Acute Total Hip Arthroplasty with or Without Internal Fixation for Acetabular Fractures in the Elderly: A Case Series.

Background and Objectives: Acetabular fractures in elderly patients are increasing in incidence and are frequently associated with osteoporotic bone, fracture comminution, marginal impaction, and pre-existing joint degeneration. Open reduction and internal fixation (ORIF) alone in this population is associated with high rates of fixation failure, post-traumatic osteoarthritis, and secondary conversion to total hip arthroplasty (THA). Acute THA, with or without concomitant internal fixation, has emerged as an alternative strategy aimed at enabling early mobilization and reducing reoperation rat

Double-plate compound osteosynthesis of a periprosthetic proximal tibial shaft fracture in an elderly patient with Paget's disease using PHILOS and LCP plates.

INTRODUCTION: Paget's disease of the bone (PDB) is characterized by abnormal bone remodeling and accelerated osteoclast activity, weakening bones and leading to deformities, fractures, or osteoarthrosis, commonly involving the tibia. Treating proximal tibial shaft fractures in PDB patients is challenging due to compromised bone quality and unique anatomical considerations. Despite ongoing research, clear guidelines or conclusive evidence on the optimal treatment strategies of these fractures remain limited. CASE REPORT: A 91-year-old male presented with severe right knee pain and inability to

EOS stereographic assessment of femoral shaft malunion after intramedullary nailing. A prospective series of 48 patients at 9 months' follow-up.

INTRODUCTION: The aim of the present study was to assess femoral shaft malunion following anterograde intramedullary nailing, using low-dose EOS stereoradiography. The study hypothesis was that our surgical technique is associated with radiological rotation disorder rates equivalent to those reported in the literature. METHODS: All patients with unilateral femoral shaft fracture treated by anterograde nailing between January 2014 and December 2016 and followed up in our structure were included in a single-center prospective study. The main endpoint was≥15° transverse malrotation compared to t

Survivorship of the Hip Joint After Acetabulum Fracture.

PURPOSE: The purposes of this study were to determine the rate of failure of the hip joint after acetabulum fracture and to identify risk factors. METHODS: Acetabulum fractures treated over 17 years at a level-1 trauma center were reviewed. Patient, injury, and treatment factors were assessed regarding possible association with failure of the hip joint: end-stage arthrosis and/or total hip arthroplasty (THA). RESULTS: Seventy percent were treated with primary open reduction and internal fixation (ORIF). Seventy-two (12.5%) of 575 fractures underwent THA; 64 were after initial ORIF. The mean

Modified Hemihamate Arthroplasty With Oblique Osteotomy of the Middle Phalanx Base to Reconstruct Proper Contour of the Articular Surface.

Treatments of unstable dorsal fracture dislocations of the proximal interphalangeal (PIP) joint are very challenging. The authors performed modified hemihamate arthroplasty for these injuries in 13 patients (13 fingers). Our surgical technique was unique in the point that we put volarly oblique osteotomy in the coronal plane at the recipient site to create an adequate reconstruction of the volar lip of the articular surface of the middle phalanx. Affected fingers are index in 2, middle in 4, ring in 3, and little in 3, and the mean duration of follow-up was 14 months. Hemihamate arthroplasty w

Case Series Evaluating the Operative and Nonoperative Treatment of Scapular Fractures.

The injury parameters and patient characteristics that affect function after scapular fracture are poorly defined. We performed a retrospective review of 594 adult patients with a minimum 12-month follow-up after scapular fracture. Functional outcomes were prospectively assessed using the American Shoulder and Elbow Surgeons (ASES) survey in 153 patients after a mean of 62 months of follow-up. The population was 78% male, and 88% had injuries caused by a high-energy event. Only 4.6% had injuries isolated to the scapula. All fractures healed primarily and the mean ASES score was 79.3, indicatin

Evaluation of cerclage wiring in the treatment of subtrochanteric fractures.

Treatment of subtrochanteric fractures is challenging because of their typical displacement pattern. Use of circumferential cerclage wires can be added to intramedullary nailing to facilitate better anatomical reduction. Concerns exist regarding additional soft tissue damage and ischemia of the periosteum. The aim of this study was to assess the effect of cerclage on union and infection rates. The postoperative results of 115 patients over 11 years were retrospectively viewed. Twenty-three patients were treated with cerclage. The primary outcome measure was 'return to theatre for fixation fail

Fixation orientation in ankle fractures with syndesmosis injury.

Accurate reduction of the syndesmosis has been shown to be an important prognostic factor for functional outcome in ankle injuries that disrupt the syndesmosis. The purpose of the present case series was to assess the fixation orientation and the position of the fibula within the tibial incisura after open reduction and internal fixation of ankle fractures with syndesmosis injury. Computed tomography was used to assess the accuracy of the reduction. Twelve patients were included in the present case series. A ratio representing the relationship between the tibia and fibula and the orientation o

Total Hip Arthroplasty After Acetabular Fracture Is Associated With Lower Survivorship and More Complications.

BACKGROUND: Despite modern fracture management techniques allowing for near anatomic reduction of acetabular fractures, there continues to be a risk of posttraumatic arthritis and need for total hip arthroplasty (THA). Few well-controlled studies have compared THA after acetabular fractures with THAs performed for other indications in terms of survivorship or complications, and none, to our knowledge, present 10-year survivorship data in this setting. QUESTIONS/PURPOSES: (1) How does the 10-year survival of THA compare between those patients who underwent THA after an acetabular fracture and

Interventions for treating fractures of the patella in adults.

BACKGROUND: Fractures of the patella (kneecap) account for around 1% of all human fractures. The treatment of these fractures can be surgical or conservative (such as immobilisation with a cast or brace). There are many different surgical and conservative interventions for treating fractures of the patella in adults. This is an update of a Cochrane Review first published in 2015. OBJECTIVES: To assess the effects (benefits and harms) of interventions (surgical and conservative) for treating fractures of the patella in adults. SEARCH METHODS: We searched CENTRAL (2020, Issue 1), MEDLINE, Emba

Cost-effectiveness and quality-of-life outcomes of proximal humerus fracture treatment in adults aged 65 years and older in the United States.

BACKGROUND: Proximal humerus fractures (PHFs) are the third most common fragility fracture in older adults, and their rising incidence in the United States contributes substantially to health care spending. Although several randomized trials have evaluated outcomes across treatment strategies, the optimal management strategy remains debated. Few studies have compared all five common PHF treatment options using an integrated cost effectiveness framework. METHODS: A cost-utility analysis was performed to compare nonoperative management, intramedullary nailing, open reduction internal fixation,

Comparing the usefulness of a fluoroscopic navigation system in femoral trochanteric fracture for orthopaedic residents with the conventional method.

INTRODUCTION: Lag screw insertion into the ideal position is essential to obtain good results in open reduction and internal fixation for femoral trochanteric fracture. Tip-apex distance (TAD) is a widely adopted method for evaluating the risk of lag screw cut-out. Adaptive positioning technology (ADAPT) is a fluoroscopic computer-assisted surgery system that enables orthopaedic surgeons to guide the screw into a proper position intraoperatively. A randomized control study concluded that ADAPT resulted in excellent TAD. However, it was not significantly better than conventional methods when pe

Comparative Assessment of Functional Outcomes of Surgical Treatment in Patients with Distal Radial Fractures.

BACKGROUND: Unstable and comminuted distal radial fractures require surgical treatment by percutaneous insertion of Kir-schner wires, open reduction and fixation with a non-locking or locking plate or with an external device. The aim of this paper was to try to answer the following question: are there differences in functional treatment outcomes in patients after surgery with the use of Kirschner wires vs LCP plate fixation? MATERIAL AND METHODS: The study group included 100 patients after surgical treatment by closed reduction and simple fixation with Kirschner wires (50 patients) and by ope
