In the frame of multiple injuries we had 30% of the fractures of the femur. We treated 113 patients with polytraumas with fractures of the femur and with the plate. We had 25% AIS grade 1, 48% AIS grade 2, 21% AIS grade 3 and 6% AIS grade 4. Mortality was 4%. Indications for osteosynthesis with plate are: severe head injuries, fractures of the pelvis, open fractures, injuries of the nerves and blood vessels and severe trauma of the abdomen.

A 69-year-old Japanese woman with erythema, severe edema on the face, Gottron's papules and poikiloderma was diagnosed as having dermatomyositis. She also noticed muscle weakness in her extremities, although her electromyogram showed neurogenic patterns. Her levels of CA19-9 and CEA were elevated and a CT of her abdomen revealed a giant, multilobular, cystic lesion in the pelvis. This tumor was diagnosed as pseudomyxoma peritonei originating from a mucinous adenocarcinoma of the appendix.

Osteochondromas arising from the long bones, pelvis, or scapulae are common. However, osteochondromas originating from the base of the skull are extremely unusual. Although these tumors are histologically benign, intracranial extension and their close proximity to the cranial nerves may require complex surgery. We present a case in which CT clearly delineated an osteochondroma arising from the base of the skull. The tumor had created a fusion with the ipsilateral coronoid process of the mandible, thus causing lockjaw.

According to their different etiology, parapelvic cysts must be differentiated from simple renal cysts, pararenal pseudocysts, intramural renal pelvis cysts and suprarenal pseudocysts. An important criterium of parapelvic cysts is their epithelial lining. According to their radiological features, these cysts must be described as "parapelvically located cysts" until the definite histological diagnosis is made by the pathologist. The exact differentiation between a parapelvic cyst and a pararenal pseudocyst may be of forensic importance in posttraumatic patients.

Complex injuries to the hand may cause considerable problems in the aftertreatment. Two possibilities are described, which may facilitate and optimize the postoperative care of the severely injured hand: 1. In case of an inguinal pedicle flap, the temporary stabilization of the hand against the pelvis by means of external fixation is advocated. 2. After multiple extensor tendon repair, the temporary immobilization of the wrist, leaving the fingers free to move, by means of a small fixateur externe is suggested.

A prospective randomized trial compared cisplatin alone versus cisplatin, doxorubicin and cyclophosphamide in the treatment of advanced carcinoma of the bladder, ureter and renal pelvis. Objective responses were noted in 7 of 45 patients (16 per cent) given cisplatin alone and in 9 of 42 (21 per cent) treated with the combination. There was no significant difference in response rate, response duration or survival but the combination was more toxic. Other combination regimens should be evaluated.

We investigated 24 patients with blunt renal trauma by excretory urography plus nephrotomography and ultrasound. The results of these investigations were correlated with the operative findings. The ultrasound diagnosis was correct in 16 of 18 patients with parenchymal injuries. Injuries to the renal pelvis, ureter and renal pedicle were suspected only in connection with excretory urography. Ultrasound provides an excellent inexpensive noninvasive diagnostic tool for the initial diagnostic study of patients with blunt renal trauma.

A previously well 3 year old Asian girl presented as an emergency with the acute onset of generalized abdominal pain. On examination her abdomen was distended with generalized tenderness and guarding most marked centrally. Under anaesthesia a central abdominal mass arising from the pelvis was palpated which on proceeding to laparotomy was found to be a blood-filled urachal cyst. This was excised and her recovery was uneventful. Subsequent investigations have revealed no associated renal tract abnormalities.

The term sternocostoclavicular hyperostosis groups affections of different pathogenicity. These result from an ossifying enthesiopathy, either inflammatory, isolated to the thoracic wall anteriorly or combined with a spondylarthropathy, particularly ankylosing spondylitis, or degenerative, the anterior thoracic hyperostosis usually being part of an ensheathing vertebral hyperostosis. A differential diagnosis is anterior thoracic hyperostosis due to an inflammatory osteopathy occurring in young patients and often associated with other bone lesions of pelvis and spine or long bones.

Complications of Crohn's disease were demonstrated by computed tomography of the abdomen and pelvis in 7 patients. Four patients had developed abscesses, 2 located in the psoas region and 2 involving the retroperitoneum or the liver. Other abnormalities documented on CT included inflammatory masses, fistulae, carcinoma, mesenteric thickening, and extraintestinal complications such as gallstones, renal calculi, and sacroileitis. Computed tomography was found to be useful in the detection and follow-up of abdominal complications of Crohn's disease.

One-thousand computed tomograms of the abdomen, chest, and pelvis in oncologic patients were reviewed to determine the incidence of serendipitous findings and their impact on clinical management. The incidence of serendipitous findings was 30% in thorax, 24% in both genitourinary and osseous structures, 23% in the abdomen, and 21% in the retroperitoneum. These serendipitous abnormalities changed clinical management or required further diagnostic procedures (thus, they were considered significant) in less than 1% of the abdominal to 12% in osseous findings.

We herein describe 9 patients with non-opaque calculi and 3 patients with soft tissue filling defects in the renal pelvis studied by ultrasound. The calculi varied in size from 1.5 cm. to staghorn calculi. All stones were imaged satisfactorily by ultrasound, allowing the diagnosis of stone to be made with confidence. Ultrasonic differentiation of renal pelvic filling defects proved to be a simple, reliable, non-invasive method to differentiate these clinically perplexing lesions.

In a series of 49 consecutive cases of Wilms' tumor from a single institution, there was a 20% incidence of nonvisualization on the initial intravenous pyelogram. Even though nonvisualization was secondary to either gross or microscopic invasion of the renal vein or renal pelvis, this did not forecast a grim progosis. Seven of the nine patients with nonvisualization are presently alive and tumor-free at least 5 yr after resection. The only two deaths in this particular group were unrelated to the Wilms' tumor.

Guided by ultrasound percutaneous antegrade pyelography can be accomplished with great ease in patients with obstructive hydronephrosis. The exact location and the depth of the dilated renal pelvis can be determined accurately with the use of ultrasound and the procedure is similar to that of a cyst puncture. Anomalies of the female genital tract were successfully characterized by gray scale ultrasonography in three cases; all three were found in association with unilateral renal agenesis. Findings at intravenous urography prompted ultrasound studies. The high incidence of genital tract anomalies associated with unilateral renal agenesis, particularly in females, indicates ultrasonography be used to examine the pelvis (as well as the renal bed). Further diagnostic procedures can then be assessed on a more informed basis.

Vaginal delivery of a fetus in breech position carries a risk three times that of vertex delivery. Patients must be carefully selected for vaginal delivery. The position should be that of a frank breech; the child should not be excessive in size; the mother's pelvis should be normal, and there should be no other complications of pregnancy. The fetal heart rate should be carefully monitored throughout labor. When patients with breech position are carefully studied, about two-thirds will be delivered by cesarean section.

This report describes a human embryo with a length of 12 mm (horizon XVII Streeterm 35 +/- 1 day old), in which the following malformations appear: Dilation on both sides of the Wolffian ducts, and the more caudal mesonephritic glomerules (mesohydronephrosis). Dilatation of both ureters and renal pelvis (uretero-hydronephrosis). These malformations are the result of the accumulation of liquid secreted by the mesonephros in a cloaca which is dilated, unwalled and blocked by an epithelial plug which is continuous with the cloacal membrane.
