Post-Transplant Thrombotic Microangiopathy due to a Pathogenic Mutation in Complement Factor I in a Patient With Membranous Nephropathy: Case Report and Review of Literature.

Thrombotic microangiopathy (TMA) is characterized by microangiopathic hemolytic anemia, thrombocytopenia and organ injury occurring due to endothelial cell damage and microthrombi formation in small vessels. TMA is primary when a genetic or acquired defect is identified, as in atypical hemolytic uremic syndrome (aHUS) or secondary when occurring in the context of another disease process such as infection, autoimmune disease, malignancy or drugs. Differentiating between a primary complement-mediated process and one triggered by secondary factors is critical to initiate timely treatment but can

Extranodal natural killer/T-cell lymphoma of the breast: a retrospective clinicopathological analysis of a consecutive 11-year case series.

BACKGROUND: Extranodal NK/T-cell lymphoma of the breast (ENKTL-Breast) is rarely detected in clinical practice, and its clinicopathological features remain unclear. RESULTS: A consecutive 11-year (2010-2020) ENKTL-Breast case series was retrospectively analyzed. Eight cases of ENKTL-Breast were selected, with three primary and five secondary lesions, accounting for 3.5% (8/228) of all breast lymphomas. All patients were female with a median age of 46 years. Lesions presented as solid breast masses (8/8, 100%) and were usually located in the upper outer quadrant of the breast (7/8, 87.5%). B-s

Organizing pneumonia and COVID-19: A report of two cases.

Organizing pneumonia (OP) is a sub-acute process of pulmonary tissue repair secondary to lung injury, defined histopathologically by intra-alveolar buds of granulation tissue within the lumen of distal pulmonary airspaces. It can be either cryptogenic or secondary (SOP) to different clinical conditions, namely infections. Despite being nonspecific, its diagnosis can be made by the association of clinical and imaging criteria. We report two cases of OP associated to SARS-CoV-2 pneumonia, admitted at a Portuguese tertiary hospital unit dedicated to COVID-19. Both patients presented with severe r

Second Tier Testing to Reduce the Number of Non-actionable Secondary Findings and False-Positive Referrals in Newborn Screening for Severe Combined Immunodeficiency.

PURPOSE: Newborn screening (NBS) for severe combined immunodeficiency (SCID) is based on the detection of T-cell receptor excision circles (TRECs). TRECs are a sensitive biomarker for T-cell lymphopenia, but not specific for SCID. This creates a palette of secondary findings associated with low T-cells that require follow-up and treatment or are non-actionable. The high rate of (non-actionable) secondary findings and false-positive referrals raises questions about the harm-benefit-ratio of SCID screening, as referrals are associated with high emotional impact and anxiety for parents. METHODS:

Mendelian susceptibility to mycobacterial disease in tuberculosis-hyperendemic South Africa.

BACKGROUND: Severe infections in the absence of secondary immunodeficiency can alert clinicians to single-gene inborn errors of immunity/primary immunodeficiency disorders (PIDDs). Mendelian susceptibility to mycobacterial disease (MSMD) is characterised by selective susceptibility to mycobacterial infections due to inborn errors in the interleukin 12-interferon gamma pathway. The South African (SA) burden of hyperendemic tuberculosis (TB) infection provides an interesting context for the study of MSMD. OBJECTIVES: To evaluate whether severe, persistent, unusual or recurrent (SPUR) definition

Secondary resistance to immunotherapy associated with β-catenin pathway activation or PTEN loss in metastatic melanoma.

BACKGROUND: While cancer immunotherapies including checkpoint blockade antibodies, adoptive T cell therapy, and even some vaccines have given rise to major clinical responses with durability in many cases, a subset of patients who initially respond subsequently develop secondary resistance to therapy. Tumor-intrinsic mechanisms of acquired immunotherapy resistance are incompletely understood. METHODS: Baseline and treatment-resistant tumors underwent molecular analysis via transcriptional profiling or genomic sequencing for oncogenic alterations and histologic analysis for T cell infiltration

Clinical evaluation and antibody responses in sheep after primary and secondary experimental challenges with the mange mite Sarcoptes scabiei var. ovis.

In this work the clinical evolution and the specific serum IgG and IgE antibody responses in sheep after primary (n=10) and secondary (n=4) experimental challenges with the mange mite Sarcoptes scabiei var. ovis were studied. The primary infection was characterized by the development of mange lesions in all sheep, a detection of live S. scabiei mites in 70% skin scrapings taken in week 10 post-challenge (PC), strongly raised and sustained specific IgG levels and a more moderate but continuous rise in specific IgE levels. Seroconversion was detected for IgG and IgE by ELISA in 90% and 60% of th

[Prevention of parasitic infections (excluding toxoplasmosis) in immunocompromised patients].

Immunocompromised patients, notably those with cell mediated immunity deficiency, are at risk for severe and life-threatening parasitic infections. Severity and frequency of Pneumocystis carinii pneumonia have led to systematically initiate prophylaxis for high-risk patients such as patients with HIV infection, hemopathy or renal transplants. Cotrimoxazole has shown the best efficacy both in primary and secondary prophylaxis. Side effects, notably skin rash, constitute the major limiting factor of cotrimoxazole therapy. However, its efficacy in preventing cerebral toxoplasmosis and to a lesser

Management of cancer-associated thrombosis in patients with gastrointestinal cancers: lights and shadows.

Cancer-associated thrombosis (CAT) is a major cause of morbidity and mortality in patients with gastrointestinal (GI) malignancies and often negatively impact anti-cancer treatment delivery. A prothrombotic risk markedly varies across primary sites, from pancreatic, gastric to colorectal cancers and is influenced by tumor-driven inflammation, disease stage, and exposure to contemporary systemic therapies, including newly, immune checkpoint inhibitors, targeted agents, and antibody-drug conjugates. Despite its clinical relevance, current risk assessment models, including the pivotal Khorana sco

Lymph Node Cytology Raising Suspicion for Evolving Secondary Hemophagocytic Lymphohistiocytosis in a Case of Mendelian Susceptibility to Mycobacterial Disease.

Mendelian susceptibility to mycobacterial disease (MSMD) is a rare type of inborn error of immunity (IEI) characterized by increased vulnerability to mycobacterial infection. Evolvement of secondary hemophagocytic lymphohistiocytosis (HLH), a life-threatening condition where hemophagocytosis is seen either in the bone marrow, the spleen or lymph node has been recently recognized in this rare disease. Herein we illustrate the role of lymph node cytology in alerting clinicians to the possibility of evolving HLH in a case of MSMD. A 4-month old male infant presented with genetically confirmed MSM

Exploration of membrane-bound ecto-phosphatase to identify potential therapeutic target for leishmaniasis.

Leishmaniasis, caused by leishmanial parasites, is a major health concern worldwide that emphasizes on the exploration of novel therapeutic targets. Membrane-bound ecto-phosphatase of Leishmania donovani (LdMAcP) is essential for parasite virulence and absence of its ortholog in human makes it an attractive drug target. Hence, LdMAcP was cloned and purified to homogeneity that exhibited optimal catalytic efficiency at acidic pH. Analysis of secondary structure established the presence of appropriate secondary structural content with more α-helices, whereas quenching studies delineated the tryp

The macrophage migration inhibitory factor/CD74 axis in traumatic spinal cord injury: lessons learned from animal and human studies.

Traumatic spinal cord injury (SCI) is a severe condition leading to long-term impairment of motor, sensory, and autonomic functions. Following the initial injury, a series of additional events is initiated further damaging the spinal cord. During this secondary injury phase, both an inflammatory and immune modulatory response are triggered that have damaging and anti-inflammatory properties, respectively. The proinflammatory cytokine macrophage migration inhibitory factor (MIF) and its receptor CD74 have been extensively studied in traumatic SCI. MIF expression is increased in spinal cord tiss

Influence of Preoperative Inflammatory Status on Outcomes of Alveolar Bone Grafting in Patients With Cleft Lip.

After secondary alveolar bone grafting, the outcome of bone formation is always a problem that leads to repeat surgery. Bone formation is closely related to the inflammatory and immune status of patients. To achieve better bone formation results, this study aimed to investigate the influence of preoperative inflammatory indicators on the bone survival ratio (BSR) of secondary alveolar bone grafting. A retrospective study was conducted on 62 patients who underwent surgery by the same surgeon between January 2016 to December 2022. Demographic and laboratory data were included as independent vari

Dietary Acid Load Correlates with Serum Amino Acid Concentrations after a Four-Week Intervention with Vegan vs. Meat-Rich Diets: A Secondary Data Analysis.

Chronic low-grade metabolic acidosis is now a common phenomenon in the Western world. The high dietary intake of sulfur-containing amino acids in the form of processed meats results in an excessive release of acid in the form of protons and non-metabolizable acidic anions. The kidneys produce increasing amounts of ammonia to excrete this acid. This process requires the breakdown of the nitrogenous amino acid glutamine, which the body provides by breaking down muscle tissue. Hitherto not examined, we hypothesized that a high dietary acid load (DAL) could alter the serum concentrations of select

Characterizing microglial gene expression in a model of secondary progressive multiple sclerosis.

Multiple sclerosis (MS) is the most common inflammatory, demyelinating and neurodegenerative disease of the central nervous system in young adults. Chronic-relapsing experimental autoimmune encephalomyelitis (crEAE) in Biozzi ABH mice is an experimental model of MS. This crEAE model is characterized by an acute phase with severe neurological disability, followed by remission of disease, relapse of neurological disease and remission that eventually results in a chronic progressive phase that mimics the secondary progressive phase (SPEAE) of MS. In both MS and SPEAE, the role of microglia is poo

Systemic lupus erythematosus and antiphospholipid syndrome after COVID-19 vaccination. A case report.

Coronavirus disease 2019 (COVID-19) vaccines have some adverse effects, mostly mild. However, by presenting an immunological challenge to the individual, they could infrequently trigger immune-mediated diseases (IMDs). We report the case of a 42-year-old woman, with no previous medical history, who received the first dose of vaccine against COVID-19 and developed inflammatory arthralgias, associated with sudden-onset dyspnoea and hypoxemia. Pulmonary thromboembolism was documented, and the diagnosis of systemic lupus erythematosus (SLE) and secondary antiphospholipid syndrome (APS) was suspect

3D Tissue Explant and Single-Cell Suspension Organoid Culture Systems for Ex Vivo Drug Testing on Human Tonsil-Derived T Follicular Helper Cells.

Research on the human immune system is often restricted to peripheral blood cells. However, these cells can be different from those found in secondary lymphoid organs. For instance, specialized T and B cells that are localized in germinal centers (GCs), which are complex anatomical structures being required for the generation of potent antibodies, are not found in peripheral blood. Most T helper cells located in GCs belong to the T follicular helper (Tfh) cell subset, which provides critical support to B cells. Bona fide human GC Tfh cells can be obtained from secondary lymphoid tissues such a

Fulminant myocarditis in adults: a narrative review.

Fulminant myocarditis (FM) is an uncommon syndrome characterized by sudden and severe hemodynamic compromise secondary to acute myocardial inflammation, often presenting as profound cardiogenic shock, life-threatening ventricular arrhythmias and/or electrical storm. FM may be refractory to conventional therapies and require mechanical circulatory support (MCS). The immune system has been recognized as playing a pivotal role in the pathophysiology of myocarditis, leading to an increased focus on immunosuppressive treatment strategies. Recent data have highlighted not only the fact that FM has s
