A concise practical clinical guide to identifying spasticity in neurological shoulder dysfunction.

BACKGROUND: Spasticity is a known complication to the hemiplegic shoulder following acquired brain injury. However, there is a current discrepancy between the diagnosis of arm spasticity and the subsequent assessment and treatment of spasticity in people with shoulder involvement. OBJECTIVE: An expert panel aimed to establish a consensus and provide a simple structured approach to identifying and assessing people with spasticity of the hemiplegic shoulder. METHODS: A consensus group of six spasticity experts were interviewed individually via 1-h video calls to ascertain how they assess peopl

Randomized Observer-Blind Study on the Effects of Neuromuscolar Taping in Parkinson's Disease Patients.

BACKGROUND: Abnormal posture occurs in about 30% of Parkinson's disease (PD) patients. The neuromuscular taping (NMT) is a new treatment that induces micromovements that stimulate skin receptors. Application of NMT with a decompression and eccentric technique expands the interstitial spaces and therefore improves circulation and absorption of liquids by reducing the pressure under the skin favoring the muscular relaxation. OBJECTIVES: We conducted a 4-week, randomized, observer-blind, trial of NMT on pharmacologically treated PD patients with postural abnormality according to the item 3.13 of

Association of occupational testicular radiation exposure with offspring sex ratio.

OBJECTIVES: The association between paternal occupational radiation exposure and the sex ratio of offspring remains controversial. Therefore, we aimed to explore testicular radiation exposure in spine surgeons and its association with their offspring's sex ratio. METHODS: The Minimally Invasive Spine Treatment Society in Japan administered a survey in October 2020. Male surgeons were divided into high- and low-radiation exposure groups according to the type of lead protectors used during the time frame surrounding the child's conception. Apron-type lead protectors were associated with high ex

Effects of combined jaw and cervicoscapular exercises on mouth opening and muscle properties in cervical extension type.

Prolonged smartphone use can lead to cervical posture deformities, with cervical extension type being a common condition characterized by increased cervical lordosis, forward head posture, and thoracic kyphosis. These changes may contribute to neck pain, restricted cervical range of motion (ROM), and increased muscle tone. Additionally, cervical extension type is linked to temporomandibular joint (TMJ) dysfunction, affecting mandibular movement and muscle activity. Given the biomechanical connection between the cervical spine and TMJ, addressing cervical dysfunction may benefit TMJ related con

Right-Predominant Pulmonary Edema After Complete Avalanche Burial: Two Cases and Mechanistic Considerations.

Avalanche burial occasionally precipitates pulmonary edema, yet its pathogenesis remains poorly understood. Two male back-country skiers (aged 39 and 53 years) were completely buried for 30 min and 20 min, respectively, by the same avalanche, and immobilized in a right-lateral posture without safety devices. They arrived 2 h post-extrication, conscious but hypothermic (32.4 °C and 34.2 °C, respectively), tachycardic, tachypneic, and hypoxemic (SpO2 88% and 83%, respectively). Chest radiography and computed tomography demonstrated extensive infiltrates or ground-glass opacities confined predomi

Standing middle cerebral artery velocity predicts cognitive function and gait speed in older adults with cognitive impairment, and is impacted by sex differences.

Upright posture challenges the cerebrovascular system, leading to changes in middle cerebral artery velocity (MCAv) dynamics which are less evident at supine rest. Chronic alterations in MCAv have been linked to hypoperfusion states and the effect that this may have on cognition remains unclear. This study aimed to determine if MCAv and oscillatory metrics of MCAv (ex. pulsatility index, PI) during upright posture are i) associated with cognitive function and gait speed (GS) to a greater extent than during supine rest, and ii) are different between sexes. Beat-by-beat MCAv (transcranial Dopple

IMU Data-Driven and PCA-Based Approach to Establish Quantifiable and Practically Applicable Measures for V2 Technique Elements in Cross-Country Skiing.

Quantifying movement coordination in cross-country (XC) skiing, specifically the technique with its elemental forms, is challenging. Particularly, this applies when trying to establish a bidirectional transfer between scientific theory and practical experts' knowledge as expressed, for example, in ski instruction curricula. The objective of this study was to translate 14 curricula-informed distinct elements of the V2 ski-skating technique (horizontal and vertical posture, lateral tilt, head position, upper body rotation, arm swing, shoulder abduction, elbow flexion, hand and leg distance, plan

Comparison of Modified Mallampati Classification in Supine and Sitting Positions to Predict Difficult Tracheal Intubation in Diabetic Patients.

BACKGROUND: Airway management of patients with long-standing diabetes poses a major challenge for anaesthesiologists due to stiff joint syndrome affecting the atlanto-occipital joint. In certain cases requiring immobilization, the Mallampati test must often be performed in the supine position for obvious reasons. OBJECTIVES: Hence, we determined the diagnostic precision (sensitivity and specificity) of the modified Mallampati test in sitting and supine positions among the diabetic population in predicting difficult tracheal intubation. METHODS: A single-center prospective observational study

Assessing the Preservation of Lumbar Lordotic Curvature in Everyday Sitting Conditions Assessed with an Inertial Measurement System.

Background/Objectives: Lumbar lordotic curvature (LLC), closely associated with low back pain (LBP) when decreased, is infrequently assessed in clinical settings due to the spatiotemporal limitations of radiographic methods. To overcome these constraints, this study used an inertial measurement system to compare the magnitude and maintenance of LLC across various sitting conditions, categorized into three aspects: verbal instructions, chair type, and desk task types. Methods: Twenty-nine healthy participants were instructed to sit for 3 min with two wireless sensors placed on the 12th thoracic

Differences in spinal axial rotation angles during the lumbar-locked rotation test between hyper and normal thoracic rotation groups: A cross-sectional study.

BACKGROUND: Trunk rotation is important in many sporting activities The thoracic spine has reciprocal relationships with the lumbar and pelvic spines, such that reduced flexibility in the lumbar or thoracic spine can lead to abnormal patterns of trunk movement and pain. However, few studies have investigated the relative trunk rotation mobilities of the thorax, lumbar, and pelvis. OBJECTIVE: To compare thoracic, lumbar, and pelvic rotation angles during the lumbar-locked rotation test between hyper and normal thoracic rotation groups. METHODS: Thirty-two young, active participants were enrol

Presumed Fourth Nerve Palsy in a Healthy and Asymptomatic Child with COVID-19 Infection.

COVID-19 can cause a wide range of ocular manifestations. The most common ocular manifestation is conjunctivitis. Neuro-ophthalmic presentations of COVID-19 are rare. Case reports suggest that COVID-19 infection can cause cranial nerve palsy, including nerves that regulate ocular movements. The present studypresented a case of fourth nerve palsy in a healthy and asymptomatic COVID-19-infected child. A healthy 10-year-old boy was referred to our eye clinic with a complaint of recent abnormal head posture and squint. His past medical history was unremarkable, and he had not received any medicati

An ergonomic focus evaluation of work-related musculoskeletal disorders amongst operators in the UAE network control centres.

Work-related musculoskeletal disorders (WMSDs) have rapidly increased during the last decade, but only a few descriptive surveys have been conducted in the United Arab Emirates (UAE). This study investigated the prevalence of WMSDs and analysed their ergonomic risks amongst operators in the network control rooms across two government organisations, X and Y, in the UAE. Essential data were collected by the Nordic Musculoskeletal Questionnaire (NMQ) and the Maastricht Upper Extremity Questionnaire (MUEQ) from online surveys and direct observations based on the Rapid Office Strain Assessment (ROS
