesis that patients with primum atrioventricular septal defect (AVSD) have an imbalance in the positions of the left ventricular papillary muscles compared with healthy subjects, and that this anatomic imbalance correlates with left deviation of the QRS axis. BACKGROUND: The function and contraction pattern of the heart is best predicted when cardiac anatomy is considered together with its electrical activation sequence. Understanding the electroanatomic relationships is essential for discovering the developmental relationships betw

Percutaneous rescue of left ventricular embolized amplatzer septal occluder device.

Transcatheter atrial septal defect (ASD) closure has proven to be safe and effective, and it is rapidly becoming the standard treatment for secundum-type ASDs. However, device embolization occurs in about 0.55% of cases, regardless of ASD and device size and/or physician's expertise. Percutaneous retrieval of the embolized device is possible in about 50% of cases, and several techniques have been described, including the use of large sheaths, gooseneck snares, or bioptomes. This is the first successful and reported case of retrieval of an embolized amplatzer septal occluder (ASO) from the left

An aneurysmal left circumflex artery-to-right atrium fistula in a patient with ischemic symptoms: accurate diagnosis with dual-source CT angiography.

In this report, we present a 55-year-old female patient with a left circumflex artery-to-right atrial fistula associated with a huge saccular aneurysm. She had undergone conventional angiography due to ischemic symptoms. In conventional angiography, a very dilated and tortuous vessel originating from the circumflex artery and continuous with a huge saccular aneurysm was visualized but the drainage site could not be demonstrated. With dual-source CT coronary angiography, the exact anatomy of this fistula was demonstrated and surgery was planned.

Unusual right coronary artery anomaly with major implication during cardiac operations.

We report a case in which an unrevealed high origin of the right coronary artery (almost 4 cm above the left coronary sinus) led to major complications during a routine atrial septal defect closure. We stress that, in absence of preoperative coronary angiography, coronary artery anomalies should always be considered and minimal ascending aorta manipulation and antegrade/ retrograde myocardial protection should be recommended.

Quantification of left to right atrial shunts with velocity-encoded cine nuclear magnetic resonance imaging.

OBJECTIVES: The purpose of this study was to evaluate the ability of velocity-encoded nuclear magnetic resonance (NMR) imaging to quantify left to right intracardiac shunts in patients with an atrial septal defect. BACKGROUND: Quantification of intracardiac shunts is clinically important in planning therapy. METHODS: Velocity-encoded NMR imaging was used to quantify stroke flow in the aorta and in the main pulmonary artery in a group of patients who were known to have an increased pulmonary to systemic flow ratio (Qp/Qs). The velocity-encoded NMR flow data were used to calculate Qp/Qs, and t

[Mitral valve prolapse in patients with post-operative atrial septal defect: a preliminary report].

To investigate the clinical significance of mitral valve prolapse (MVP) in patients with atrial septal defect, we investigated 90 patients using various techniques including phonocardiography, mechanocardiography and echocardiography. The examinations were repeated pre- and post-operative periods, and the particular attention was paid on the behavior of the mitral valve motion after surgery. Post-operatively, the incidence of MVP was 56% (50 of 90 patients) compared with the preoperative incidence of 78%. Improvement of MVP was only 24%, and there was no case in which MVP newly developed after

Children Born with Congenital Heart Defects and Growth Restriction at Birth: A Systematic Review and Meta-Analysis.

UNLABELLED: Newborns with congenital heart defects tend to have a higher risk of growth restriction, which can be an independent risk factor for adverse outcomes. To date, a systematic review of the relation between congenital heart defects (CHD) and growth restriction at birth, most commonly estimated by its imperfect proxy small for gestational age (SGA), has not been conducted. OBJECTIVE: To conduct a systematic review and meta-analysis to estimate the proportion of children born with CHD that are small for gestational age (SGA). METHODS: The search was carried out from inception until 31
