The life-changing impact of peripartum cardiomyopathy: an analysis of online postings.

BACKGROUND: Peripartum cardiomyopathy (PPCM), a form of acute heart failure, is a life-altering condition affecting thousands of pregnant or postpartum women. Little is known about the overall impact of PPCM on women's lives. PURPOSE: To describe the contents of postings made on the My Space® PPCM support group Web site by women diagnosed with PPCM. METHODS: A mixed methods design. Two hundred and forty-seven postings made by 156 people from 2005 to 2008 were copied from the Web site. Key words and phrases were sorted into categories, quantified, and then arranged into themes using the five

Antibiotics on urethral catheter withdrawal: a hit and miss affair.

Globally, millions of patients undergo urethral catheterization every year. Our objective was to study the current use of prophylactic antibiotics on urinary catheter withdrawal. A questionnaire (N = 300) was sent to healthcare professionals involved in the management of patients with urinary catheters (consultant microbiologists, infection control nurses, consultant urologists, specialist nurses in urology, continence advisers and consultants in the care of older people). The questionnaire asked about their use of prophylactic antibiotics on the withdrawal of a urethral catheter. Sixty percen

Midwife-led care and obstetrician-led care for low-risk pregnancies: A cost comparison.

OBJECTIVE: Low-risk pregnant women cared for by midwives have similar birth outcomes to women cared for by physicians, although experiencing fewer medical procedures. However, limited research has assessed cost implications in the United States. Using national data, we assessed costs and resource use of midwife-led care vs obstetrician-led care for low-risk pregnancies using a decision-analytic approach. METHODS: We developed a decision-analytic model of costs (health plan payments to clinicians) and use of medical procedures during childbirth (epidural analgesia, labor induction, cesarean bi

Effect of a Nurse-Led Community Health Worker Intervention on Latent Tuberculosis Medication Completion Among Homeless Adults.

BACKGROUND: Tuberculosis (TB) disproportionately affects marginalized and impoverished homeless adults. Although active TB can be prevented by treating latent TB infection (LTBI), individual factors, such as high prevalence of depression and anxiety, drug and alcohol use, and unstable housing, lead to poor LTBI treatment adherence and completion among homeless adults. OBJECTIVES: We hypothesized that the delivery of a tailored nurse-led, community health worker (RN/CHW) program across the LTBI continuum of care (e.g., screening, diagnosis, and treatment) that delivers 3HP treatment (3HP: rifa

Pilot study of nurse-led rheumatic heart disease echocardiography screening in Fiji--a novel approach in a resource-poor setting.

We designed a pilot study of a training module for nurses to perform rheumatic heart disease echocardiography screening in a resource-poor setting. The aim was to determine whether nurses given brief, focused, basic training in echocardiography could follow an algorithm to potentially identify cases of rheumatic heart disease requiring clinical referral, by undertaking basic two-dimensional and colour Doppler scans. Training consisted of a week-long workshop, followed by 2 weeks of supervised field experience. The nurses' skills were tested on a blinded cohort of 50 children, and the results w

Safe practices for enteral nutrition in critically ill patients.

Promoting patient safety in the enterally fed critically ill patient is dependent on nursing surveillance and recognition of potential areas of patient harm and medical errors. Identifying areas for potential human error, administrative and organizational conditions that are conducive to error, and the patient's own tolerance to EN need to be recognized by the critical care nurse if the risk for EN complications is to be minimized.

Evaluation of a successful fetal alcohol spectrum disorder coalition in Ontario, Canada.

Leading a successful coalition that benefits both the members and the community is a difficult task. Coalitions are complex and require a great deal of skill to initiate, lead, and evaluate. This article examines a successful coalition, developed to build community capacity to address fetal alcohol spectrum disorder (FASD). FASD is a complex, multidimensional health issue common in many communities. Coalitions can be effective in tackling these types of issues and fit with community capacity-building approaches to health promotion. The Internal Coalition Outcome Hierarchy (ICOH) model (Cramer,

Assessment and care of the newborn with Down syndrome.

Well-defined, distinctive phenotypic features and natural history characterize Down syndrome, the most frequent form of developmental disability caused by a microscopically demonstrable chromosomal aberration. Triplicate state or trisomy of all or a portion of chromosome 21 causes Down syndrome. There are 3 genetic mechanisms leading to Down syndrome, or trisomy 21: nondisjunction, translocation, and mosaic. With advances in prenatal testing techniques, the diagnosis can occur before birth. This article explores the embryology and pathogenesis of Down syndrome and its multisystemic effects on

Measuring restorative care provided by nursing assistants: reliability and validity of the Restorative Care Behavior Checklist.

BACKGROUND: Most prior research on the outcomes of restorative care programs has been focused on resident outcomes including such things as functional performance, behavior, and quality of life. Treatment fidelity issues and compliance with the programs were not addressed in these studies. Without such an evaluation, it is impossible to evaluate the effectiveness of restorative care activities. PURPOSE: To test the reliability and validity of the Restorative Care Behavior Checklist (RCBC), which is an observed measure of restorative care activities performed by nursing assistants (NAs). Moral distress among physicians and nursing staff in surgical settings: a comparative study.

BACKGROUND: Moral distress is a relevant occupational health issue in hospital settings, particularly in high-complexity environments such as surgical units, where organizational constraints, ethical dilemmas, and interprofessional dynamics shape professionals' experiences. Comparative evidence is needed to understand how moral distress manifests across professional groups working within the same clinical context. METHODS: This cross-sectional comparative study examined moral distress among physicians and nursing staff working in surgical settings at a public university hospital in southern B
