A review of in-vitro fibrocartilage tissue engineered therapies with a focus on the temporomandibular joint.

The inability of fibrocartilage, specifically the temporomandibular joint (TMJ) disc, to regenerate and remodel following injury presents a unique problem for clinicians. Tissue engineering then offers a potential regenerative therapy. In vitro testing provides a valuable screening tool for potential tissue engineered solutions. The conclusions drawn for TMJ in vitro research were compared against state of the art fibrocartilage studies in the knee meniscus, and annulus fibrosus of the intervertebral disc (IVD). For TMJ disc regeneration, in vitro tissue engineered approaches, focused on cellu

Local infiltration of analgesia and sciatic nerve block provide similar pain relief after total knee arthroplasty.

BACKGROUND: Although femoral nerve block provides satisfactory analgesia after total knee arthroplasty (TKA), residual posterior knee pain may decrease patient satisfaction. We conducted a randomized controlled trial to clarify the efficacy of the sciatic nerve block (SNB) and local infiltration of analgesia with steroid (LIA) regarding postoperative analgesia after TKA, when administrated in addition to femoral nerve block (FNB). METHODS: Seventy-eight patients were randomly allocated to the two groups: concomitant administration of FNB and SNB or FNB and LIA. The outcome measures included p

Intraarticular arthrofibrosis of the knee alters patellofemoral contact biomechanics.

BACKGROUND: Arthrofibrosis in the suprapatellar pouch and anterior interval can develop after knee injury or surgery, resulting in anterior knee pain. These adhesions have not been biomechanically characterized. METHODS: The biomechanical effects of adhesions in the suprapatellar pouch and anterior interval during simulated quadriceps muscle contraction from 0 to 90° of knee flexion were assessed. Adhesions of the suprapatellar pouch and anterior interval were hypothesized to alter the patellofemoral contact biomechanics and increase the patellofemoral contact force compared to no adhesions.

Peak activation of lower limb musculature during high flexion kneeling and transitional movements.

Few studies have measured lower limb muscle activation during high knee flexion or investigated the effects of occupational safety footwear. Therefore, our understanding of injury and disease mechanisms, such as knee osteoarthritis, is limited for these high-risk postures. Peak activation was assessed in eight bilateral lower limb muscles for twelve male participants, while shod or barefoot. Transitions between standing and kneeling had peak quadriceps and tibialis anterior (TA) activations above 50% MVC. Static kneeling and simulated tasks performed when kneeling had peak TA activity above 15

Asymmetry of Inter-joint Coordination during Single Leg Jump after Anterior Cruciate Ligament Reconstruction.

Little is known about the contralateral asymmetry in inter-joint coordination after anterior cruciate ligament reconstruction (ACL-R) during multi-segmental movements. This study aimed to evaluate inter-joint coordination asymmetry between the injured (IL) and non-injured leg (NIL) in patients after ACL-R during single-leg jumping. 12 male patients having undergone ACL-R (7.3 months post-surgery) and 12 healthy males performed maximal vertical single-leg jumps with the right and left leg. The kinematics of each jump were recorded. The inter-joint coordination between the ankle, knee and hip jo

Surgical Technique of Anterolateral Approach for Tibial Plateau Fracture.

A 66-year-old woman had sustained crush injury 3 hours prior to her presentation to our hospital. The diagnosis was defined as lateral tibial plateau fracture of the right knee (Schatzker III). Supine position was set up and a pad was put under the affected hip. After sterilization of the surgical field the sterilized sheets were placed beneath the leg in order to be higher than the other side. A rolled sheet was put under the knee joint so that the knee joint was flexed around 30° to 40°. After the surgical field was draped the skin was incised. Iliotibial band was incised by blade (not by el

Periprosthetic fractures following total knee arthroplasty.

Periprosthetic fractures after total knee arthroplasty may occur in any part of the femur, tibia and patella, and the most common pattern involves the supracondylar area of the distal femur. Supracondylar periprosthetic fractures frequently occur above a well-fixed prosthesis, and risk factors include anterior femoral cortical notching and use of the rotational constrained implant. Periprosthetic tibial fractures are frequently associated with loose components and malalignment or malposition of implants. Fractures of the patella are much less common and associated with rheumatoid arthritis, us

Knee osteoarthritis, body mass index and pain: data from the Osteoarthritis Initiative.

OBJECTIVE: This study aimed to determine whether BMI increases knee pain as measured from self-reported surveys even when controlling for OA severity as measured by osteophytes and joint space narrowing visible on X-rays. METHODS: Data available through the Osteoarthritis Initiative (OAI) were analysed, which included a sample of 4769 individuals, to answer the above question regarding OA, excess weight and pain. OA severity was assessed through baseline X-rays on right knees that were scored on a composite quasi-Kellgren and Lawrence grade. Weight was assessed through BMI. Pain was assessed

Patellofemoral pain syndrome in Iranian female athletes.

Patellofemoral pain syndrome (PFPS) is the most common overuse syndrome in athletes. It is one of the causes of anterior knee pain in athletic population who come to the sports medicine clinic. Patellofemoral pain is more common among female athletes especially adolescents and young adults. Symptoms include: persistent pain behind the patella or peripatella. Pain increases on ascending and descending stairs and squatting and prolonged sitting. The aim of this study was to evaluate the prevalence of PFPS in Iranian female athletes. 418 female athletes aged 15-35 years were examined in five spor

Coronal plane knee laxity measurement: Is computer-assisted navigation useful?

INTRODUCTION: The goal of this observational study is to measure the physiological laxity of a knee, supposedly normal in the coronal plane, at 0 and 90° of flexion with a navigation system that can be used during total knee replacement. HYPOTHESIS: The physiological laxity measured by this navigation system is different from the results already published using other measurement devices. MATERIALS AND METHODS: Twenty patients consecutively operated on for an isolated anterior cruciate ligament injury were selected. Medial and lateral laxities at 0 and 90° of knee flexion were measured by the

[Mini-invasive double-incision for patellar tendon harvesting in anterior cruciate ligament reconstruction].

PURPOSE OF THE STUDY: Injury to infrapatellar branches of the medial saphenous nerve are incriminated in disorders of the anterior aspect of the knee, particularly following bone-tendon-bone reconstruction. We demonstrated in prior anatomic work the usefulness of using a double-incision minimal approach for harvesting the patellar transplant in order to spare the nerve branches. MATERIAL AND METHODS: The patellar transplant is harvested via two vertical incisions, one on the apex of the patella and the other along the protrusion of the anterior tibial tubercle. After harvesting the bony trans

Lower extremity mechanics of iliotibial band syndrome during an exhaustive run.

Injury patterns in distance running may be related to kinematic adjustments induced by fatigue. The goal was to measure changes in lower extremity mechanics during an exhaustive run in individuals with and without a history of iliotibial band syndrome (ITBS). Sixteen recreational runners ran to voluntary exhaustion on a treadmill at a self-selected pace. Eight runners had a history of ITBS. Twenty-three reflective marker positions were recorded by an eight-camera 120 Hz motion capture system. Joint angles during stance phase were exported to a musculoskeletal model (SIMM) with the iliotibial b

Development of an artificial meniscus using polyvinyl alcohol-hydrogel for early return to, and continuance of, athletic life in sportspersons with severe meniscus injury. II: animal experiments.

Due to the prognosis and sporting needs of young athletes who have severe meniscus injury, we have developed an artificial meniscus using polyvinyl alcohol-hydrogel (PVA-H). Biomechanical studies have shown that the human meniscus has various mechanical functions; in our mechanical tests, specimens of human meniscus (part I of this study) demonstrated unique viscoelastic properties. Those tests also showed that a high water-content PVA-H has very similar mechanical properties to those of human meniscus and potential as a substitutive meniscus implant. To assess further the use of PVA-H as an a

Sagittal plane knee translation and electromyographic activity during closed and open kinetic chain exercises in anterior cruciate ligament-deficient patients and control subjects.

Using electrogoniometry and electromyography, we measured tibial translation and muscle activation in 12 patients with unilateral anterior cruciate ligament injury and in 12 control subjects. Measurements were made during an active extension exercise with 0-, 4-, and 8-kg weights and during squats on two legs and on one leg where the projection of the center of gravity was placed over, behind, and in front the feet. In the uninjured subjects, tibial translation increased with increasing load except during the squat with the center of gravity behind the feet, which produced the smallest transla

Collagenase activity in anterior cruciate ligament: protective role of the synovial sheath.

To evaluate the protective role of the synovial sheath of the anterior cruciate ligament (ACL), we have developed a synovectomy model that exposes the ACL substance to the intra-articular environment with and without hemarthrosis. Histology and the level of collagenase activity were studied to assess intrinsic ligament alterations. The treatment groups studied were as follows: ACLs of sham-operated knees receiving arthrotomy only, ACLs of knees receiving arthrotomy and acute hemarthrosis, ACLs of knees that underwent synovectomy, and ACLs of knees that underwent both synovectomy and acute hema

Energy cost of paraplegic locomotion.

We measured the physiological energy expenditure that was associated with several modes of mobility in 151 individuals with paraplegia that resulted from spinal cord injury. The relationship of the neurological level of the spinal lesion, extent of paralysis, orthotic requirement, and type of gait pattern was evaluated. The patients who required a bilateral knee-ankle-foot orthosis in order to walk with a swing-through crutch-assisted gait had an average rate of oxygen consumption that was 43 per cent greater than that of the patients who used a wheelchair and 38 per cent greater than was requ

Radiology of sports-related injuries.

Participation in organized sports and fitness activity continues to increase in the United States. As a result, more acute bone and soft-tissue injuries are occurring in this patient population. Chronic overuse syndromes are also more common today. It is important for radiologists to understand the mechanism of injury so that they can properly approach the imaging of these patients. Although magnetic resonance imaging (MRI) is frequently the technique of choice, other imaging methods are also important and may, in certain cases, be preferred to MRI. In this review, we discuss extremity injurie

Young Athletes After Anterior Cruciate Ligament Reconstruction With Single-Leg Landing Asymmetries at the Time of Return to Sport Demonstrate Decreased Knee Function 2 Years Later.

BACKGROUND: Previous work shows that young athletes after anterior cruciate ligament reconstruction (ACLR) demonstrate single-leg (SL) landing movement asymmetries at the time of return to sport (RTS); however, the effect of movement asymmetries on longitudinal knee-related function after ACLR has not been examined. Hypothesis/Purpose: The purpose of this study was to examine the effect of SL drop-landing movement symmetry at the time of RTS on knee-related function 2 years later in young athletes after ACLR. The first hypothesis was that young athletes who demonstrated SL drop-landing asymmet
