Comparison of Visual Analog Pain Score Reported to Physician vs Nurse.

BACKGROUND: The visual analog scale (VAS) is considered a reliable and validated measure of patient-reported acute pain. Patient-reported outcome measures are becoming the standard of care throughout the orthopedic community, but interpretation and clinical applications are still under investigation. The aim of the current study was to compare preoperative patient-reported VAS scores reported at the same visit to nursing staff and the treating surgeon. Our hypothesis was that there would be no difference in the scores reported. METHODS: This study is a retrospective cohort of 201 consecutive

Pedal power measurement as a diagnostic tool for functional vascular problems.

BACKGROUND: Cyclists with flow limitations in the iliac arteries complain of pain and loss of power. To investigate whether pedal power measurement has added value in diagnosing the underlying cause of flow limitations in the iliac arteries, we explored the sensitivity and specificity of various pedal power measurement variables. Moreover, it was assessed what the added value of pedal power measurement is compared to diagnosis based on the conventional ankle-brachial blood pressure index. METHODS: 25 healthy participants and 45 patients with unilateral arterial flow limitations were recruited

Effectiveness of Exercise Programs for Alleviation of Upper Body Pain in Patients with Spinal Cord Injury: A Systematic Review.

(1) Background: Upper body pain, particularly in the limbs and shoulders, is a common symptom among patients with spinal cord injury (SCI) and wheelchair users. Despite the focus on resistance muscle training as a suitable intervention for SCI individuals, findings across different populations and conditions have been inconsistent. (2) Methods: We conducted a systematic review to elucidate the correlations among exercise interventions, muscle strength enhancement, and pain reduction. A comprehensive literature search was performed using the keywords "spinal cord injury," "pain," "exercise," "d

Validation studies of rheumatoid arthritis patient-reported outcome measures in populations at risk for inequity: A systematic review and analysis using the OMERACT summary of measurement properties equity table.

BACKGROUND: Existing patient-reported outcome measures (PROMs) in rheumatoid arthritis (RA) may be limited in their applicability to populations that are at risk for inequities. We conducted a systematic review to identify and rate evidence in the validation studies for PROMs in populations at risk for inequity. METHODS: A systematic review of MEDLINE and EMBASE was completed. The search strategy was developed to identify measurement property studies for PROMs of interest (selected pain, disease activity, global evaluation and quality of life scales) in patients with RA. We identified experim

The OPTIMIZE study: protocol of a pragmatic sequential multiple assessment randomized trial of nonpharmacologic treatment for chronic, nonspecific low back pain.

BACKGROUND: Low back pain is a prevalent condition that causes a substantial health burden. Despite intensive and expensive clinical efforts, its prevalence is growing. Nonpharmacologic treatments are effective at improving pain-related outcomes; however, treatment effect sizes are often modest. Physical therapy (PT) and cognitive behavioral therapy (CBT) have the most consistent evidence of effectiveness. Growing evidence also supports mindfulness-based approaches. Discussions with providers and patients highlight the importance of discussing and trying options to find the treatment that work

Combined transcutaneous electrical nerve stimulation and low-level laser photobiomodulation for postoperative pain after cesarean section: A randomized controlled trial.

INTRODUCTION: Postoperative pain after cesarean section may delay recovery and impair function. Transcutaneous electrical nerve stimulation (TENS) and low-level laser photobiomodulation (PBM) are used for pain control, but the additional effect of combining these therapies remains unclear. This study investigated this effect. METHODS: A randomized clinical trial with four groups: combined TENS + low-level laser photobiomodulation (PBM), TENS alone, placebo, and control. Assessments were performed at 8-12 h (t1), 20-24 h (t2), and 44-48 h postpartum (t3). Outcomes included pain intensity, func

Dual benefit of esketamine-based opioid-free anesthesia on postoperative pain and mental health outcomes in patients undergoing bariatric surgery: a randomized controlled trial.

Patients with morbid obesity are at a high risk of comorbid depressive and anxiety symptoms. While bariatric surgery is an effective treatment, conventional opioid-based anesthesia carries significant perioperative risks. Opioid-free anesthesia (OFA) presents a safer alternative. Esketamine, known for its dual analgesic and rapid antidepressant effects, is a promising component of OFA. However, clinical evidence supporting its simultaneous benefits for both physical and psychological recovery in bariatric patients remains limited. In this double-blind, randomized clinical trial, patients sched

The Effects of Invasive Motor Cortex Stimulation for Neuropathic Pain: Do We See the Full Picture?

INTRODUCTION: Invasive motor cortex stimulation (iMCS) can be a last-resort treatment of chronic neuropathic pain syndromes. There is evidence that iMCS was found to influence pain intensity, quality of life scores, and pain medication intake in clinical practice. Albeit, qualitative studies published over the last years showed that the expectations and experiences of patients have a significant impact on treatment outcomes. This paper focus on these experiences of chronic neuropathic pain patients treated with iMCS. OBJECTIVES: The aim of this interview study was to map the experiences of pa

Effectiveness and safety of nurse-led different epidural analgesia methods based on symptom management theory in patients with severe acute pancreatitis: A single-center RCT protocol.

BACKGROUND: Patients with severe acute pancreatitis (SAP) often experience intense pain that significantly impacts their quality of life and may hinder recovery. Current pain management approaches have limitations. Although continuous thoracic epidural analgesia (TEA) is generally effective, some patients still need additional intermittent injections of local anesthetics for sufficient pain relief. In the ICU, nurses are the primary providers responsible for pain management. Symptom Management Theory (SMT), though used in various fields, has not yet been fully validated for nurse-led pain mana

Modified Single-Side-Lying Quadratus Lumborum Block Combined with Liposomal Bupivacaine versus Transversus Abdominis Plane Block for Post-Cesarean Analgesia: A Randomized Controlled Trial.

BACKGROUND: Inadequate post-cesarean analgesia remains a common clinical issue, associated with increased opioid use and risks of chronic pain and postpartum depression. Although the anterior quadratus lumborum block (QLB III) is more effective than transversus abdominis plane (TAP) block, it requires repeated positioning and may cause quadriceps numbness. To address these limitations, we designed a modified QLB to overcome these drawbacks. This study evaluated its analgesic efficacy and safety when combined with liposomal bupivacaine in cesarean delivery. METHODS: This single-center, randomi

Intravenous Ketamine reduces psychological pain in suicidal inpatients: A secondary analysis of a six-week randomized placebo-controlled trial.

BACKGROUND: Psychological pain has been conceptualized as an unbearable inner experience and a possible precondition for suicidal ideation. No randomized placebo-controlled study has specifically investigated psychological pain as a treatment target in psychiatric populations. We examined ketamine's effect on psychological pain in suicidal psychiatric inpatients. METHODS: This secondary analysis used KETIS, a 6-week randomized, double-blind, placebo-controlled trial. Participants received two 40-minute intravenous infusions of ketamine (0.5 mg/kg) or placebo (0.9% saline) 24 h apart, alongsid

Audio-visual stimulation therapy for chronic neuropathic pain: a sham-controlled randomized clinical trial.

BACKGROUND: Neuropathic Pain (NP) affects 10% of the general population, decreasing quality of life for millions of Americans and contributing to higher physical and mental health care costs. The most widely used treatments for NP involve medications that show limitations in efficacy and burdensome side effects. METHODS: This randomized controlled trial explored the efficacy of a wearable Audio-Visual Stimulation neuromodulation device (Sana) as a novel intervention for chronic NP in 75 participants. Outcomes were assessed at baseline, after 8-weeks of daily use of the assigned Sana or Sham de
