Decoding BDNF in neurodevelopmental, neurodegenerative, and neurological disorders: mechanisms and therapeutic perspectives.

Brain-Derived Neurotrophic Factor (BDNF) is an essential neurotrophin involved in neuronal survival, synaptic plasticity, and neurogenesis, critical for normal brain function as well as the pathology of neurological and psychiatric disorders. It primarily functions by activating TrkB receptors, which subsequently modulate intracellular signalling pathways such as PI3K-Akt, Ras-MAPK, and PLC-γ1. The expression of BDNF is precisely controlled by genetic, epigenetic, and transcriptional mechanisms, with environmental and activity-dependent factors providing further modulation. However, it is wort

Proangiogenic Mechanisms and Modifications of Mesenchymal Stem Cells with a Focus on Neurological Disorders.

Mesenchymal stem cells (MSCs) are adult stem cells with extensive differentiation potential, sourced from bone marrow, adipose tissue, umbilical cord blood, and other tissues. MSCs from different origins exhibit distinct functional characteristics. These cells have demonstrated therapeutic efficacy in various neurological disorders, primarily by modulating immune responses, promoting neovascularization, and aiding neural circuit reconstruction. Notably, the strong proangiogenic properties of MSCs play a crucial role in disease treatment and regression. This review focuses on the application of

Enhanced Gait Variability Index in Neurological and Geriatric Rehabilitation: A Scoping Review.

BackgroundGait variability is a sensitive marker of neuromotor function and a clinically relevant indicator of fall risk and mobility impairment in neurological and geriatric populations. The Enhanced Gait Variability Index (eGVI) has emerged as a promising composite measure that captures multidimensional gait irregularity in a single interpretable score, offering a practical advantage over single-parameter approaches. Despite growing clinical interest, the evidence base supporting its standardized implementation has not been mapped.ObjectivesTo characterize populations and clinical contexts w

Carboxymethyl chitosan for neurological drug delivery: Current trends and future prospects.

The complexity of drug delivery across the blood-brain barrier (BBB) makes disorders of the central nervous system (CNS), such as Alzheimer's disease, Parkinson's disease(PD), brain tumors, and other neurodegenerative conditions, a growing global health burden. Poor brain bioavailability and systemic side effects are common problems with traditional therapies. Delivery systems based on nanotechnology have shown promise as solutions to these problems. These include carboxymethyl chitosan (CMC), a water-soluble, biocompatible, and biodegradable chitosan derivative that has drawn a lot of interes

Mindfulness-based stress reduction for functional neurological disorder: A feasibility study.

BACKGROUND: Functional neurological disorder (FND) is common and associated with substantial disability, yet evidence-based treatment options remain limited. Because maladaptive attentional processes are central to FND pathophysiology, mindfulness-based interventions may be beneficial. Mindfulness-Based Stress Reduction (MBSR) has strong empirical support across physical and mental health conditions, but evidence in FND is sparse. OBJECTIVES: To assess feasibility, acceptability, and preliminary clinical outcomes of MBSR for patients with FND. METHODS: In this monocentric, single-arm feasibi

Mesenchymal Stem Cell Therapy for Neurological Complications of Prematurity: A Narrative Review.

Background: Preterm birth is a leading cause of neonatal mortality and long-term disability worldwide. Injury in premature infants is demonstrated by disrupted organ development from inflammation, oxidative stress, hypoxia, and impaired vascular maturation. Current therapies largely provide supportive care and do not directly promote tissue regeneration. Mesenchymal stem cell (MSC)-based therapies have emerged as a potential strategy to enhance endogenous repair across organ systems commonly affected by prematurity. Results: Evidence indicates that MSCs exert therapeutic effects primarily thro

The Ketogenic Diet: A Possible Intervention for Improving Hippocampal Function in Neurological Disorders.

With a focus on the hippocampus, in this review we examined the emerging role of the ketogenic diet (KD) in treating neurological disorders. There are multiple pathways through which various versions of the KD influence the hippocampus: energy metabolism shifts, neurotransmitter modulation, neuroinflammation control, and synaptic plasticity and epigenetic regulation modifications. Both animal studies and clinical research, with emphasis on epilepsy and Alzheimer disease, have revealed the therapeutic potential of KDs. By modifying energy metabolism and lowering neuroinflammation, KDs may have

Spatiotemporal abnormalities in brain networks as a signature of neurological damage in Wilson's disease.

OBJECTIVES: Resting-state electroencephalogram (EEG) microstates serve as dynamic markers of intrinsic brain activity, reflecting the transient coordination of large-scale neural networks. However, their role in Wilson's disease (WD) remains largely unexplored. This study aimed to investigate the spatio-temporal dynamics of EEG microstates in WD and explore their relationship with clinical manifestations. METHODS: Resting-state EEG data were collected from 17 patients with WD and 17 healthy controls (HCs). Microstate parameters were compared between the two groups. Four commonly recognized ty

Neurological manifestations and genotype-phenotype correlations in NDUFAF6-associated mitochondrial disease.

NDUFAF6 encodes a mitochondrial complex I assembly factor essential for the proper biogenesis and stability of the nicotinamide adenine dinucleotide (NAD) + hydrogen (H) (NADH)-ubiquinone oxidoreductase complex. Pathogenic variants in NDUFAF6 have been increasingly recognized as a cause of mitochondrial disease, particularly Leigh syndrome, a severe neurodegenerative disorder characterized by bilateral symmetrical lesions in the central nervous system. To date, fewer than 50 patients with NDUFAF6-related mitochondrial disease have been reported, displaying a broad phenotypic spectrum ranging f

Understanding the long-term neurological effects of SARS-CoV-2 infection.

Post-COVID-19 condition (PCC), also known as long COVID, is a heterogeneous condition marked by persistent symptoms following acute SARS-CoV-2 infection. As approximately 6% of people who have experienced acute COVID-19 are estimated to develop PCC, the potential population is vast. Many of the key symptoms of PCC reflect involvement of the nervous system, ranging from cognitive impairment ('brain fog'), headaches and fatigue to anxiety and depression. This Review summarizes the spectrum of neurological and psychological symptoms that occur following acute SARS-CoV-2 infection, with a particul

What are the neurological, neurovascular, and psychological responses to stretching across the general population?

Stretching is extensively studied and implemented to improve flexibility, as a component of warm-ups to prepare for activity, to mitigate musculotendinous injury risk, improve cardiovascular health parameters, and optimize post-exercise recovery, among others. However, less attention has focused on the effects of stretching on neurological, neurovascular, and psychological outcomes. Stress represents a pervasive element in contemporary life, with chronic mental and physical stress contributing to adverse health consequences. Evidence suggests that a bout of stretching can acutely increase symp
