Transplantation of congenitally opaque corneas.

AIMS: To assess retrospectively the prognosis and complications of corneal grafting for congenital opacities. METHODS: Fifty eight eyes of infants and young children with congenital corneal opacities were studied retrospectively. Preoperative diagnoses included sclerocornea (27 eyes), Peters' anomaly (17 eyes), partial sclerocornea (12 eyes), and congenital glaucoma (two eyes). Penetrating keratoplasty was performed between 5 days and 65 months of age with a mean follow up of 40 (SD 29) months. RESULTS: The overall success (including regrafts) was 70% in eyes with sclerocornea, 83% for parti

Infectious ocular complications in orthotopic liver transplant patients.

We report the frequency and type of infectious ocular complications following orthotopic liver transplantation (OLT) and review diagnostic and therapeutic strategies. During the period September 1988 through November 1994, 684 patients underwent OLT at Mount Sinai Hospital (New York). Nine orthotopic liver transplant patients (1.3%) developed ocular infections: Candida albicans endophthalmitis (2), Aspergillus fumigatus endophthalmitis (1), cytomegalovirus retinitis (4), herpes simplex virus keratitis (1), and varicella-zoster virus panophthalmitis (1). The mean time from OLT to ocular symptom

Results of transantral orbital decompression in 428 patients with severe Graves' ophthalmopathy.

We reviewed records from 428 consecutive patients with severe Graves' ophthalmopathy to determine early and late results after transantral orbital decompression. Optic neuropathy was present in 217 (50.7%) patients. Post-operatively, 402 (89%) of 453 eyes with preoperative visual acuity worse than 20/20 improved or remained the same. Visual field scotomas improved or resolved in 245 (91%) of 269 eyes tested pre- and postoperatively. Preoperative papilledema resolved or improved in 99 (94%) of 105 eyes, and preoperative exposure keratitis improved or resolved in 178 (92%) of 195 eyes. Average p

Ocular involvement in toxic epidermal necrolysis.

Toxic epidermal necrolysis (TEN) or Lyell's syndrome is an acute life-threatening disease often triggered by an adverse drug reaction and/or infection. It is characterized by an extensive epidermal loss and severe mucosal involvement. Its pathophysiology is unresolved, but several studies support the hypothesis of an immune mediation. The authors review the ocular involvement of ten drug-induced TEN cases observed in the last four years (eight females and two males; mean age=47). An epidermal loss ranging from 24 to 95% of body surface area was associated with ocular (10/10), oral (9/10), geni

An outbreak of acute haemorrhagic conjunctivitis in Kaduna, Nigeria.

Clinical studies were carried out on two groups of patients with acute haemorrhagic conjunctivitis (AHC) during an epidemic in 1985 in Northern Nigeria. Group 1 consisted of 99 students attending a girls' boarding school, group 2 of 200 patients selected randomly from 1000 examined at the local clinic. Moderate to severe hyperaemia and papillary responses were present in the palpebral conjunctiva of all patients, and 234 (66%) had subconjunctival haemorrhages. Transient superficial punctate keratitis was noted in over 60% of patients. A transient flare suggestive of a low grade iritis was seen

Tropical disease in the immunocompromised host: melioidosis and pythiosis.

Melioidosis is an infection of humans and animals caused by a gram-negative motile bacillus, Pseudomonas pseudomallei. Forty-nine patients with melioidosis complicating diabetes mellitus, collagen vascular disorders, leukemia/lymphoma, and other hematologic malignancies are described. Twenty-nine of these patients had disseminated/septicemic infection, two developed toxic shock syndrome, and one with AIDS experienced recrudescent melioidosis. Patients with disseminated melioidosis often have a variety of defects in cellular immunity both in vitro and in vivo. In humans with recrudescent melioi

PAS-positive polymorphonuclear leucocytes in corneal ulcers.

Fifty consecutive cases of severe keratitis with clinical signs of corneal ulcers were examined histologically for stage of inflammatory reaction. Further, 14 similar cases were studied ultrastructurally. Cell counts of polymorphonuclears (PML), eosinophils, lymphocytes and plasma cells were carried out. Further the corneas were grouped according to their content of PAS-positive PML. The state of ulceration was estimated microscopically. PML at a cell count of greater than 10 cells/mean field (x 1000) were found in 32/50 cases, and were significantly more frequent in corneal areas with epithel

Epidemic keratoconjunctivitis and chronic papillary conjunctivitis in London due to adenovirus type 19.

Since July 1973 cases of keratoconjunctivitis resembling epidemic keratoconjunctivitis were observed in the External Eye Disease Clinic at Moorfields Eye Hospital; City Road, London. Adenovirus type 19 was isolated in human embryonic kidney cells from 21 patients. The majority were males between 20 and 40 years old. A small hospital outbreak involving six patients occurred. Clinical features of the disease, consisting of moderate to severe follicular conjunctivitis, major subepithelial punctate keratitis, sometimes with pseudomembrane and scarring, were closely similar to those of epidemic ker

Identification of Acantamoeba isolated from water resources located in the northwest of Iran by using the high-resolution melting analysis assay.

BACKGROUND: Free-living amoebae (FLA), such as Acanthamoeba, are protozoan parasites that take advantage of opportunities with a widespread distribution in various environmental sources, such as a wide range of water sources. The amoeba can accidentally infect individuals and cause a variety of diseases, including amoebic encephalitis and keratitis, in both immunocompetent and immune-deficient individuals. The amoeba can act as reservoirs and carriers for pathogenic microorganisms, increasing the risk of pathogenicity in humans. The objective of this research was to identify the genotypes, bes
