PURPOSE OF THE STUDY: Recurring and multidirectional instability of the knee joint is a therapeutic problem mainly due to a limited possibility of harvesting grafts for its treatment. The authors present the methodology of the solution of recurring and multidirectional instabilities of the knee joint by means of allografts. They inform about the technique of harvesting and preservation of grafts and include the necessary examinations and early results.

A consecutive series of 387, one-piece, 8-mm tibial components were implanted in 313 patients. All tibial prostheses were manufactured with 4.4 mm of polyethylene directly molded to a 3.6 mm cobalt chrome metal baseplate. The average age of the patients at surgery was 70.6 years. Accurate restoration of normal limb alignment is crucial for the long-term survivorship of total knee arthroplasty (TKA). Tibial deformity secondary to previous fracture or osteotomy requires corrective osteotomy in some patients undergoing total knee arthroplasty (TKA). This can be performed in either two stages or coincident with the arthroplasty. Literature on coincident tibial osteotomy and TKA has been published previously, but details of the complicated surgical technique are lacking for surgeons performing this procedure for the first time. This article details the preoperative planning involved and the intraoperative technique used in tibial osteotomy coincident with TKA.

Clinical and radiographic data were collected in 139 patients with 195 posterior cruciate retaining total condylar knee prostheses to evaluate the relationship of the proximal tibial resection level with long term results. Six patients with snapping in the knee were evaluated with clinical history, examination, radiographs, and other imaging. All patients were younger than 40 years and none had an arthritic process. A retrospective review of the frontal and lateral knee radiographs of 200 patients with calcium pyrophosphate dihydrate (CPPD) crystal deposition disease was performed. The osteoarthritis of knee is a common disorder but no always to be treated satisfactorily.
