6-Mercaptopurine metabolic profiles and clinical outcomes in TPMT and NUDT15 phenotypes during maintenance therapy for Thai paediatric acute lymphoblastic leukaemia.

AIM: This study aimed to investigate the real-world profiles of 6-mercaptopurine (6-MP) treatment in various TPMT/NUDT15 phenotype groups among paediatric patients with ALL. METHODS: We conducted a prospective cohort study of 104 paediatric patients with ALL during the maintenance phase, collecting baseline data and following up monthly for 5 months. Patients were genotyped and classified as normal (NM), intermediate (IM) or poor (PM) metabolizers based on the TPMT and NUDT15 genetic variations. Additionally, TPMT activity and metabolite concentrations (6-thioguanine nucleotides; 6-TGN and 6-

Derivation and external validation of a venous thromboembolism risk prediction model in asparaginase-treated ALL.

The incidence of venous thromboembolism (VTE) in patients with acute lymphoblastic leukemia (ALL) receiving asparaginase-based induction is high despite primary thromboprophylaxis. Our aim was to derive and externally validate a VTE risk prediction model in patients with ALL receiving asparaginase-based induction. We conducted a multicenter retrospective cohort study of patients (aged ≥18 years) with newly diagnosed ALL receiving asparaginase-based induction. The derivation and external validation cohorts included 306 and 94 patients, respectively. Primary outcome was VTE at any site. A cause-

[Case of brexu-cel at first relapse in an elderly patient].

INTRODUCTION: Relapsed or refractory B-cell acute lymphoblastic leukemia (R/R B-ALL) represents a complex clinical scenario, and the advent of immunotherapy has radically changed the therapeutic options available for these patients. The introduction of CAR-T cell therapy for patients over 25 years old offers a new treatment opportunity, with high remission rates and durable responses. CLINICAL CASE: A 67-year-old female patient with multiple comorbidities was diagnosed with Philadelphia-negative B-ALL and treated with induction therapy according to the GIMEMA LAL1913 protocol plus rituximab w

Philadelphia chromosome-negative but BCR::ABL1-positive acute lymphoblastic leukemia: a real-world multicenter cohort study.

Chronic myelogenous leukemia that carries the BCR::ABL1 fusion gene without cytogenetically detectable Philadelphia (Ph) chromosomes is termed masked Ph. However, the prevalence and clinical relevance of masked Ph in acute lymphoblastic leukemia (ALL) remain unclear. Using the Hokkaido Leukemia Net real-world multicenter cohort, we analyzed 160 B-cell ALL patients diagnosed between 2017 and 2024 with available cytogenetic and molecular data. Among 92 BCR::ABL1-positive cases, 19 (20.7%) lacked a cytogenetically visible Ph chromosome and were classified as masked-Ph ALL. Compared with Ph + ALL,

Survival disparities in Philadelphia chromosome-positive vs. Philadelphia chromosome-negative B-cell acute lymphoblastic leukemia in the era of modern therapeutic approaches: a decade-long surveillance, epidemiology, and end results (SEER) data based investigation (2010-2021).

Philadelphia chromosome (Ph) status is a critical prognostic marker in B-cell acute lymphoblastic leukemia (B-ALL). This study evaluates the impact of Ph-positive (Ph+) and Ph-negative (Ph-) status on overall survival (OS) and cancer-specific survival (CSS) while analyzing the role of demographic and treatment variables. A retrospective cohort study involving 14,175 patients diagnosed with B-ALL from 2010 to 2021 was analyzed in this investigation. Primary outcomes were OS and CSS, analyzed using hazard ratios (HRs) with 95% confidence intervals (CIs) and associated p-values. Subgroup analysis

Genomic profiling of Mexican patients with B-cell precursor acute lymphoblastic leukemia reveals clinically significant somatic and potential germline variants.

B-cell precursor acute lymphoblastic leukemia (preB-ALL) is characterized by pathogenic variants currently used in precision oncology. However, the mutational landscape of Mexican children with preB-ALL has not yet been thoroughly explored and defined in terms of the clinical significance. We used a custom-designed next-generation sequencing exome panel, along with high-resolution chromosome microarrays and gene fusion-targeted assays, to characterize the mutational landscape of 73 Mexican children with preB-ALL, exploring the profile of clinically significant variants. We classified the varia

Air pollution and mortality in acute lymphoblastic leukemia patients across different age groups in Northern Thailand (1999-2020).

This study investigated the impact of air pollution on 604 patients diagnosed with acute lymphoblastic leukemia (ALL) between 1999 and 2020 in upper northern Thailand. Patients were categorized into three age groups: pediatric (<15 years), adolescent and young adult (AYA) (15-39 years), and adult (≥40 years). The cohort included 344 (57.0%) pediatric, 152 (25.1%) AYA, and 108 (17.9%) adult patients, with 5-year survival rates of 64.5%, 19.6%, and 6.7%, respectively. Concentrations of PM2.5, PM10, O₃, and CO were significantly higher in patients who died, particularly among pediatric cases. Amo

Results of Third-Line Therapy in Philadelphia Chromosome-Positive Adult Acute Lymphoblastic Leukemia.

BACKGROUND: The historical results in adults with Philadelphia chromosome (Ph)-positive acute lymphoblastic leukemia (ALL) in second relapse treated with third-line therapy (Salvage 2) are extremely poor. Using standard chemotherapy resulted in complete response rates of 20% and a median overall survival of 2 to 3 months. This may be changing with the recent availability of novel therapeutic strategies, including targeted antibodies and engineered chimeric antigen receptor T-cell therapy, as well as more potent BCR::ABL1 tyrosine kinase inhibitors. STUDY AIMS: Analyze the outcome of adults wi

Clinical Implications of CD19-Negative Relapse Following CD19-Directed Therapy in B-Cell Acute Lymphoblastic Leukemia.

CD19-directed therapy remains the mainstay treatment for relapsed/refractory B-cell acute lymphoblastic leukemia (ALL). However, treatment patterns and outcomes following CD19-negative (CD19-) relapse remain poorly defined. We retrospectively analyzed 65 adult patients with ALL who developed CD19-relapse after CD19-directed therapy. TP53 mutations and BCR::ABL1-like ALL were each identified in 27.7% (n = 18) of patients. Forty-six patients (70.8%) received one CD19-targeted therapy, whereas 19 patients (29.2%) received ≥ 2 prior to CD19-relapse. Overall, 60 patients (92.3%) received blinatumom

Wearable Sleep Monitoring in Pediatric Acute Lymphoblastic Leukemia: Associations With Subjective Sleep Ratings and Neurocognitive Functioning.

BACKGROUND: Sleep disturbances are associated with increased fatigue, reduced quality of life, and neurocognitive dysfunction and have emerged as a common complication among pediatric cancer survivors. Sleep disturbances are particularly concerning given their potential to exacerbate existing neurocognitive impacts of cancer treatments. This pilot study examined the feasibility and acceptability of a home-wearable EEG-based sleep device (Sleep ProfilerTM) for acute lymphoblastic leukemia (ALL) survivors as well as associations between specific sleep parameters and neurocognitive functioning.

Sudden unexpected death due to B-cell acute lymphocytic leukaemia (B-ALL) in a 3-year-old child.

Sudden unexpected death due to haematologic disease in children is unusual. A 3-year-old girl had been ill for several weeks, but her parents did not consult a doctor. Her complaints included a cough, fever, vomiting and fatigue. On the day of her death, the girl fell from the toilet and was unresponsive. In hospital, severe dyspnoea and a haemoglobin value of only 1.7 mg/dl were diagnosed, as well as hypothermia and haemorrhaging from mouth and nose. Despite intensive medical care and cardiopulmonary resuscitation, the girl died. Main autopsy findings were severe anaemia and the underlying di

Impact of age and sex on survival outcomes in patients aged 1-45 years with acute lymphoblastic leukemia treated according to the stratification used in the NOPHO ALL2008 trial.

Age and sex have historically been associated with differences in the survival of patients with acute lymphoblastic leukemia (ALL). The NOPHO ALL2008 trial included patients aged 1-45 years with BCR::ABL1-negative B-precursor and T-cell ALL, but neither sex nor age was integrated into risk group allocation. Among 1,771 trial patients stratified into protocol- appropriate risk groups, we estimated the impact of age and sex on survival (even after relapse) and toxicities prospectively registered at 3-month intervals. In multivariate Cox regression analysis adjusted by sex, age group, and risk gr

Incorporation of Immunotherapy Into Adult B-Cell Acute Lymphoblastic Leukemia Therapy.

Blinatumomab and inotuzumab ozogamicin have demonstrated efficacy in treating relapsed/refractory B-cell acute lymphoblastic leukemia (B-ALL) and improving outcomes compared with conventional chemotherapy. Encouraging results have been observed in both younger and older patients with Philadelphia chromosome (Ph)-positive and Ph-negative B-ALL treated with these immunotherapy agents across several clinical trials. Treatment with inotuzumab ozogamicin and/or blinatumomab leads to high rates of deep measurable residual disease negativity and may enhance survival compared with chemotherapy-only ap

Prolonged COVID-19 infection in a patient with B-cell acute lymphoblastic leukemia maintained on convalescent plasma until recovery with monoclonal antibodies.

INTRODUCTION: Patients with hematologic malignancies who undergo immunosuppressive therapies such as chimeric antigen receptor T-cell (CAR-T) therapy are at high risk of prolonged SARS-CoV-2 infection due to impaired humoral immunity. Treatment options remain limited, with variable efficacy, in such settings. METHODS: We describe a 21-year-old man with Down syndrome and B-cell acute lymphoblastic leukemia complicated by B-cell aplasia following CD19-directed CAR-T therapy. The patient developed COVID-19 and experienced persistent symptomatic infection, with high viral load and prolonged rever
