Bilateral fast MR imaging of the rheumatoid wrist.

To evaluate the structural changes in the carpal tunnel and possible intrinsic median nerve damages in RA patients, quantitative bilateral magnetic resonance imaging (MRI) of the wrists was performed by means of a fast imaging sequence. Thirty-three women with RA and 42 controls were examined. The length of the carpal tunnel, the carpal tunnel volume/wrist volume (CTV/WV) ratio and the signal intensity of the nerve were calculated in both groups, bilaterally. The CTV/WV ratio was 0.12 in the patients and 0.11 in the control group (p = 0.007). A negative association was found between disease du

Vasculitis confined to peripheral nerves.

The clinical, electrophysiological and pathological features and prognosis of 25 patients with vasculitis selectively affecting the peripheral nervous system were evaluated. Although most patients had a history of mononeuritis multiplex or an asymmetrical neuropathy six out of 25 had a symmetrical neuropathy, both clinically and on neurophysiological testing, by the time of presentation. There were no signs of accompanying systemic vasculitis in any of the patients and serological abnormalities were limited to an elevated erythrocyte sedimentation rate (ESR) in nine out of 21 patients and low

Massive nerve root enlargement in chronic inflammatory demyelinating polyneuropathy.

OBJECTIVE: To report three patients with chronic inflammatory demyelinating polyneuropathy (CIDP) presenting with symptoms suggestive of cervical (one patient) and lumbar root disease. METHODS: Nerve conduction studies, EMG, and nerve biopsy were carried out, having found the nerve roots to be very enlarged on MRI, CT myelography, and at surgery. RESULTS: Clinically, peripheral nerve thickening was slight or absent. Subsequently one patient developed facial nerve hypertrophy. This was mistaken for an inner ear tumour and biopsied, with consequent facial palsy. Neurophysiological tests sugges

A comparison of transcranial magnetic stimulation with electroneuronography as a predictive test in patients with Bell's palsy.

The aim of this study was to examine the neuronographic findings of electrical and transcranial magnetic stimulation of the facial nerve and to compare their ability to predict clinical recovery from idiopathic facial nerve palsy (Bell's palsy). Eighty-six patients were examined clinically and neurophysiologically immediately on presentation to Tampere University Hospital. Electroneuronography (ENoG) and transcranial magnetic stimulation (TMS) were performed 1-6 times for each patient. The time interval between each examination varied from 2 to 7 days. Seventy-eight patients were followed for

Chronic tension-type headache: amitriptyline reduces clinical headache-duration and experimental pain sensitivity but does not alter pericranial muscle activity readings.

In a double-blind, placebo-controlled trial, the effect of 75 mg of a slow-release formulation of amitriptyline on the clinical severity of chronic tension-type headache and on headache-associated neurophysiological parameters (EMG activity, exteroceptive suppression of temporal muscle activity, contingent negative variation (CNV) and experimental pain sensitivity) was investigated. All of the patients treated had a history of headaches of many years' standing and many of them had failed attempts at treatment. In the amitriptyline group, a significant reduction in daily headache duration was a

[Transcranial magnetic stimulation in diagnosis of trigeminal neuralgia].

Patients with trigeminal neuralgia are usually investigated by elicitation of the orbicularis oculi reflex and trigeminal evoked potentials. These neurophysiological methods do not allow direct judgement of the trigeminal nerve. By transcranial magnetic stimulation, however, non-invasive investigation of the efferent part of the trigeminal nerve is possible. 10 patients (4 men, 6 women, aged from 43 to 73 years) with trigeminal neuralgia affecting the second or third division were examined. In all patients bilateral long-latency responses after stimulation of the tractus corticonuclearis and s

Myoclonus and sensorimotor integration in a patient with Ramsay Hunt syndrome.

Clinical and neurophysiologic studies were done on a patient with action myoclonus secondary to Ramsay Hunt syndrome (dyssynergia cerebellaris myoclonica). Myoclonic jerks in the arms were much more common during movements directed to a target than in other movements. They appeared to be triggered primarily by external sensory inputs relevant to the movement rather than by the motor activity itself. Both somatosensory and visual inputs appeared able to trigger the myoclonic jerks. Myoclonic jerks in the deltoid muscle followed finger contact with a target by approximately 100 msec. Electrical

A quantitative analysis of pendular motion of the lower leg in spastic human subjects.

The purpose of this study was to examine gravity-induced oscillations of the lower leg in normal and spastic subjects, with a view towards evaluating a clinical test of spasticity called the "pendulum" test. Motivations for studying the pendulum test were to determine if realistic aspects of spasticity and neuromuscular control could be incorporated into a description of the motion, and to better understand the underlying neurophysiological disturbances in spasticity. For passive limb motion (in which no reflex excitation occurred), a second-order linear model did not provide an adequate descr

[Hypothyroid myopathy. Clinico-pathologic study of 20 cases].

20 patients afflicted with primary hypothyroidism were studied in order to evaluate the association of clinical or sub-clinical myopathy, detected by neurophysiological (electromyography) (EMG) or neuropathological methods (muscular biopsy with enzymatic study). 70% of the patients had muscular weakness (moderate in 30% and severe in 40%) of the scapular and pelvic muscles. 60% of the patients had muscular cramps. There was no myodema nor muscular atrophy or hypertrophy. Seric CPK was high in 70% of the cases. EMG was myopathic in 65%. All cases with weakness registered EMG alterations. The hi

[Palm stimulation in carpal tunnel syndrome].

In order to verify the sensory nerve conduction velocity (SNCV) by median nerve palmar stimulation regard to each single finger, we tested 48 patients with typical CTS symptoms, confirmed by neurophysiological examination. The control group consisted of twenty-seven healthy volunteers, of similar mean age. The SNCVs were obtained by stimulating the II finger (M2), III (M3), IV (M4) and II and III intermetacarpo-phalangeal site (respectively P2 and P3) for median nerve; IV finger (U4) and IV intermetacarpophalangeal site (P4) for ulnar nerve. Effects of non-opioid antitussives on epileptiform activity and NMDA responses in hippocampal and olfactory cortex slices.

Three commonly used antitussive compounds were tested for their ability to block epileptiform activity recorded extracellularly from hippocampal and olfactory cortex slices maintained in vitro. Antitussives were bath-applied to brain slices either before or after epileptiform activity was induced. Dextromethorphan (DM) prevented electrically evoked epileptiform afterdischarges and arrested spontaneous bursting induced by exposure to added NMDA or to Mg2(+)-free medium. In contrast, caramiphen (CM) and carbetapentane (CB) were effective against epileptiform activity induced by Mg2(+)-free mediu

Brain life and brain death--the anencephalic as an explanatory example. A contribution to transplantation.

The current debate regarding the suitability of anencephalics as organ donors is due primarily to misunderstandings. The anatomical and neurophysiological literature shows that the anencephalic lacks a cerebrum because of the failure of neuralplate fusion. However, even the incomplete function of an atrophic brain stem is currently accepted at law in most if not all countries as sufficient for brain life: which is to say, cessation of breathing is currently required in order to make the diagnosis of brain death. Because of the extensive incompleteness of the anencephalic's brain, it is not pos

Somatosensory projections of cerebellar granule cell layer of giant bushbaby, Galago crassicaudatus.

Recent neurophysiological studies of the granule cell (GC) layer in opossums and rats revealed extensive somatosensory projections to the cerebellar hemispheres and caudal vermis. These projections are organized as asomatotopic mosaics that are species-specific. To determine whether similar projections exist in a primate with a relatively small and simple cerebellum, we explored the GC layer of exposed folial crowns of anterior and posterior lobe cerebellar cortex of anesthetized giant galagos using juxtathreshold natural stimulation of mechanoreceptors and in-depth microelectrode micromapping

Diagnostic value of somatosensory evoked potentials as compared to neurological score, EEG and CT in infancy--a prospective study.

The present investigation was performed to check, whether additional Med-SEP (somatosensory evoked potentials after stimulation of the median nerve) registration is necessary in the clinical and laboratory work-up in neurologically abnormal infants. For this purpose 28 infants suspected to develop cerebral palsy underwent neurological, neurophysiological and radiological examination at the age of 1 to 10 months. They were prospectively followed-up to their second year of life. On follow-up examination 22 of them showed a pathological development. 12 of them developed cerebral palsy, 7 were mod

[Visual and acoustic evoked potentials in multiple sclerosis].

A central topic of recent research on multiple sclerosis (MS) concerns the development of sensitive neurophysiological techniques with the ability to detect lesions at the earliest possible stage. Specifically, the recording of evoked potentials has during the past decade demonstrated its diagnostic value by disclosing clinically silent lesions especially of the optic nerve and brainstem region. The visual evoked potential (VEP) was registered in 177 patients with definite, probable and possible MS (McAlpine). Abnormal latencies were obtained in 86/46/34% of these three patient groups, respect

Median and ulnar nerve transections treated with microsurgical interfascicular cable grafting with autogenous sural nerve.

Interfascicular nerve grafting was used in 37 median and 26 ulnar nerves, all completely transected. In a follow-up period of 2.5 to 5 years, useful motor recovery (M3 or higher) was achieved in 84% of median nerve lesions; in ulnar nerve lesions, useful motor recovery (M2+ or higher) was achieved in 73%. Sensory recovery with some return of two-point discrimination sense was found in 63% of low median and 50% of low ulnar nerve lesions. In the median nerve group, results for patients younger than 20 years of age were significantly better than in older patients. Neurophysiological investigatio

Physical Training in Parkinson's Disease: A Systematic Review of Neurobiological Mechanisms, Brain-Muscle Adaptations, and Implications for Personalized Rehabilitation.

BACKGROUND: Physical Training (PT), defined a planned, structured, and long-term exercise intervention, is recognized as an important non-pharmacological therapy in Parkinson's disease (PD). Beyond its symptomatic benefits, experimental evidence suggests that PT may also modulate pathological mechanisms involved in dopaminergic neurodegeneration. This systematic review summarizes the neurobiological and neurophysiological adaptations induced by PT in animal models of PD and in patients with PD, and proposes mechanistically driven recommendations for personalized exercise prescription. "Real-Time Neuromonitoring" Increases the Safety and Non-Invasiveness and Shortens the Duration of Idiopathic Scoliosis Surgery.

Introduction: A practical solution to the incidental unreliability of intraoperative neuromonitoring (IONM) may be the simultaneous neurophysiological recording and control of the surgical field through a camera (the concept of "Real-time" IONM). During "Real-time" IONM, the surgeon is immediately warned about the possibility of damage to the neural structures during, but not after, standard idiopathic scoliosis (IS) corrective surgery procedures (the concept of "Surgeon-neurophysiologist" interactive, verbal IONM). This study aimed to compare the advantages, utilities, reliabilities, and time
