Does a bone deformity of the distal phalanx undergo remodeling after removal of a congenital ectopic nail?: A case with periodic radiographic follow-up.

BACKGROUND: Congenital ectopic nails are rare and are characterized by the presence of nail-like tissue mainly at the tip of a finger or toe. OBJECTIVE: Although the accompanying bone deformity might undergo remodeling after removal of the ectopic nail, it remains unknown whether complete bone remodeling can be eventually achieved, and whether such remodeling is necessary to improve the final cosmetic appearance of the concerned fingertip. METHODS AND RESULTS: Follow-up with periodic radiographic examination for 1 year after surgery in a child with congenital ectopic nail revealed no bone re…

The relationship between the extensor tendon enthesis and the nail in distal interphalangeal joint disease in psoriatic arthritis--a high-resolution MRI and histological study.

OBJECTIVES: Diffuse swelling of the distal interphalangeal (DIP) joint beyond the joint margin is a common feature of arthritis in psoriatic arthritis (PsA). The purpose of this study was to explore the microanatomical basis for the inflammation and nail disease in PsA using a combined high-resolution magnetic resonance imaging (MRI) and histological studies. METHODS: High-resolution MRIs of the DIP joint were obtained in 30 subjects [10 PsA, 10 osteoarthritis (OA) and 10 normal volunteers]. The relationship between the DIP joint capsule and associated tendon enthesis and the nail bed and roo

Distal metaphyseal fractures of tibia: a prospective randomized trial of closed reduction and intramedullary nail versus open reduction and plate and screws fixation.

BACKGROUND: To compare closed intramedullary nailing with open plate and screw fixation and set the indications for each treatment modality … I and 88.2% in Group II (p = 0.71). CONCLUSION: Our results have shown that locked intramedullary nails have an advantage in the duration of operation, restoration of motion, and reduced wound problems, and anatomic plate and screws can restore alignment better than intramedullary nails. It can be concluded from this study that intramedullary nails ar …

New concepts in the treatment of ankle joint fractures. The IP-XS (XSL) and IP-XXS (XXSL) nail in the treatment of ankle joint fractures.

INTRODUCTION: The most important factor in the treatment of ankle joint fractures is stable anatomical reconstruction of the syndesmosis and joint surface. In the course of this, attention must be paid to soft-tissue damage with the risk of deep infections. Early functional therapy and exercise tolerance must be called for. The choice of surgical access route, in particular in the case of critical arterial circulation, and the possible irritation of the soft tissue by the osteosynthesis material prompted us to seek alternative osteosynthesis techniques. MATERIAL AND METHODS: Following a precl

Mechanical complications associated with the use of the unreamed AO femoral intramedullary nail with spiral blade: first experiences with thirty-five consecutive cases.

OBJECTIVE: To evaluate the unreamed femoral nail with spiral blade (UFN-SB) in the treatment of subtrochanteric femur fractures. DESIGN: Retrospective review of a consecutive series of twenty-eight fractures. Sixteen patients had metastatic disease. SETTING: Academic teaching hospital. MAIN OUTCOME MEASURES: Clinical and radiographic examination. RESULTS: Five mechanical complications were observed in four patients. In one patient with a metastastic fracture, we noted a breakage of the SB twice. In patients with traumatic subtrochanteric fracture, migration of the SB was observed three tim

Treatment of pathological fractures of the humeral shaft due to bone metastases: a comparison of intramedullary locking nail and plate osteosynthesis with adjunctive bone cement.

In a retrospective study 37 patients were surgically treated for 38 fractures (27 actual and 11 impending) caused by metastatic lesions of the humerus shaft. A comparison between plate osteosynthesis with bone cement (20 cases) and intramedullary nailing (18 cases) was made. There was no mortality related to the surgical procedure. The patients' survival rate was 61% after 3 months and 44% after 6 months; six were alive after 1 year. Overall, a subjective and objective relief of pain was achieved in 92% and 79%, respectively. Restoration of arm function was improved in 95%. The operative cours

Psoronychomycosis: A New Term for an Old Problem.

Occasionally, psoriatic nail changes are seen in psoriatic patients. The prevalence of psoriasis of the nails has been reported to range from 15% to 79%. Even with effective systemic treatment for psoriasis, the nails may not improve, and these nails are considered nonresponsive psoriatic nails. Psoriatic nails are rarely investigated, and it is assumed that the nail changes are simply attributed to psoriasis because of their clinical similarity. Even patients with nails unresponsive to systemic treatment, or psoriasis treated topically or with phototherapy, onychomycosis is often forgotten, a…

[Brachyonychia of different types in a mother and daughter].

Different types of brachyonychia were observed in a mother and her daughter. The mother presented a unilateral racket thumb-nail (localized brachyonychia "en raquette" of Basset). In the case of the daughter the thumbs and three middle fingers of both hands were involved, the nails of the little fingers being normal (generalized brachyonychia "en raquette" of Basset). Also, the daughter's toe-nails were short and wide. There was a family history of short nails "on all fingers" occurring in the two sisters of the mother's father. We believe that localized and generalized types of racket nails a…

Modern trends in internal fixation of femoral shaft fractures in children. A critical review.

Elastic intramedullary (IM) devices, which avoid the physeal region like Nancy and Ender nails do, can be used in children younger than even 4 years without compromising the trochanter physis or risking an avascular necrosis. Intramedullary fixation of femoral shaft fractures leads to satisfactory results of alignment and union. Prerequisites for intramedullary closed nailing are a traction table, especially for older children and adolescents, and an image intensifer. Interlocking IM, Ender, and Nancy nails all have the equal advantage of early weight bearing and reduced hospitalization time c…

Extracorporal noninvasive acute retraction of STRYDE® for continued lengthening in cases with limited nail stroke: a technical less invasive solution to reload the STRYDE®.

With STRYDE® nails (NuVasive Specialized Orthopedics, San Diego, CA), lengthening nails for full weight-bearing during callus distraction have been available CE-certified since February 2019 in Europe. At present only antegrade femur nails and tibia nails with various lengths and diameters are available. Due to a mismatch of bone length and realizable (implantable) nail length e.g., in cases of skeletal dysplasia or achondroplastic dwarfism, it may happen that a desired lengthening of 6-8 cm cannot be realized because of a nail stroke from only 5 cm. Retrograde usage of antegrade piriformis na…

Staged Reconstruction of Distal Femoral Osteosarcoma in a 10-year-old Girl using a Hybrid Endoprosthesis-Motorised Lengthening Nail Construct: A Case Report.

BACKGROUND: Reconstruction of tumour defects in skeletally immature patients poses a particular challenge due to the resection of growing physes and resulting limb-length discrepancies. Expandable prostheses allow outpatient lengthening with fewer surgical revisions, but the limb is lengthened within the prosthesis rather than native bone. CASE DESCRIPTION: A 10-year-old girl with no medical history presented with distal right thigh pain and swelling. Imaging and biopsy confirmed osteosarcoma. She underwent wide local excision and initial endoprosthetic reconstruction, followed by a planned s

Application of a self-developed femoral artery compression hemostasis device in proximal femoral nail anti-rotation surgery for intertrochanteric fractures: a case report.

BACKGROUND: Intertrochanteric fractures of the femur are a prevalent type of hip fracture among the elderly. While surgical treatment is the preferred approach for managing these fractures, conventional tourniquets cannot be applied in hip surgery due to anatomical constraints. As a result, effectively controlling intraoperative and perioperative blood loss has long posed a significant clinical challenge. CASE PRESENTATION: A 70-year-old male patient with a comminuted fracture in the left intertrochanteric region of the femur underwent closed reduction and internal fixation using a proximal f
